Assessment of airway characteristics in Pierre Robin sequence using computed tomography-based 3-dimensional reconstruction and numerical simulation.

BACKGROUND: Pierre Robin sequence (PRS) syndrome has clinical features of small mandible, tongue drooping, and respiratory obstruction. This study compared the 3-dimensional (3D) reconstruction of the upper airway anatomy based on computed tomography (CT) data between PRS and normal control (NC) children, and analyzed the dynamical changes and airflow field using computer fluid dynamics (CFD) between the two populations. METHODS: A retrospective study was conducted on 66 PRS and 23 NC children, and all CT data were obtained and used for reconstruction and numerical simulation. 10 anatomical l

A review of tooth formation in children with cleft lip/palate.

The literature on tooth formation in children with cleft lip and/or palate is reviewed. The main focus of interest is the association of cleft type and dental abnormalities in number, size, shape, timing of formation, and eruption and cause of the abnormalities. The upper lateral incisor is the most susceptible to injury in the area of cleft in both deciduous and permanent dentitions. This tooth is affected in most instances, even in the cases of microforms of the cleft lip. The prevalence of hypodontia increases strongly with the severity of cleft. More teeth are congenitally missing from the

Does the inferior facial angle predict respiratory status in Robin sequence infants?

UNLABELLED: Robin sequence (RS) is a rare congenital condition characterized by retrognathia, glossoptosis, and upper airway obstruction, with highly variable respiratory severity at birth. Prenatal measurement of the inferior facial angle (IFA) is used to support diagnosis, but its prognostic value for respiratory outcomes remains uncertain. This study assessed whether fetal MRI-measured IFA predicts early respiratory status in infants with RS. We conducted a retrospective monocentric study including consecutive infants with prenatal suspicion and postnatal confirmation of RS who underwent fe

Comparative Effectiveness of Preepiglottic Baton Plates and Mandibular Distraction in Infants with Robin Sequence.

BACKGROUND: Mandibular distraction osteogenesis (MDO) and preepiglottic baton plates (PEBP) are both effective for early management of upper-airway obstruction (UAO) in infants with Robin sequence (RS), but have not been directly compared. The purpose of this study was to compare early airway, feeding, and growth outcomes between these treatments. METHODS: This is a bicentric retrospective cohort study from 2015 through 2021 including infants with RS treated with MDO or PEBP before 6 months of age with pretreatment and posttreatment sleep studies and follow-up at least through age 1 year. The

Does Mandibular Distraction Vector Influence Airway Volumes and Outcome?

PURPOSE: The goal of mandibular distraction in the Pierre-Robin sequence is to maximally expand the oropharyngeal airway. It has been hypothesized that a steep oblique distraction vector might allow greater airway enlargement compared with horizontal distraction. This study compared vector orientation in relation to airway volume and overall clinical outcome. MATERIALS AND METHODS: Micrognathic infants who underwent mandibular distraction with sufficient computed tomographic data were retrospectively reviewed. Demographic, diagnostic, perioperative, and distraction data were recorded. Groups

Three-dimensional assessment of facial development in children with Pierre Robin sequence.

Newborns with Pierre Robin sequence (PRS) have mandibular hypoplasia, glossoptosis, and possibly cleft palate. Their facial appearance is characteristic. The further facial development is controversial. The aim of this study was to analyze the facial development of children with PRS.In a prospective, cross-sectional study, 344 healthy children and 37 children with PRS and cleft palate younger than 8 years were scanned three-dimensionally. Twenty-one standard anthropometric landmarks were identified, and the images were superimposed. Growth curves for normal facial development were calculated.

Orofacial clefts in the National Birth Defects Prevention Study, 1997-2004.

Orofacial clefts are among the most common types of birth defects, but their clinical presentation has not been well described in a geographically diverse US population. To describe the birth prevalence and phenotype of nonsyndromic clefts, we used data from the National Birth Defects Prevention Study (NBDPS), a multi-site, population-based, case-control study aimed at identifying genetic and environmental risk factors for birth defects. Included in the study were infants born during 1997-2004 with a cleft lip (CL), cleft lip with cleft palate (CLP), or cleft palate (CP). Infants with clefts a

From birth to maturity: a group of patients who have completed their protocol management. Part II. Isolated cleft palate.

BACKGROUND: The optimal management of the cleft palate patient from birth to completion of treatment continues to present a formidable challenge to the plastic surgeon. The management by multidisciplinary teams is well established, but long-term outcome data of cases managed by protocol remain sparse. This study continues to present the results of the Australian Craniofacial Unit of patients with isolated cleft palate who completed protocol management at the unit under the care of the senior author (D.J.D.) during the 29-year period from 1974 to 2003. METHODS: A retrospective study of the out

Mandibular distraction osteogenesis in Pierre Robin sequence: application of a new internal single-stage resorbable device.

Pierre Robin sequence may result in physiologically significant obstructive apnea in the neonatal and infant period. This may be life threatening and is most often treated by tracheostomy. To avoid tracheostomy or allow for early decannulation in severely affected infants and children, the authors have developed a new class of neonatal and infant mandibular bone distraction devices. These devices require a single operative procedure for placement and no operative removal is necessary. Fifteen infants (aged 7 days to 11 months; mean age, 3 months) and five children (aged 2 to 8 years; mean age,

Flexible endoscopic intubation of the neonate.

The development of the ultrathin bronchoscope has provided a means of providing flexible endoscopic intubation for the neonate and small infant. We report our experience of 26 such endoscopic intubations in 23 neonates with birth weights from 1,200 to 4,600 g and post-conceptional ages varying from 31.5 to 60 weeks. The most common indications for the procedure were dysmorphic airways with variable degrees of micrognathia, acquired airway lesions, and severe degrees of hydrocephalus. With the use of the current 2.2 mm and 2.7 mm diameter instruments, it is now possible to utilize this techniqu
