Measles remains one of the leading causes of childhood morbidity and mortality in developing countries. The World Health Organization has identified effective case management as one of the specific strategies to reduce the burden of this disease. Measles, mumps, and rubella vaccine carries a risk of joint symptoms particularly in children under 5 years. A boy who presented with an inflamed knee after measles and mumps vaccination is reported; synovial fluid aspirated from the joint contained 4.3 x 10(9)/l leucocytes. It is thought that the mumps component is the aetiological cause of acute monoarthritis.

In this review we briefly summarise virological and immunological aspects of acute and persistent measles virus (MV) infections as found in man and further investigated in animal models and tissue culture. Several mechanisms contributing to MV persistence are discussed. Recent findings, especially the analysis of CD46 and moesin as receptor acting molecules for MV, correlate well with the tropism of MV and open new fields for the investigation of MV associated immunosuppression and central nervous system (CNS) complications.

OBJECTIVES: To provide accurate information on the common sequelae of measles-mumps-rubella (MMR) vaccination and to compare post-vaccine symptoms in children vaccinated at 13 and 15 months.
DESIGN: Prospective cluster randomized controlled trial.
SETTING: Twenty-two family practices in southwestern Ontario.
PATIENTS: All 376 infants who were due to receive MMR vaccine in the next year, 253 (67.3%) successfully completed the study.
INTERVENTION: MMR vaccine administered at 13 months by half of the family physicians and at 15 months by the remaining half.

The effect of primary measles vaccination after orthotopic liver transplantation was evaluated in 18 children. Immunity developed in seven children by serologic criteria. There were no complications directly attributable to the vaccine. These preliminary observations suggest that further study of measles vaccination in this group is warranted.

Severe neurological complications are presented in the course of measles in three older children. In one child encephalomeningitis was diagnosed, and in two children encephalomeningingomyelitis was recognise. Favourable results were achieved after the use of hormonal therapy under antibiotic cover. The children were cured. In order to prevent effectively measles and itd complications, the prophylaxis should be intensified through rigorous programme of vaccination against this disease.

Since 1982, a network of general practitioners registers measles in Belgium. The trend is notably downwards in the whole country, but significantly more in the Flemish region than in Wallonia. A possible explanation is the lower vaccination coverage in Wallonia. In Flanders this coverage for persons under 20 years old is estimated at 65%. Natural killer cells are postulated to play an important role in host anti-viral defences. We measured natural killer cell activity in 30 individuals with acute measles (73 +/- 21 lytic units (LU)/10(7) cells) and 16 individuals with other infectious diseases (149 +/- 95 LU) and found it reduced compared with values for adults (375 +/- 70 LU; P less than 0.001) or children (300 +/- 73 LU, P less than 0.01) without infection. A retrospective analysis of 208 cases of measles admitted to Sabbah Children's Hospital, Juba is presented. Seventeen per cent of the children were less than 10 months of age. The overall case fatality rate was 23%. The death rate was lowest in the first 9 months of life and increased with age, as did the incidence of malnutrition. It is recommended that measles vaccination should commence at 6 months of age in the Juba area, and that intensive health education about the dangers and management of measles be instituted.

Epidemics of measles had been seen biennially over the past two decades at Princess Mary Hospital, Auckland, until 1980. After a four year gap, there has been a further epidemic. The medical records of 201 measles cases seen between July 1984 and August 1985 were reviewed. Maori and Pacific Island children were over-represented (86%), as were children aged less than 2 years (53%). Two children died. A significant proportion of the illness and death of diarrhoeal diseases in the developing world is estimated to be due to the diarrhoea associated with measles. During February 1983-January 1984 a prospective study of measles in a hospital in Northeastern Thailand was conducted. A total of 550 cases of measles were studied. Two hundred and one parents attending three child health clinics were questioned about both measles and immunisation against measles. Most parents were unaware of the symptoms and possible complications of measles and did not believe immunisation to be effective in preventing measles. They did not remember having talked to health professionals about immunisation.
