Approach to treatment of Legg-Calvé-Perthes disease.

The essence of management of Legg-Calvé-Perthes disease (LCPD) should be the provision of containment and its maintenance during epiphyseal remodeling. Containment may be achieved by a variety of nonoperative and surgical methods. The choice for individual patients depends on the patients' age at onset and the degree of epiphyseal involvement. The specifics of treatment and its duration are presented in 2 tables for practical assistance in the proper choices. Comparative analysis has confirmed that improved results can be obtained when patients are treated by this approach.

In vivo quantification of knee-orthosis valgus-corrective moment during gait: Effects of medial strut length on external knee adduction moment in medial knee osteoarthritis.

Osteoarthritis (OA) is a progressive degenerative disease affecting articular surfaces, and knee orthoses are commonly employed to reduce the external knee adduction moment (EKAM). However, quantitative data on the corrective moments generated by orthoses during gait and their relationship with EKAM remain limited. This study quantified the corrective moments generated by a knee orthosis using a six-axis force sensor and evaluated the impacts of medial lower-leg strut length on these moments and EKAM, aiming to elucidate the biomechanical mechanisms by which orthotic design facilitates unloadi

Effects of short- or long-term use of orthotic interventions on functional performance in individuals with chronic ankle instability: A systematic review.

BACKGROUND: Individuals with chronic ankle instability (CAI) have problems in function. The purpose of this study was to systematically investigate the short- or long-term effects of various orthoses on function of these people. METHODS: Randomized controlled trial (RCT) studies reporting on the short- or long-term effects of orthotic intervention on functional performance in individuals with CAI were identified through PubMed, Cochrane Library, PEDro, MEDLINE via Ovid, and Scopus databases from inception to July 2022. Two reviewers independently extracted data and assessed the risk of bias a

Effect of short-term spinal orthosis and insoles application on cobb angle, plantar pressure and balance in individuals with adolescent idiopathic scoliosis.

BACKGROUND: Spinal orthosis applications are preferred for conservative treatment of adolescent idiopathic scoliosis, and holistic biomechanical approaches are recommended. METHODS: This was single-blind, prospective, randomized, controlled study. It included 42 patients (29 females/13 males) aged 10-18 years with adolescent idiopathic scoliosis, Cobb angle of 20°-45°, who were deemed suitable for spinal orthosis use. Patients were randomly divided into two groups: control, spinal orthosis group (n = 21), and insoles and spinal orthosis group (n = 21). All participants used spinal orthoses fo

Outcomes following conservative treatment of extension fractures in the setting of diffuse idiopathic skeletal hyperostosis: is external orthosis alone a reasonable option?

OBJECTIVE: Extension fractures in the setting of diffuse idiopathic skeletal hyperostosis (DISH) represent highly unstable injuries. As a result, these fractures are most frequently treated with immediate surgical fixation to limit any potential risk of associated neurological injury. Although this represents the standard of care, patients with significant comorbidities, advanced age, or medical instability may not be surgical candidates. In this paper, the authors evaluated a series of patients with extension DISH fractures who were treated with orthosis alone and evaluated their outcomes. M

Twelve-week standard of care protocol longer than median time to normalization among IIc hips treated with Pavlik harness.

Best treatment protocols for infants with developmental dysplasia of the hip (DDH) are poorly defined. This study estimates the time to normalization among Graf IIc hips undergoing Pavlik harness treatment. Following institutional review board approval, patients referred for DDH evaluation at a pediatric institution between 2009 and 2018 (n = 1424 hips/712 patients) were identified. We isolated all Graf IIc hips that underwent Pavlik harness treatment (n = 132 hips/n = 106 patients). Demographic and outcome measures were collected. Normalization was defined as alpha angle greater than or equal

Alternative Noninvasive Treatment of Unstable Extra-articular Proximal and Middle Phalangeal Fractures: A Static Linear Traction Orthosis Design.

Extra-articular unstable proximal and middle phalanx fractures are typically managed with surgical means with common complications of tendon tethering, stiffness, and secondary hand dysfunction. As a result, alternative conservative measures are being explored. The use of static linear traction through the application of an orthosis allows for fracture reduction and anatomic healing, with successful range of motion outcomes.

Developmental Dysplasia of the Hip: A Review.

Developmental dysplasia of the hip (DDH) is the most common musculoskeletal disorder of the infant age. Its incidence ranges from 0.06/1000 to 76.1/1000 live births and is more frequent in female infants. Breech position, family history and firstborn children are the main risk factors for DDH and this disorder is also associated with the presence of other congenital deformities. Anatomically, the acetabulum remains shallow and the femoral head grows in a wrong position. Clinical examination is important and tests such us Barlow and Ortolani give indications only for a part of the spectrum of t

Suturectomies Assisted by Cranial Orthosis Remodeling for the Treatment of Craniosynostosis Can Be Performed Without an Endoscope.

BACKGROUND: Minimally-invasive endoscopic strip-craniectomy (or suturectomy) for the repair of craniosynostosis combined with postoperative cranial orthotic molding has been widely adopted in the past 2 decades, proving itself as a safe and effective procedure. Over time the authors transitioned from performing an endoscopic strip-craniectomy, to performing the same surgery without the endoscope. The authors here describe our technique and compare its results to those published in the literature for endoscopic suturectomies. METHODS: A retrospective chart review was performed for patients wit

Multidisciplinary integrated approach for older adults with symptomatic osteoarthritis: SIMFER and SI-GUIDA Joint Position Statement.

Multidisciplinary approach to osteoarthritis (OA) in older patients, whose functional decline is multifactorial and who present with multiple symptoms, has been long advocated, but it is still seldom implemented in daily practice. Therefore, further indications for the management of OA are eagerly awaited and should consider the specific clinical features of this population, including the presence of frailty and comorbidities. This clinical approach should be based both on well-grounded evidence and practical experience of experts in OA management. This manuscript comments the multidisciplinar
