[A new method for the treatment of supraventricular rhythm disorders].

Initial experience of His-bundle ablation is presented in 12 patients with intractable supraventricular arrhythmias which were not amenable to treatment with conventional drugs. In 8 patients His-bundle ablation resulted in total AV-block and in 2 patients significant improvement without total AV-block was achieved. During long-term follow-up (mean 5.3 months) tachycardia recurred in 1 patient with total AV-block resulting in an overall success rate of 75%. A permanent pacemaker was implanted in 10 patients. 1 patient developed deep leg vein thrombosis as a direct result of this procedure and

Transhepatic Left Bundle Branch Area Pacing in a Patient With Ebstein Anomaly and Prior Glenn Surgery: A First Report.

BACKGROUND: Venous access exhaustion poses a significant challenge for the implantation of intracardiac devices. Epicardial alternatives are often used in these cases, despite their long-term deleterious effects. This case presents a transhepatic approach for left bundle branch pacing in a patient with Ebstein anomaly and a prior Glenn procedure. METHODS: Following pre-procedural evaluation with triphasic CT, ultrasound-guided hepatic vein puncture was performed. Guidewires were advanced into the right heart chambers, allowing passage of a delivery sheath for physiologic pacing. Atrial lead p

Global R-wave peak time for diagnosis of left bundle branch capture.

BACKGROUND: QRS axis deviation and rS configuration in V6 affect the ability of V6 R-wave peak time (RWPT) criterion to discriminate capture type during left bundle branch area pacing (LBBAP). OBJECTIVE: We hypothesized that combining RWPTs from lateral leads: I, aVL, V5, and V6 may better reflect left ventricular activation time and that such a global RWPT may be insensitive to changes in QRS configuration. METHODS: The analysis included 519 electrocardiograms (ECGs) with nonselective left bundle branch pacing (nsLBBP) and 176 ECGs with left ventricular septal pacing (LVSP). Optimal RWPT cu

Left bundle branch pacing versus biventricular pacing for cardiac resynchronization therapy: A systematic review and meta-analysis.

INTRODUCTION: Cardiac resynchronization therapy (CRT) reduces heart failure (HF) hospitalization and all-cause mortality in HF patients with left bundle branch block (LBBB). Biventricular pacing (BVP) is the gold standard for achieving CRT, but about 30%-40% of patients do not respond to BVP-CRT. Recent studies showed that left bundle branch pacing (LBBP) provided remarkable results in CRT. Therefore, we conducted a meta-analysis aiming to compare LBBP-CRT versus BVP-CRT in HF patients. METHODS: We systematically searched the electronic databases for studies published from inception to Decemb

Left bundle branch pacing in alternating bundle branch block.

We present a case of symptomatic intermittent AV block showing during monitorization alternating bundle branch block. Presuming a high need of pacing, conduction system pacing was considered a more physiological alternative in this patient. Left bundle branch pacing restored a stable atrioventricular synchrony with a paced QRS complex similar to the native one and might be a good alternative in these cases.

Antidromic and orthodromic reciprocating tachycardias over a novel left-sided accessory pathway involving the vein of Marshall and coronary sinus musculature.

INTRODUCTION: Herein, we present a rare case of the successful ablation of an accessory pathway (AP) involving the Marshall Bundle (MB) and coronary sinus musculature (CSM) in a 40-year-old man with Wolff-Parkinson-White syndrome. METHODS AND RESULTS: An orthodromic reciprocating tachycardia (ORT) was inducible with the earliest atrial activation site located at the posterolateral mitral annulus. The local conduction and the cycle length of ORT was prolonged by peri-mitral ablation; however, it failed to block the AP. The atrial insertion of the AP was identified by remapping during ORT at th

Safety and efficacy of left bundle branch pacing in comparison with conventional right ventricular pacing: A systematic review and meta-analysis.

BACKGROUND: Right ventricular pacing (RVP) has been widely accepted as a traditional pacing strategy, but long-term RVP has detrimental impact on ventricular synchrony. However, left bundle branch pacing (LBBP) that evolved from His-bundle pacing could maintain ventricular synchrony and overcome its clinical deficiencies such as difficulty of lead implantation, His bundle damage, and high and unstable thresholds. This analysis aimed to appraise the clinical safety and efficacy of LBBP. METHODS: The Medline, PubMed, Embase, and the Cochrane Library databases from inception to November 2020 wer

Cardiac resynchronization with His-CRT-D in a patient with severe heart failure and Scimitar syndrome.

Cardiac resynchronization therapy with His-bundle pacing is evolving rapidly as a viable cardiac device strategy for the treatment of severe chronic heart failure. The success of this technique in patients with congenital heart disease is facilitated by advanced integrated imaging modalities. We report a case of cardiac resynchronization therapy with His-bundle pacing with defibrillator for the management of a patient with heart failure with severely reduced ejection fraction, left bundle branch block, and congenital heart disease characterized by Scimitar syndrome with cardiac dextroposition.

Assessment of ventricular mechanical synchronization after left bundle branch pacing using 2-D speckle tracking echocardiography.

