Feasibility of C2 translaminar screw as an alternative or salvage of C2 pedicle screws in atlantoaxial instability.

STUDY DESIGN: A retrospective outcome study. OBJECTIVE: The purpose of this study was to analyze the clinical outcomes of our experience with the C2 translaminar screw technique and evaluate its feasibility as an alternative or salvage of the pedicle screw. SUMMARY OF BACKGROUND DATA: Pedicle screws are the most biomechanically stable screws for use in atlantoaxial fixation. However, in cases with elevated risk of neurovascular complications or failure of screw insertion attempts, a reliable alternative technique is required. METHODS: Fourteen patients with atlantoaxial instability underwen

Cranial subluxation of the odontoid process in rheumatoid arthritis.

In eighteen patients who had long-standing severe rheumatoid polyarthritis, cranial subluxation of the odontoid process was caused by erosion and collapse of both the occipitocervical and the atlantoaxial facet joints. In five of the patients, the subluxation caused impairment of cranial nerves. One patient was tetraparetic. Six patients had a posterior fusion of the spine; of these, three also had laminectomy of the atlas. Operative treatment seemed to arrest the subluxation, but there was appreciable functional improvement in only four of the six patients. During an average of four years of

Odontoid fractures, with special reference to the elderly patient.

A retrospective review of 262 consecutive cervical spine fractures revealed that 41 (16%) were odontoid fractures. Twenty patients were younger than 40 years of age (19 fractures were the result of a motor vehicle accident), whereas 19 fractures occurred in subjects older than 60 (13 were the result of a fall). Fractures were of the Anderson-D'Alonzo Types I (one patient), II (19 patients), and III (21 patients). Significant displacement was present in 31 patients. Anterior and posterior displacement occurred equally, but most of the anterior fractures were seen in patients younger than 40, an

Posterior Screw-Rod Fixation and Selective Axial Loosening for the Treatment of Atlantoaxial Instability or Dislocation Caused by Os Odontoideum: A Case Series for a Single Posterior Approach.

OBJECTIVE: To evaluate the effect of screw-rod fixation and selective axial loosening in the treatment of atlantoaxial instability or dislocation (including reducible and irreducible) caused by os odontoideum (OO) via a single posterior approach. METHODS: A consecutive series of patients with OO surgically treated in our hospital were retrospectively analyzed. For atlantoaxial instability and reducible atlantoaxial dislocation, C1-C2 screw-rod fixation and fusion were performed. OO combined with irreducible atlantoaxial dislocation was reduced after posterior axial loosening, followed by scre

Advances in classification, basic mechanisms and clinical science in ankylosing spondylitis and axial spondyloarthritis.

The field of spondyloarthritis (SpA) has seen huge advances over the past 5 years. The classification of axial disease has been redefined by the axial SpA criteria that incorporate disease captured before radiographic damage is evident as well as established erosive sacroiliac joint disease. Our knowledge of genetics and basic immunological pathways has progressed significantly. In addition, revolutionary progress has been achieved with the availability of tumour necrosis factor inhibitors for treating patients with moderate to severe disease. In parallel, several of novel biomarkers have been

Surgical Management of Odontoid Fracture in Elderly.

Odontoid type II fractures are associated with high mortality in the elderly. No formal guidelines are present regarding the treatment of such fractures. Their management can be done either conservatively or surgically, however, surgical treatment is technically demanding and relatively new. We report a case of 75 years old man who presented to our clinical setting, following a history of fall. The CT scan and MRI showed odontoid type II fracture. With time, the neurological deficit got marked and he underwent transoral odontoidectomy and occipitocervical fixation using rods and screws via pos

Efficacy and safety of anticoagulation on patients with cirrhosis and portal vein thrombosis.

BACKGROUND & AIMS: Portal vein thrombosis (PVT) is a frequent event in patients with cirrhosis; it can be treated with anticoagulants, but there are limited data regarding safety and efficacy of this approach. We evaluated this therapy in a large series of patients with cirrhosis and non-neoplastic PVT. METHODS: We analyzed data from 55 patients with cirrhosis and PVT, diagnosed from June 2003 to September 2010, who received anticoagulant therapy for acute or subacute thrombosis (n = 31) or progression of previously known PVT (n = 24). Patients with cavernomatous transformation were excluded.

Foramen magnum syndrome caused by a dolichoodontoid process.

