Wideband Acoustic Reflex Growth in Adults With Cystic Fibrosis.

Purpose Individuals with cystic fibrosis (CF) are often treated with intravenous (IV) aminoglycoside (AG) antibiotics to manage life-threatening bacterial infections. Preclinical animal data suggest that, in addition to damaging cochlear hair cells, this class of antibiotics may cause cochlear synaptopathy and/or damage to higher auditory structures. The acoustic reflex growth function (ARGF) is a noninvasive, objective measure of neural function in the auditory system. A shallow ARGF (small reflex-induced changes in middle ear function with increasing elicitor level) has been associated with

Neuronal circuits involved in the middle-ear acoustic reflex.

Human and animal studies have shown that certain aromatic solvents such as toluene can cause hearing loss and can exacerbate the effects of noise. The latter effects might be due to a modification of responses of motoneurons controlling the middle-ear acoustic reflex. In the present investigation, the audition of Long-Evans rats was evaluated by measuring cubic (2f1 - f2) distortion otoacoustic emissions (f1 = 8000 Hz; f2 = 9600 Hz; f1/f2 = 1.2) prior to, during, and after activation of the middle-ear acoustic reflex. A noise suppressor was used to modify the amplitude of the 2f1 - f2 distorti

Stapedial reflex and ears with high static acoustic admittance.

PURPOSE: To evaluate modified acoustic reflex diagnostic protocols for a group of individuals (n = 9) with high peak compensated static acoustic admittance (Y(tm)) tympanograms. METHOD: A modified procedure designed to improve acoustic stapedius reflex threshold (ASRT) measurements in individuals with high-admittance tympanograms was employed in both an experimental and a control group. ASRTs were measured at 0.5, 1.0, and 2.0 kHz, ipsilateral and contralateral. Measurements were obtained within each condition for 7 ear canal pressures that were set to 0, +/-50, +/-100, and +/-150 daPa (relat

Acoustic reflex thresholds at varying click rates in children.

OBJECTIVE: Previous studies conducted with young adults have shown that ipsilateral acoustic reflex thresholds improve with an increase in the click-rate. Older adults also show an improvement in acoustic reflex thresholds with an increase in the click-rate. However, the click-rate induced facilitation (RIF) of acoustic reflex thresholds is significantly reduced in older adults. These results suggest that slower temporal processing in older adults can be documented by measuring the click-RIF of acoustic reflex thresholds. Thus, the click-RIF of acoustic reflex thresholds may prove to be a valu

Acoustic reflex threshold tenth percentiles and functional hearing impairment.

This study addressed the issue of whether functional impairments can be identified or confirmed using tonal acoustic reflex thresholds (ARTs). Tonal ARTs from 74 ears with functional impairments were compared to 10th percentiles of the ARTs for normal and cochlear-impaired ears at 500 to 2000 Hz (Gelfand, Schwander, and Silman, 1990). Only one ear (5.3%) was correctly identified among the 19 ears in which all of the voluntary thresholds at 500 to 2000 Hz were < or = 55 dB HL. Acoustic reflex thresholds correctly identified functional losses in 70.9 percent, 78.6 percent, and 85.7 percent of th

The acoustic middle ear muscle reflex in albino rats.

The acoustic middle ear muscle reflex was studied in albino rats anesthetized with chloralose. The best frequency of the reflex and the threshold at this frequency were on average about 3 kHz and 57 dB SPL, respectively. The threshold increased as frequency increased above, and decreased below, the best frequency at a rate of about 20 dB/octave. Above about 12 kHz, the muscular response showed instability and habituation. Thresholds were similar between stapedius and tensor tympani reflexes and between ipsilateral and contralateral reflexes. The middle ear transmission loss due to the reflex w

The human acoustic tensor tympani reflex. A case report.

In a patient with unilateral lower motoneuron facial paralysis secondary to Bell's palsy, we evaluated the characteristics of what we believe to be an acoustically-evoked reflex contraction of the tensor tympani muscle. Ipsilateral stimulation of the involved ear yielded a pattern of impedance change different from the pattern characterizing normal stapedius muscle contraction. Threshold of the reflex was elevated, amplitude was large and variable, onset latency was greater, onset rise was more gradual, and offset decay was more rapid. Contralateral stimulation, with probe in the involved ear,

Normal values of the ipsilateral acoustic stapedius reflex threshold.

It is frequently taken for granted that the acoustically evoked stapedius reflex is bilaterally symmetrical. Contrary to this, Møller described an asymmetry of the acoustic stapedius reflex with an ipsilaterally 2--14 dB lower threshold. The determination of normal values of the ipsilateral acoustic stapedius reflex threshold with a large number of patients is difficult as the intensity of the stimulus depends considerably on the position of the probe in the acoustic meatus and is therefore not defined with sufficient accuracy. For this reason we determined the values of the ipsilateral thresh

Acoustic reflex measures of cochlear damage--a normative study.

