A 28-year-old woman presented with a history of clicking and mandibular clenching. She was studied clinically and with axial and coronal CT. The patient was going through a tense emotional period and reported tightening of her teeth (clenching); she was under psychological and neurological treatment for depression including pharmacological therapy. We report here a case of arhinia with its complete manifestation, which is a very rare congenital abnormality. The newborn had complete absence of external nose with that area being totally flat and firm on palpation. A tracheostomy was performed on this child as the parents insisted on early discharge from the hospital without any reconstruction.

A 19-year-old male consulted our department, complaining of a painful scrotal mass. On palpation, the mass was discriminated from testis, epididymis, and spermatic cord in the left scrotum. He underwent total excision of the mass. Pathologic examination revealed cavernous hemangioma. Scrotal hemangioma is a relatively rare lesion and this is the 37th case reported in Japan.

A 40-year-old man was admitted with a 6-day history of fever and abdominal pain. His right upper quadrant was tender on palpation. A blood smear revealed trophozoite forms of Plasmodium vivax and Plasmodium falciparum. Abdominal ultrasound findings were consistent with acute acalculous cholecystitis. He was treated successfully with quinine and doxycycline and discharged in good clinical condition.

Inserting a thoracic drain is an invasive procedure which can cause several complications. We describe a new complication unreported to date: perforation-intubation of the subclavian vein. This case confirms the need to carefully choose the drainage point before insertion. Deformation of the thorax, especially in obese patients may require incision and digital palpation of the pleural space before insertion of the drain.

32 patients suffering from rectal carcinoma were examined by endorectal sonography. The preoperative staging by sonography was compared with digital palpation, colon enema and computertomography of the pelvis. In 90% of the cases the sonographic determination of the grade of invasion was identical to the histopathological examination. The efficiency of local destruction methods of rectal carcinoma (cryotherapy) can be controlled by this method.

A sixty-seven-year-old man found to have a seminoma of an undescended intra-abdominal testis is believed to be the oldest reported patient with this presentation. This case emphasizes the importance of careful palpation of the testis as part of the routine physical examination at any age. Testicular tumor or metastases should be included in the differential diagnosis of retroperitoneal mass.

A bacteriologic analysis of the gallbladder bile in eighty patients who underwent operation was performed in a prospective study. In all twenty patients with a normal gallbladder as assessed by oral cholecystography and intraoperative palpation, cultured were negative. Among patients with chronic cholecystitis only 30 per cent had positive bile cultures and this rose to 47 per cent among patients with acute cholecystitis.

In a child with painless rectal bleeding, a mass palpated during rectal examination is usually diagnosed initially as being a rectal polyp. Biceps tendon dislocation is an uncommon condition that may be overlooked due to its association with more common shoulder pathology. Diagnosis may be further obscured by the difficulty of palpating the biceps tendon in a muscular or obese patient. A patient with biparietal lesions demonstrated complex stereognostic hallucinations and recurrent haptic sensations. Although there was complete astereognosis and palpatory apraxia, she palpated the hallucinated objects appropriately. A second patient with ictal haptic hallucinations is described, and the one case previously reported in the literature. Some observations and speculations are made concerning these rare phenomena.