A 21-year-old Japanese man with basilar impression with an anomalous configuration of the odontoid process and many other vertebral anomalies is reported. We thought that posterior decompression alone would be hazardous; therefore, in one session, the odontoid tip and anterior arch of the atlas were removed transorally, and posterior fixation between the occipital squama and the lamina of C-3 using acrylic plastic was performed. This treatment resulted in marked clinical improvement and required only a short hospital stay.

A case of chordoma invading multiple neuroaxial bones: report of ten years follow up.

AIM: Chordoma is a rare, slow-growing primary malignant tumor of the axial skeleton, arising from the embryonic cells of primitive notochord. Chordomas may arise at different sites of the vertebral column simultaneously or more probably they may metastasise along the neural axis insidiously. Recurrence despite radical surgery and following adjuvant therapy is possible. MATERIAL AND METHODS: A 46-year-old female patient presented weakness and numbness of the lower extremities. She was operated for clivus chordoma five years ago at another institute. RESULTS: First the patient underwent surger

Increased Cdx protein dose effects upon axial patterning in transgenic lines of mice.

To investigate the link between Cdx protein concentration and axial patterning in embryos, we made lines of mice OE1, OE2 and OE4 that overexpress each of the Cdx genes Cdx1, Cdx2 and Cdx4, respectively. The lines carry Cdx transgenes under the transcriptional control of their own promoter/enhancer elements. Transgenic embryos show Cdx transcription at 8.5 to 8.7 days within normal spatial domains for Cdx expression (primitive streak/tailbud), yet, overall, they contain elevated levels of Cdx proteins. Increased doses of Cdx proteins result in homeotic shifts in vertebral types along most of t

Movement of the external ear in human embryo.

INTRODUCTION: External ears, one of the major face components, show an interesting movement during craniofacial morphogenesis in human embryo. The present study was performed to see if movement of the external ears in a human embryo could be explained by differential growth. METHODS: In all, 171 samples between Carnegie stage (CS) 17 and CS 23 were selected from MR image datasets of human embryos obtained from the Kyoto Collection of Human Embryos. The three-dimensional absolute position of 13 representative anatomical landmarks, including external and internal ears, from MRI data was traced

Morphological variations in the transverse foramen of the axis in Japanese serows (Capricornis crispus).

The presence of transverse foramina in the axes of Japanese serows, a special national natural treasure in Japan, has been reported to be unstable, but other variations are unknown. In this study, we analysed the shape, cross-sectional area, length, and volume of the transverse foramen in the axes of 19 specimens using gross anatomy and computed tomography (CT) scan. There were four types in the transverse foramen: type 1, having the transverse foramina; type 2, having two cranial openings; type 3, sifting a caudal opening to the ventral side of the transverse process; and type 4, having no tr

[Injuries to the craniocervical junction].

Injuries to the upper cervical spine (C0-C2) play a major role in surgical treatment of traumatic sequelae in the entire cervical spine. Even though the number of such operations has increased in recent years, there are no clear treatment recommendations for most types of cervical spine injuries. In view of the wide range of injury types and the correspondingly large number of treatment options, this review focuses mainly on the following types of injuries: C0 fractures, occipital condyle fractures (OCF), atlanto-occipital dislocation (AOD), atlas fractures, atlantoaxial dislocation (AAD), and

Two-stage surgical approach for type III odontoid fracture with C1-C2 fracture dislocation: a case report and management overview.

INTRODUCTION AND IMPORTANCE: Odontoid fractures of the second cervical vertebra (C2) are categorized into three types, with type III extending into the body of the axis. These fractures, often resulting from high-energy trauma, can cause significant instability and neurological issues. This case report discusses a 43-year-old male with a type III odontoid fracture and C1-C2 fracture dislocation, demonstrating the effectiveness of traditional neurosurgical techniques in managing such complex injuries. CASE PRESENTATION: A 43-year-old male presented with upper cervical pain and right upper limb

CT scan study of atlantoaxial rotatory mobility in asymptomatic adult subjects: a basis for better understanding C1-C2 rotatory fixation and subluxation.

STUDY DESIGN: Normal rotation was evaluated in a group of 40 asymptomatic adults. OBJECTIVE: To determine the normal rotational limits of C1-C2 in adults and define when a rotatory fixation occurs in the limits of normality or in subluxation. The term subluxation should be used only when C1-C2 is rotated beyond normal limits. SUMMARY OF BACKGROUND DATA: Concepts about rotatory fixation were established by accepting that it may occur within the limits of normal range of motion. Although nowadays CT is the current image method used to evaluate any case of torticollis, no study has been perform
