Cartilage-retaining wafer resection osteotomy of the distal ulna.

UNLABELLED: Ulnar-sided wrist pain resulting from ulnar impaction is common. We describe a new cartilage-retaining wafer resection osteotomy designed to keep the cartilage intact and decompress the ulnocarpal articulation without requiring internal fixation. We retrospectively reviewed seven patients with ulnar impaction who had the procedure. The minimum followup was 14 months (mean, 30 months; range, 14-38 months). The mean change in ulnar variance was -1.29 mm. Patients showed radiographic healing by a mean of 11 weeks. Our preliminary results suggest the cartilage-retaining wafer resection

Type four fracture of the medial epicondyle: a true indication for surgical intervention.

Fourteen consecutive cases with type 4 fracture of the medial epicondyle were evaluated following open reduction and internal fixation of the displaced medial epicondyle. The mean age was 9.7 years (range 6-16) and the mean follow-up was 17.2 months (range 12-24). Operative treatment yielded excellent results with no loss of functional range of motion, residual deformity or instability. There were three cases with pre-operative symptoms of ulnar nerve injury which made a good recovery following neurolysis of the ulnar nerve. Type 4 fractures are commonly associated with intra-articular entrapm

Outcome study of ulnar nerve compression at the elbow treated with simple decompression and an early programme of physical therapy.

Cubital tunnel syndrome is the second most common entrapment neuropathy of the upper limb. This paper presents the experience of treating cubital tunnel syndrome with simple decompression in 131 patients (164 ulnar nerves) over the past 12 years. 85% of these patients had mild or moderate ulnar nerve disease. In 146/164 ulnar nerves (89%), simple decompression resulted in good or excellent immediate post-operative relief of symptoms. After an average follow-up of 4.3 years (range, 0.8-12.0 years), 130/164 (79%) still reported good or excellent relief. The independent predictors of a better lon

Study of the ulnar nerve compromise at the wrist of patients with carpal tunnel syndrome.

INTRODUCTION: That the ulnar nerve compromise occurs concomitant with the carpal tunnel syndrome (CTS) has been cited by previous studies. It seems that the previously mentioned incidence is much higher than what we observe in our electrodiagnosis studies. MATERIAL AND METHOD: A prospective study was designed to evaluate the incidence of ulnar nerve compromise in patients with electrodiagnostic evidence of CTS according to age and sex, and also to determine the site of ulnar nerve involvement. RESULTS: One hundred and sixty five limbs with standard elestrodiagnostic criteria of CTS were eval

Carpal tunnel syndrome in fabry disease.

Carpal tunnel syndrome (CTS) is a common peripheral mononeuropathy affecting up to 4% of the general population, typically women in late middle age. The incidence in patients with Fabry disease (FD) is unclear, but may affect 25% of patients with this X-linked lysosomal storage disease. We report three cases of CTS in young Caucasian male patients with classical FD, who developed CTS symptoms with supportive nerve conduction study (NCS) findings. Two patients had bilateral CTS and two had evidence of concurrent ulnar nerve neuropathy on NCS, suggesting a systemic process contributed to nerve c

Peripheral nerve injuries in the athlete.

Peripheral nerves are susceptible to injury in the athlete because of the excessive physiological demands that are made on both the neurological structures and the soft tissues that protect them. The common mechanisms of injury are compression, traction, ischaemia and laceration. Seddon's original classification system for nerve injuries based on neurophysiological changes is the most widely used. Grade 1 nerve injury is a neuropraxic condition, grade 2 is axonal degeneration and grade 3 is nerve transection. Peripheral nerve injuries are more common in the upper extremities than the lower ext

Development and clinical significance of the musculus dorsoepitrochlearis in men.

Musculus dorsoepitrochlearis is a typical muscle variation, which, if in full extent, is represented by the muscular or fibromuscular slip detached from the anteroinferior border of the musculus latissimus dorsi. It passes over the axilla under the axillary fascia crossing the medial side of the brachial plexus and continues as a septum intermusculare mediale brachii distally to the medial epicondyle of humerus. Its full extent is rarely developed-the connection into the intermuscular septum being mostly absent. Muscular slips from the musculus latissimus then insert on various structures in t

Mononeuropathies associated with liver transplantation.

Mononeuropathies associated with orthotopic liver transplantation were evaluated in a prospective manner. Ten percent of liver transplant recipients were noted to have focal peripheral nerve lesions in the postoperative period. The ulnar nerve was most commonly involved, with intraoperative compression or postoperative trauma as possible mechanisms of injury. Other upper extremity mononeuropathies were likely a result of vascular cannulations. No brachial plexus injuries occurred. Diabetes and alcoholism were not risk factors for the development of a mononeuropathy.

Anterior submuscular transposition of the ulnar nerve in severe cubital tunnel syndrome. Personal experience.

AIM: The authors report the results of anterior submuscular transposition, on a clinical series of selected patients with severe cubital tunnel syndrome. In these patients, the degree of ulnar nerve compression was valued using a grading system that includes measurements of motor and sensitive function. In this series, a technique of flexor-pronator mass Z-lengthening was adopted. METHODS: From 1998 to 2002, 18 patients were surgically treated with anterior deep submuscular transposition for cubital tunnel syndrome. The preoperative status was determined using Dellon's classification. Fifteen

Normative values for high voltage electrical stimulation across the brachial plexus.

