External Cephalic Version: ACOG Practice Bulletin Summary, Number 221.

In the United States, there is a widespread belief that the overall cesarean birth rate is higher than necessary. Efforts are being directed toward decreasing the number of these procedures, in part by encouraging physicians to make changes in their management practices. Because breech presentations are associated with a high rate of cesarean birth, there is renewed interest in techniques such as external cephalic version (ECV) and vaginal breech delivery. The purpose of this document is to provide information about ECV by summarizing the relevant evidence presented in published studies and to

External Cephalic Version: Is it an Effective and Safe Procedure?

External cephalic version (ECV) is an effective procedure for reducing the number of cesarean sections. To date, there is no video publication showing the methodology of this procedure. The main objective is to show how to perform ECV with a specific protocol with tocolysis before the procedure and analgesia. Moreover, we describe and analyze the factors associated with successful ECV, and also compare to deliveries in the general pregnant population. A retrospective and descriptive analysis of ECV carried out at the Hospital Clinico Universitario Virgen de la Arrixaca in Murcia (Spain) betwee

Development and validation of a clinical score to predict the probability of successful procedure in women undergoing external cephalic version.

INTRODUCTION: External Cephalic Version (ECV) reduces breech presentation at term and thus contribute to the reduction of cesarean section. This study was done to determine the factors associated with the successful ECV in women with breech presentation after 36 weeks' gestation and also to develop and validate a clinical score which could be utilized for individual patient counseling in future. METHODS: This was a retrospective cohort study conducted in a tertiary care center and teaching hospital in south India. Prospectively collected data from the register maintained for all ECVs performe

Hypnosis for the control of pain associated with external cephalic version: a comparative study.

OBJECTIVE: To assess the effectiveness of hypnosis to reduce pain and facilitate external cephalic version (ECV). DESIGN: Cohort study. SETTING: Geneva University Hospitals, Switzerland. PARTICIPANTS: 63 women attempting ECV under hypnosis from 2010 to 2011 were compared with 122 women who received standard care from 2005 through 2008. INTERVENTION: Immediately after the ECV attempt, both groups completed the same questionnaire evaluating the participants' pain (visual analogue and verbal rating scales) and experience with the procedure. Physicians also completed a questionnaire that elici

External cephalic version for breech presentation with or without spinal analgesia in nulliparous women at term: a randomized controlled trial.

OBJECTIVE: To compare the success of external cephalic version using spinal analgesia with no analgesia among nulliparas. METHODS: A prospective randomized controlled trial was performed in a tertiary referral center delivery suite. Nulliparous women at term requesting external cephalic version for breech presentation were randomized to receive spinal analgesia (7.5 mg bupivacaine) or no analgesia before the external cephalic version. An experienced obstetrician performed the external cephalic version. Primary outcome was successful conversion to vertex presentation. RESULTS: Seventy-four wo

Pregnancy outcome after successful external cephalic version for breech presentation at term.

OBJECTIVE: Our purpose was to review the outcome of pregnancies after external cephalic version at term, in particular the incidence and indications of intrapartum cesarean section after successful external cephalic version. STUDY DESIGN: A prospective study was performed of 241 term pregnancies that had a total of 243 external cephalic versions. Each case with successful external cephalic version was matched to two control cases with cephalic presentation to compare pregnancy outcome. RESULTS: External cephalic version was successful in 169 attempts (69.5%), of which 7 (4.1%) reverted to br

The effect of epidural anesthesia on safety and success of external cephalic version at term.

The purpose of this study was to assess the effect of epidural anesthesia on the success and safety of external cephalic version (ECV) performed at term. A retrospective record review of all pregnant women > 36 wk gestation who had attempts at ECV at Arnold Palmer Hospital for Children and Women between April 2, 1992, and April 30, 1993, was performed. The standard contraindications to ECVs were observed, and the use of tocolytics and lumbar epidural anesthesia was based on personal preference of the patient's physician. Sixty-one patients underwent 69 attempts, with eight patients having two

Considering the timing and appropriateness of trial consent during childbirth: a secondary analysis from the qualitative process evaluation of the ROTATE trial.

BACKGROUND: The ROTATE trial is a multi-centre randomised controlled trial of manual versus instrumental rotation of the fetal head in malposition at birth, to evaluate if manual compared with instrumental rotation reduces the risk of severe maternal perineal trauma, without increasing caesarean birth rates. ROTATE internal pilot phase ran from June 2022 to June 2023 with an embedded qualitative process evaluation (QPE). The QPE aimed to explore the feasibility, acceptability, and appropriateness of the trial for parents and healthcare professionals (HCPs). ROTATE had a two-stage consent proce

Breech presentation: Clinical practice guidelines from the French College of Gynaecologists and Obstetricians (CNGOF).

OBJECTIVE: To determine the optimal management of singleton fetuses in breech presentation. MATERIALS AND METHODS: Consultation of the PubMed database, the Cochrane Library and guidelines issued by the French and foreign obstetrical societies or colleges. RESULTS: In France, 5% of women have breech deliveries (level of evidence [LE] 3). One third of them have a planned vaginal delivery (LE3), and 70% of these give birth vaginally (LE3). External cephalic version (ECV) is associated with lower rates of both breech presentation at birth (LE2) and of cesarean deliveries (LE3) without any increa

Mode of delivery following external cephalic version and induction of labor at term.

External cephalic version should be followed by fetal and maternal surveillance until delivery. To shorten this period of surveillance, a protocol of version at term, followed by immediate induction of labor, was adopted. The aim of the present study was to evaluate the effect of this protocol on the subsequent mode of delivery. Two hundred and ninety-one singleton breech pregnancies were included in this study. Of these, 74 cases underwent version, 75% of which were successful. Success of version compared to failed version was associated with statistically significant (p < 0.001) lower rate o

[External over-rotation of the head in the treatment of high longitudinal shoulder position].

The shoulder dystocias are an anomaly of shoulder presentation which render difficult or prevent complete extraction of the trunk following birth of the head. Acute hypoxia ensues, threatening the infant's life. Additionally, termination of delivery, which must be effected without delay and is surgically difficult, is frequently associated with often permanent damage to the upper limbs. Macrosomatia of the child and a vaginal-surgical delivery initiated before the preceding part has reached the pelvic floor increase the probability of high longitudinal shoulder position occurring. Early and fo

Systematic review of adverse outcomes of external cephalic version and persisting breech presentation at term.

The aim of this study was to determine the frequency of adverse maternal and fetal outcomes of both external cephalic version (ECV) and persisting breech presentation at term. We conducted a systematic review of the literature using Medline, Embase and All Evidence Based Medicine (EBM) Reviews databases. Data were extracted from studies that compared women who had an ECV from 36 weeks' gestation with a similar control group of women enrolled at the same gestational age, eligible for, but who did not have an ECV. Eleven studies with a total of 2503 women were included. Adverse outcomes related

Spontaneous rupture of the unscarred uterus.

OBJECTIVE: By presentation of cases of spontaneous (nontraumatic) ruptures of previously intact uteri, we sought to emphasize important aspects of this rare and dangerous event. STUDY DESIGN: Two case presentations of oxytocin-associated unscarred uterine rupture and review of pertinent literature are used to study risk factors and accompanying clinical characteristics. RESULTS: Both spontaneous ruptures of previous unscarred uteri were associated with low-dose oxytocin augmentation, bradycardia, and uterine hyperstimulation monitor patterns and occurred at the onset of the second stage of l
