Social inclusion and exclusion of people with mental illness in Timor-Leste: a qualitative investigation with multiple stakeholders.

BACKGROUND: Social inclusion is a human right for all people, including people with mental illness. It is also an important part of recovery from mental illness. In Timor-Leste, no research has investigated the social experiences of people with mental illness and their families. To fill this knowledge gap and inform ongoing mental health system strengthening, we investigated the experiences of social inclusion and exclusion of people with mental illness and their families in Timor-Leste. METHODS: Eighty-five participants from the following stakeholder groups across multiple locations in Timor

Modelling the health impact and cost-effectiveness of lymphatic filariasis eradication under varying levels of mass drug administration scale-up and geographic coverage.

BACKGROUND: A global programme to eliminate lymphatic filariasis (GPELF) is underway, yet two key programmatic features are currently still lacking: (1) the extension of efforts to all lymphatic filariasis (LF) endemic countries, and (2) the expansion of geographic coverage of mass drug administration (MDA) within countries. For varying levels of scale-up of MDA, we assessed the health benefits and the incremental cost-effectiveness ratios (ICERs) associated with LF eradication, projected the potential savings due to decreased morbidity management needs, and estimated potential household produ

Vaccines for fertility regulation of wild and domestic species.

The application of fertility control technologies for the management of animal populations is still a relatively new concept and remains to be proven on a large scale. While there have been demonstrations of its utility using crude antigenic preparations of porcine zona pellucida, refined, cheap and easily delivered vaccines are still in the early phases of development, especially those required for remote delivery to free-ranging wild animal populations. The relative slow emergence of such technologies for practical application clearly reflects the complexity of the science and the requiremen

Weaponized Virtue: Teachers' Kabayanihan (Heroism) as a Form of Ideological Control and the Normalization of Systemic Neglect in Philippine Education.

I examine how the teacher-as-hero trope, rooted in the Filipino concept of bayani, functions as a weaponized virtue. Although kabayanihan (heroism) traditionally signifies collective service to the bayan (community), it now operates as cultural hegemony, reframing systemic neglect as personal virtue and depoliticizing chronic underfunding and exploitative labour. Through neoliberal critique and structural violence frameworks, I trace how heroic narratives normalize invisible labour, intensify emotional demands, and sustain burnout as an institutional outcome rather than an individual failing.

Reimagining public understanding: Toward an institutional model.

Public understanding of science is often framed as either factual knowledge or generalized trust in experts. This article advances an alternative: an institutional model, grounded in cultural awareness of the norms, practices, and boundaries through which science produces credible knowledge. Existing approaches struggle to distinguish scientific controversy from broader social conflict, limiting their ability to explain why science's legitimacy falters in politicized contexts. An institutional perspective instead shifts the focus from individual deficits to science's role as a social system-em

Health equity follows racial equity: learning the impact of historic racism through a summer reading assignment in a graduate public health course.

Prior to a fall semester graduate course in public health, students are encouraged to complete a summer reading assignment based on a book chosen from a curated list of popular literature relevant to public health concepts. From 2020 to 2024, this assignment enabled students to explore the history of structural and systemic racism in the United States and its impacts on current public health policy and programs. Themes from these books are woven into class discussions and further explored in subsequent course assignments. This paper describes the assignment and shares responses from the 1,141

Pushing for Equity, Pulling at Heartstrings: Perinatal Care Access for Uninsured Migrants.

Despite widely accepted clinical care guidelines, uninsured pregnant migrants in Canada face health inequalities and there are ethical implications of denying them publicly-funded care. Uninsured migrants face multiple barriers (financial, systemic, and cultural) to their perinatal care access, which can negatively impact their health and psychosocial outcomes, as well as those of their families. Drawing on interviews with 10 uninsured migrant women in Montreal, Canada, we explore the factors that enabled their access to perinatal care. Our findings underline the importance of health professio

How warm are political interactions? A new measure of affective fractionalization.

Affective polarization measures account for partisans' feelings towards their own party versus its opponent(s), but not for how likely partisans are to encounter co-partisans versus out-partisans. However, the intensity of out-party dislike and the probability with which this comes into play both determine the social impact of cross-party hostility. We develop an affective fractionalization measure that accounts for both factors, and apply it to longitudinal survey data from 20 Western publics. From this perspective, countries with fewer dominant parties may be more harmonious because partisan

Snake antivenom production in Ecuador: Poor implementation, and an unplanned cessation leads to a call for a renaissance.

Snakebite envenomation is a global health problem. This health problem asymmetrically affects rural populations in developing countries to such an extent that it recently has been listed as a priority neglected tropical disease (NTD). It is estimated that 5.4 million individuals are bitten by snakes each year, causing at least 2.7 million envenomations and more than 100,000 deaths each year. Ecuador has one of the highest snakebite envenomation incidence rates in Latin America, mostly in the coastal and Amazonian provinces. Envenomations in these regions are the result of bites primarily by sp

Privilege and burden of im-/mobility governance: On the reinforcement of inequalities during a pandemic lockdown.

