Chlorhexidine gluconate–impregnated central access catheter dressings as a cause of erosive contact dermatitis: a report of 7 cases.

BACKGROUND: Chlorhexidine gluconate-impregnated dressings have become widely adopted as a means to reduce the risk for catheter-associated bloodstream infections. These dressings release antiseptic under occlusion onto the skin surrounding catheter insertion sites. Although chlorhexidine gluconate is a known cause of contact dermatitis, the phenotypic range of this adverse effect of chlorhexidine gluconate–impregnated dressings in critically ill patients has not been described. OBSERVATIONS: We report 7 cases of erosive irritant contact dermatitis due to chlorhexidine gluconate-impregnated tr…

Clinical and pathological changes in the knee after accidental chlorhexidine irrigation during arthroscopy. Case reports and review of the literature.

We describe six knees in five patients, referred to us after accidental irrigation with chlorhexidine 1% in aqueous solution during arthroscopy. All six knees developed persisting pain, swelling and crepitus with loss of range of movement. Radiographs showed loss of joint space in all three compartments due to extensive chondrolysis, with many loose bodies and synovitis. Histological examination showed partial necrosis of the cartilage, with slight non-specific inflammation and fibrosis of synovial specimens. Care is needed in checking irrigation fluids, and these should have a distinctive col…

A 6-month home usage trial of a 1% chlorhexidine toothpaste (1). Effects on plaque, gingivitis, calculus and toothstaining.

Research and development has been aimed towards the development of toothpastes for oral hygiene and gingival health benefits. The most effective antiplaque agent to date, chlorhexidine, is difficult to formulate into active toothpaste products. Despite some encouraging findings for experimental chlorhexidine toothpastes, there are to date no commercially available products. Based on positive findings from an experimental gingivitis study, this investigation evaluated 2 chlorhexidine toothpastes, one with fluoride, as adjuncts to oral hygiene. The study was a double-blind controlled, parallel, …

Standardized Centella asiatica extract (ECa 233) mitigates chlorhexidine-induced cytotoxicity and promotes oral wound repair via immunomodulation and angiogenesis.

OBJECTIVE: Although 0.12% Chlorhexidine (CHX) is the gold standard oral antiseptic, its significant cytotoxicity often delays tissue repair. This study investigated the potential of ECa 233, a standardized Centella asiatica extract, to mitigate CHX-induced damage and promote oral wound regeneration. DESIGNS: Antimicrobial efficacy was assessed via minimum inhibitory concentration (MIC) against key oral pathogens. Human Gingival Fibroblasts (HGFs) were treated with CHX and ECa 233 to evaluate viability (MTT) and migration (scratch assay). Anti-inflammatory effects were measured by quantifying …

Long-Term Daily Chlorhexidine Foot Cleansing Reduces Staphylococcal Burden on the Feet of People with Prior Diabetic Foot Complications.

BACKGROUND: Individuals with diabetes remain at high risk for diabetic foot ulcers (DFUs) even after ulcer healing. Dysbiosis of the skin microbiome has been associated with ulcer formation. Topical chlorhexidine gluconate (CHG) is a broad-spectrum antiseptic commonly used to reduce microbial burden. In a prior randomized clinical trial comparing daily CHG foot treatment with soap-and-water treatment, no statistically significant reduction in new DFUs was observed, prompting evaluation of whether CHG produced durable changes in the skin microbiota. OBJECTIVE: To compare changes in foot skin m…

Long-term efficacy of chlorhexidine-containing cutaneous dressings on ventriculostomy-related infection: a 10-year before-and-after study.

OBJECTIVE: Growing evidence suggests that dressings containing chlorhexidine-gluconate (CHX) have the potential to lower the incidence of external ventricular drain (EVD)-associated infections (EVDAIs). A previous prospective randomized trial (NCT02078830) detected significant reductions in bacterial cutaneous and catheter colonization. However, the study was underpowered to demonstrate significant clinical efficacy. The present study aimed to report infection rates within the same institution before and after the abovementioned randomized controlled trial (RCT) with consecutive introduction o…

The Sustained-released Polylactic Co-glycolic Acid Nanoparticles Loaded with Chlorhexidine as Antibacterial Agents Inside the Dental Implant Fixture.

INTRODUCTION: Titanium-based implants are widely used due to their good biocompatibility and high corrosion resistance. Infections after implant placement are the main reason for the failure of implant treatment. Some recent studies have also shown that microbial contamination can occur at the implant-abutment level in implants with healthy or diseased surrounding tissue. The purpose of this study is to investigate the antibacterial effect of slow-release nanoparticles of polylactic co-glycolic acid (PLGA) loaded with chlorhexidine (CHX) inside the implant fixture. MATERIALS AND METHODS: Thir…
