Traumatic atlantoaxial rotatory dislocation and displaced ossiculum terminale epiphysiolysis treated with a halo device: a case report.

INTRODUCTION: The horizontal synchondroses of the infant axis are biomechanical weak regions in trauma. CASE: We report the case of a 6-year-old girl who presented with delayed atlantoaxial dislocation and displaced ossiculum terminale epiphysiolysis after a fall from a tree. TREATMENT: The patient was treated with halo traction for 3 days after which a CT scan showed normal position of the C1/C2 joint, and an acceptable position of the ossiculum terminale whereafter a halo brace was applied. Because of delayed union on CT scans, the treatment was prolonged to a total of 21 weeks. RESULT: A

Interdisciplinary orthodontics-implants management of a challenging hypodontia case complicated by a class II division 2, bimaxillary retroclination.

INTRODUCTION: The case in this study had moderate hypodontia, with both lower lateral incisors and the lower-left second premolar missing. A Class II division 2 relationship with severe crowding in the upper arch and a traumatic deep bite over a skeletal I base complicated the occlusion. MATERIALS AND METHODS: The plan was to extract the upper first premolars to relieve upper arch crowding and the lower-left impacted second premolar to preserve the bilateral class I molar relationship. A class I occlusal relationship was achieved through space opening in the lower lateral incisors region and

Case report of severe unilateral posterior scissors bite in children treated with clear aligners.

UNLABELLED: Posterior scissors bite is a clinically common severe malocclusion, which often leads to impaired masticatory function, unilateral mastication habit and progressive facial asymmetry. This case report presented an 8-year-old boy with severe unilateral posterior scissors bite in mixed dentition, who received early interceptive orthodontic treatment with bracket-free clear aligners combined with alternating intermaxillary traction. The treatment procedures included staged expansion of dental arch width, uprighting of severely lingually inclined molars, and complete correction of poste

Concept for the Treatment of Class III Anomalies with a Skeletally Anchored Appliance Fabricated in the CAD/CAM Process-The MIRA Appliance.

OBJECTIVE: Intermaxillary elastics, anchored skeletally, represent a promising concept for treatment in adolescent patients with skeletal Class III anomalies. A challenge in existing concepts is the survival rate of the miniscrews in the mandible or the invasiveness of the bone anchors. A novel concept, the mandibular interradicular anchor (MIRA) appliance, for improving skeletal anchorage in the mandible, will be presented and discussed. CLINICAL CASE: In a ten-year-old female patient with a moderate skeletal Class III, the novel MIRA concept, combined with maxillary protraction, was applied

Interceptive orthodontics: awareness and prevention is the first cure.

It is widely recognised by the scientific dental community that the correct development of the deciduous and mixed dentitions is paramount to the oral health of paediatric patients. In this respect, interceptive orthodontics plays a fundamental role in the process. Specifically, the paediatric dentist monitors the condition of the mouth from early childhood, distinguishing three age brackets for intervention, each defined by their own characteristics. It would also be desirable for other professionals who treat young patients and their mothers to various extents, such as paediatricians, gynaec

Corticotomy-assisted adult rapid maxillary arch expansion and ridge augmentation: An interdisciplinary case report with 7-year follow-up.

Adult orthodontic treatment involving maxillary transverse deficiency is a challenge for an interdisciplinary team. Surgically assisted rapid palatal expansion to segment the maxilla was once the treatment of choice, but the invasiveness, bone deficiency, and gingival recession hindered its acceptance. Corticotomy-assisted rapid maxillary arch expansion with ridge augmentation has the advantage of augmenting alveolar bony housing to accommodate and facilitate tooth movement. This approach was used to correct a severely constricted maxilla with bilateral posterior crossbite and anterior crowdin

Differences in bonding to acid-etched or Er:YAG-laser-treated enamel and dentin surfaces.

STATEMENT OF PROBLEM: Er:YAG (erbium-doped yttrium aluminium garnet) lasers have been effective in the removal of dental tissues. It has been suggested that they are also useful for preparing dental surfaces for adhesion, but results to date have been controversial. PURPOSE: This study compared the tensile strength of bracket-tooth bonds obtained after preparation of the surface for adhesion (dentin or enamel) by conventional acid-etching or by Er:YAG laser etching and investigated microstructure of resin-tooth interfaces using the 2 procedures. MATERIAL AND METHODS: Eighty healthy human pre

Pulpal response in electrothermal debonding.

An alternative method to conventional bracket removal that minimizes the potential for ceramic bracket failure as well as trauma to the enamel surface is electrothermal debonding (ETD). However, the potential for pulpal damage using ETD on ceramic brackets still needs assessment. The purpose of this research is to investigate and assess any pulpal damage caused by ETD. Ten patients requiring four premolar extractions each were randomly selected (5 boys and 5 girls). Ceramic brackets were bonded to experimental and control teeth. A total of 30 teeth were used to provide histologic material of t

Comparison of anchorage reinforcement with temporary anchorage devices or a Herbst appliance during lingual orthodontic protraction of mandibular molars without maxillary counterbalance extraction.

BACKGROUND: Orthodontic protraction of mandibular molars without maxillary counterbalance extraction in cases of aplasia or extraction requires stable anchorage. Reinforcement may be achieved by using either temporary anchorage devices (TAD) or a fixed, functional appliance. The objective was to compare the clinical effectiveness of both methods by testing the null-hypothesis of no significant difference in velocity of space closure (in mm/month) between them. In addition, we set out to describe the quality of posterior space management and treatment-related factors, such as loss of anchorage

Rapid and selective removal of composite from tooth surfaces with a 9.3 µm CO2 laser using spectral feedback.

OBJECTIVE: Dental composite restorative materials are color matched to the tooth and are difficult to remove by mechanical means without excessive removal or damage to peripheral enamel and dentin. Lasers are ideally suited for selective ablation to minimize healthy tissue loss when replacing existing restorations, sealants, or removing composite adhesives such as residual composite left after debonding orthodontic brackets. METHODS: In this study, a carbon dioxide laser operating at 9.3-µm with a pulse duration of 10-20-microsecond and a pulse repetition rate of ∼200 Hz was integrated with a…