In order to contain the COVID-19 pandemic, nation states have focused on the governance of im-/mobilities: certain mobility restrictions have been enforced, while simultaneously some forms of mobility have been maintained or even enhanced in order to keep the system running in crisis mode. With a special focus on Austria, we analyse the specific politics of im-/mobilities concerning the organization of paid work and show how the socio-spatial conditions of who is permitted, denied or urged to work are inextricably linked to inequalities. It becomes apparent that while in principle all bodies a

Worldwide Access to Stereotactic Radiosurgery.

Stereotactic radiosurgery is a safe and effective technology that can address a variety of neurosurgical conditions, but in many parts of the world, access remains an issue. Although the technology is increasingly available in the United States, Canada, Europe, and parts of Asia, poor access to central nervous system (CNS) imaging and inadequate treatment equipment in other parts of the world limit the availability of radiosurgery as a treatment option. In addition, epidemiologic data about cancer and CNS metastases in low-income countries are sparse and much less complete than in more develop

The 2016 AANS Presidential Address: Leading the way.

This AANS presidential address focuses on enduring values of the neurosurgical profession that transcend the current political climate. The address was delivered by Dr. Batjer during a US presidential election year, but the authors have intentionally avoided discussing the current chaos of the American health care system in the knowledge that many pressing issues will change depending on the outcome of the 2016 elections. Instead, they have chosen to focus on clarifying what neurosurgeons, and the American Association of Neurological Surgeons, in particular, stand for; identifying important ch

Anthrax, toxins and vaccines: a 125-year journey targeting Bacillus anthracis.

Bacillus anthracis is the causative agent of anthrax, a disease that plagues both humans and various animal species. Effective vaccines are available, but those approved for human use are crude culture supernatants that require multiple injections and a yearly boost. Many experts agree that it is now time for the next generation of human vaccines against anthrax. Accordingly, this review will succinctly focus upon: pathogenesis of B. anthracis, with particular emphasis upon the immune system; the pertinent biophysical nature of protective antigen, which includes how the protein toxin component

The Sri Lanka tsunami experience.

The Indian Ocean tsunami of 2004 killed 31,000 people in Sri Lanka and produced morbidity primarily resulting from near-drownings and traumatic injuries. In the immediate aftermath, the survivors brought bodies to the hospitals, which hampered the hospitals' operations. The fear of epidemics led to mass burials. Infectious diseases were prevented through the provision of clean water and through vector control. Months after the tsunami, little rebuilding of permanent housing was evident, and many tsunami victims continued to reside in transit camps without means of generating their own income.

Learning lessons through disaster globalisation: Preparing and mitigating globally while responding locally.

The National Response Framework (NRF) provides a guideline on how the USA responds to disasters and emergencies.1 The NRF is designed as a tiered response such that incidents are handled at the lowest level. It states that it is built on scalable, flexible and adaptable concepts identified in the National Incident Management System.2 Globalisation, whether by its inherent structure or the effect of the disaster itself, no longer geographically limits the extent of today's disasters. Climate change, international economic integration, supply chain infrastructure, cyberattacks, infectious diseas

Louse-borne relapsing fever in Ethiopia: an urgent call for WHO recognition as a neglected tropical disease.

Louse-borne relapsing fever (LBRF), caused by Borrelia recurrentis and transmitted by the human body louse, remains a persistent health emergency in Ethiopia, resulting in preventable mortality among young adults. Its continued endemicity reflects systemic failures and amplifies the syndromic effects of poverty, displacement and social stigma-locally termed 'Qmalam'-which deters care-seeking. Clinical management is further complicated by the potentially fatal Jarisch-Herxheimer reaction following antibiotic treatment. Despite these severe consequences, LBRF remains off the official WHO list of

Struggling with exactitude in a fragmented state: Intelligence testing in early twentieth-century China.

This article examines the rise and decline of the enthusiasm for intelligence testing in early twentieth-century China, focusing on the appeal, the challenges, and the critiques revolving around this psychological instrument. The introduction of intelligence testing reflected not only China's urgent needs in modernizing its merit system, but also Chinese psychologists' aspirations for pursuing exactitude and redefining the racial characteristics of their compatriots against foreign interpretations. But despite psychologists' endeavors, the political and geographical fragmentation of Republican

Embedding viral hepatitis into primary healthcare: results of a strategic landscape analysis in Vietnam and the Philippines.

Chronic viral hepatitis is a significant public health concern in the Western Pacific, including in Vietnam and the Philippines. To accelerate progress toward meeting the 2030 elimination goals, the World Health Organization (WHO) encourages countries to adopt an integrated, people-centered health sector response to hepatitis, grounded in Primary Health Care (PHC). A review of the academic and grey literature, along with policy documents, was conducted to describe the national health system and PHC response to hepatitis B and C in Vietnam and the Philippines. Information was analyzed against t

This Is America: Systemic Racism and Health Inequities Amidst the COVID-19 Pandemic.

The Coronavirus 2019 (COVID-19) continues to devastate the world and the United States remains number one of reported COVID-19 cases and deaths. Research demonstrates that Blacks and Hispanics in the United States are disproportionately impacted by COVID-19, especially among highly marginalized people at the intersection of immigration and incarceration. Social distancing is a privilege and contact tracing is a deterrent for historically oppressed populations. Public health professionals have attempted a multicausal approach to prevent the spread of infectious diseases, but they have been unsu