BACKGROUND: The left bundle branch pacing (LBBP) makes the ventricular depolarization closer to the physiological state and shortens QRS duration. The purpose of this study is to explore the ventricular systolic mechanical synchronization after LBBP in comparison with traditional right ventricular pacing (RVP) using two-dimensional strain echocardiography (2D-STE). METHODS: Thirty-two patients who received LBBP (n = 16) or RVP (n = 16) from October 2018 to October 2019 and met the inclusion criteria were included in this retrospective study. Electrocardiogram (ECG) characteristics, pacing par

Left Bundle Branch Block: Current and Future Perspectives.

Left bundle branch block may be due to conduction system degeneration or a reflection of myocardial pathology. Left bundle branch block may also develop following aortic valve disease or cardiac procedures. Patients with heart failure with reduced ejection fraction and left bundle branch block may respond positively to cardiac resynchronization therapy. Lead placement via the coronary sinus is the mainstay approach of cardiac resynchronization therapy. However, other options, including physiological pacing, are being explored. In this review, we summarize the salient pathophysiologic and clini

Efficacy and safety of cryoablation of parahissian accessory pathways in children: a single institution study.

BACKGROUND: Catheter cryoablation of supraventricular tachycardias involving the perinodal regions is considered to be a safer alternative compared to radiofrequency ablation. Limited information is available for efficacy, midterm outcomes, and complications regarding the ablation of parahissian accessory pathways (APs) in pediatric patients. METHODS: A retrospective review of all pediatric patients who underwent cryoablation for treatment of a parahissian AP was performed. RESULTS: Twenty-five patients (median age 13 years and weight 45.6 kg) underwent cryoablation of a parahissian AP. Medi

Alternate site pacing in patients at risk for heart failure.

Cardiac pacing from the right ventricular apex is the most common site of cardiac pacing. During the last decade, several studies demonstrated the harmful effects of the iatrogenic left bundle branch block, which is observed in cardiac pacing from the right ventricular apex. These observations led to an interest in alternative right ventricular pacing sites aiming to achieve a more "physiological" pattern of ventricular activation. Alternate site pacing may involve His bun- dle, other right ventricular sites (outflow or septal sites), or left ventricular sites in either unifocal or bifocal or

Ventricular tachycardias with left bundle branch block morphology.

The differential diagnosis of VTs with LBBB morphology includes several well-defined syndromes. Although the majority of cases are attributable to acquired structural heart disease, including ischemia, prior infarction, or dilated cardiomyopathy, consideration of specific right ventricular processes is essential to proper evaluation and treatment. The approach to older patients or those with evidence for heart disease should begin with an evaluation for coronary artery disease and an assessment of biventricular function. Careful evaluation for bundle branch reentry should be performed during e

Right ventricular infarction: unusual electrocardiographic and electrophysiological manifestations.

Right ventricular infarction occurs in 19-43% of patients with acute inferior wall infarction (Lorell et al., 1979). Its clinical, hemodynamic, and anatomic features are well known and include associated inferior wall infarction, distended neck veins, clear lung fields, hypotension, and heart block (Cintron et al., 1981; Coma-Canella et al., 1979; Lloyd et al., 1981; Lopez-Sendon et al., 1981; Raabe and Chester, 1978; Rotman et al., 1974). Isolated right ventricular infarction is less frequent and occurs in 2.5-4.6% of autopsy studies of myocardial infarction (Cohn et al., 1974; Erhardt et al.

Programmed chest-wall stimulation to evaluate the progress of A-V block after pacemaker insertion in patients with trifascicular disease.

Twenty six patients (aged 46-80, mean age 64) with bifascicular block in the presence of prolonged H-V interval (trifascicular block), were followed for an average of 31 months after inserting an R-wave inhibited pacemaker (PM) because of syncope and/or dizzy attacks. The underlying rhythm was evaluated at 4-6-month intervals by three different techniques: 1) 12-lead ECG when intrinsic patient rate was faster than PM rate; 2) abrupt PM inhibition (APMI) by the rapid chest-wall stimulation technique, and 3) progressive PM inhibition (PPMI) using a programmed chest-wall stimulation technique cap

Similar QRS morphology in sinus rhythm and ventricular tachycardia.

A 74-year-old man with coronary artery disease and bifascicular block suffered recurrent tachycardia. Although the similarity of QRS morphology in tachycardia and sinus rhythm suggested a supraventricular origin, His bundle electrocardiography and programmed stimulation showed a ventricular origin. Digitalis toxicity was not an etiological factor and there was no evidence to suggest a His bundle tachycardia. The case illustrates that in occasional circumstances the onset of ventricular tachycardia may produce no change in QRS morphology thus making the diagnosis by conventional electrocardiogr

Effect of intermittent left bundle branch block on left ventricular performance in the normal heart.

The effect of left bundle branch block on left ventricular performance was studied in a patient with rate-dependent left bundle branch block and an otherwise normal heart. The timing of left heart events, hemodynamic pressures and indexes of left ventricular contractility were determined during normal intraventricular conduction and during left bundle branch block. The results indicate that in the normal heart left bundle branch block has no significant effect on left ventricular performance.

Recurrent ventricular tachycardia due to reentry within the bundle branches.

A case of recurrent ventricular paroxysmal tachycardia in a young man without obvious heart disease is described. Posttachycardiac T-wave inversion followed the arrhythmic episodes. Programmed stimulation of the heart in association with intracardiac recordings showed evidence in favor of reentry within the bundle branches and its divisions during tachycardia. A critical prolongation of the H2-V3 interval preceded each paroxysm of tachycardia.