It has been argued that certain reflex measures may indicate the presence of end-organ hearing loss. It has been noticed that the difference between wide band noise and pure tone thresholds is diminished, that the reflex threshold duration function is flattened and that the reflex relaxation index, as specified by Norris et al. (1974b), is reduced in cases of end-organ hearing loss. These aspects of the acoustic stapedial reflex, together with its growth with stimulus intensity, have been investigated in 25 normally hearing subjects. The overriding feature of the measures taken of the acoustic

Detection of the acoustic reflex below 80 dB HL.

A new method for detecting the acoustic reflex that utilizes standard otoacoustic emissions recording techniques is introduced and discussed. Two successive identical tone bursts of 100 ms duration and 10 ms interstimulus interval are presented in the occluded ear canal at a repetition rate of one per second. If the acoustic reflex is elicited, the contraction of the stapedius muscle is delayed with respect to the onset of the first stimulus. Hence, the acoustic compliance in the ear canal decreases primarily during the second stimulus. The difference of the microphone signals produced by the

Calibration of ipsilateral stimulus transducer for acoustic reflex measurements.

Pure-tone Reference Equivalent Threshold Sound Pressure Level (RETSPL) of the ipsilateral stimulus receiver for acoustic reflex measurements on Madsen Electronics type Zodiac 901 impedance audiometer is provided. The results, obtained from 20 normal-hearing subjects, are achieved by comparing hearing threshold levels measured using a TDH 39 telephone (calibrated to ISO 389) with thresholds recorded using the ipsilateral stimulus insert phone. The calibration is referenced to an IEC-711 ear simulator and comprises the following frequencies: 125, 250, 500, 750, 1000, 1500, 2000, 3000, 4000, 6000

Acoustic reflex in patients with cochlear implants (analog stimulation).

Stapedius reflexes were tested in 21 deaf patients with the Vienna cochlear implant using intra- or extracochlear electrodes. Sinusoidal bursts of 125, 500, 1,000 and 2,000 Hz were used as stimuli presented in a randomized sequence. The contralateral acoustic reflex was monitored by synchronized sampling of the acoustic impedance of the ear. Results obtained were compared to psychoacoustic sensation values. The reflex threshold always was located close to the uncomfortable loudness level within the subjective dynamic range, or at least above the most comfortable loudness level. Practical appli

Acoustic reflex crossover artifacts in infants and young children.

Crossover artifacts occur when the probe microphone of an impedance instrument directly responds to vibratory-acoustic energy that crosses the skull from the stimulus transducer on the contralateral ear. The resulting meter deflections are subjectively similar to and may be interpreted as actual acoustic reflexes (ARs) by unsuspecting examiners. In this study, infants and young children aged 10 days to 6 years 9 months were fit with earplugs to eliminate the AR and were examined for artifact incidence and threshold. Results indicate a high rate of occurrence of the crossover artifact in the yo

Acoustic reflex and loudness discomfort in acute facial paralysis.

An extensive research protocol was used to evaluate the conditions of 48 consecutive patients with acute facial paralysis. The results indicated that after nerve excitability testing, the acoustic reflex was the most efficient indicator of impending nerve degeneration and predictor of recovery. The presence of increased sensitivity to intense acoustic stimuli (loudness discomfort level) indicated poor prognosis. The data demonstrate that dysacusis (hyperacusis) is not related to stapedial muscle paralysis and also question the validity of "topographic diagnosis" in determination of the site of

The acoustic reflex and temporary threshold shift: temporal characteristics.

One ear of each of seven normal-hearing subjects was exposed to a continuous 1000-Hz tone at 110 dB SPL for three minutes. During exposure, a broad-band noise at 100 dB SPL was presented to the contralateral ear. The noise was either continuous or pulsed. Four pulsed conditions employed repetition periods of 360, 180, 90, or 9 msec with a 50% duty cycle. A control condition in which no noise was presented was also included. Temporary threshold shift was measured at selected postexposure times at the frequency one-half octave above the exposure frequency. TTS2 was greatest for the control condi

Acoustic reflex test in neuro-otologic diagnosis. A review of 24 cases of acoustic tumors.

Acoustic and other tumors of the cerebellopontine angle are not uncommon, and they usually have otologic symptoms or findings. Early diagnosis depends on a high index of suspicion and a thorough evaluation of patients with suspected tumors. This is a report of 24 cases selected from a group of 184 surgically confirmed tumors. Had the cases been evaluated only on the basis of pure tone or speech audiometry or on the basis of plain roentgenograms, the diagnosis might not have been made. Audiological examination should include the acoustic reflex test. The results of this test indicated a retroco