OBJECTIVES: To obtain normative values for High Voltage Electrical Stimulation across the brachial plexus between Erb's point, C8 root and T1 root. A case study of probable true thoracic outlet syndrome is used to illustrate the usefulness of the method. METHODS: 22 patients were tested for use in normative date pool with complaints unrelated to the ulnar nerve, the majority of which were pure carpal tunnel syndrome. High voltage stimulation was performed at Erb's point, C8 root and T1 root. Recording was from the abductor digit minimi muscle. RESULTS: Upper limit for absolute latencies were

Evoked responses in the diagnosis of thoracic outlet syndrome.

The lack of an objective method for diagnosing thoracic outlet syndrome and monitoring the results of treatment has prevented a clear understanding of its natural history and the indications for surgery. Although one can detect vascular compression, the majority of patients have symptoms that are primarily neurogenic; and neither electromyograms nor studies of nerve conduction velocity has proved consistently helpful in their evaluation. We have studied evoked responses in 22 patients and believe that this technique allows selection of patients with significant neural compression for operative

Superimposed Carpal Tunnel Syndrome in a Patient with Charcot-Marie-Tooth Disease Type 1A: Case Report and Diagnostic Considerations.

INTRODUCTION: We present a case of Charcot-Marie-Tooth disease type 1A (CMT1A) complicated by symptoms concerning for superimposed carpal tunnel syndrome (CTS). Diagnostic interpretation was challenging due to overlapping electrodiagnostic and ultrasound findings typical of CMT1A. CASE PRESENTATION: A 54-year-old woman with genetically confirmed CMT1A reported bilateral intermittent burning, numbness, and tingling in digits 1-3, worse at night and with hand use. Physical exam revealed positive Tinel's and Phalen's signs bilaterally with preserved motor strength and no thenar atrophy. Electrod

Median Nerve Compression in the Forearm: A Clinical Diagnosis.

Background: Median nerve entrapment in the forearm (MNEF) without motor paralysis is a challenging diagnosis. This retrospective study evaluated the clinical presentation, diagnostic studies, and outcomes following surgical decompression of MNEF. Methods: The study reviewed 147 patient medical charts following MNEF surgical decompression. With exclusion of patients with combined nerve entrapments (radial and ulnar), polyneuropathy, neurotmetic nerve injury, or median nerve motor palsy, the study sample included 27 patients. Data collected include: clinical presentation and pain, strength, prov

Non-invasive magnetoneurography for 3D-monitoring of human compound action current propagation in deep brachial plexus.

Compound action current (CAC) propagation along nerve fibers running deep in the human brachial plexus was 3D-visualized based on non-invasive 49-channel superconducting quantum interference device (SQUID) magnetoneurography. Spatio-temporal mappings over the upper thoracal quadrant of magnetic fields (<100 fT) evoked upon alternating median and ulnar nerve stimulation in seven healthy volunteers showed consistently smoothly propagating dipolar patterns for both the CAC depolarization and repolarization phases. Multipolar current source reconstructions (i) distinguished spatially CAC propagati

Contraceptive subcutaneous device migration: what does an orthopaedic surgeon need to know? A case report and literature review.

Subdermal contraceptive implant is approved in more than 60 countries and used by millions of women around the world. Although relatively safe in nature, their implantation and removal may be associated with potential complications, some of which may require surgical intervention. Two types of peripheral neurological complications are reported: complications related to compressive neuropathy caused by device decubitus and complications related to device improper removal. An healthy 35-year-old woman come to our attention for paresthesia from medial side of right elbow to fourth and fifth finge

Prevalence of cubital tunnel syndrome among dock workers, Saint Sebastian, São Paulo, Brazil.

BACKGROUND: Ports play a substantial role in the Brazilian economy.Despite the large number of port workers, few studies report that the most common musculoskeletal disorders among them involve the upper limbs, including carpal tunnel syndrome, rotator cuff syndrome, cervicalgia and cubital tunnel syndrome. The latter is a neuritis caused by compression of the ulnar nerve at the cubital tunnel (CuTS) on the elbow. OBJECTIVE: To estimate the prevalence of a suggestive diagnosis of CuTS among port workers. METHOD: Seventy-two independent port workers registered with the Labor Management Organ (

Ulnar Nerve Compression due to Anconeus Epitrochlearis: A Case Report and Review of the Literature.

CASE: A 32-year-old right-handed surgeon presented with a history of intermittent pain at the right medial epicondyle, a mild Tinel's sign, and dysesthesia in the ulnar nerve distribution. Dynamic ultrasound demonstrated a hypertrophic anconeus epitrochlearis bilaterally, and chronic irritation of the ulnar nerve. Anterior release with myectomy of the accessory muscle was performed. No compressive symptoms were present at 1-year follow-up. CONCLUSIONS: The anconeus epitrochlearis is an often-underappreciated cause of ulnar nerve compression that can lead to significant functional impairment.

Bony entrapment of ulnar nerve after closed forearm fracture: a case report.

We report a case of ulnar nerve palsy following forearm fracture in a 13-year-old girl. Significant anterior angulation and displacement of the ulna were noted. Operation was performed 3 months after the injury, when no recovery of numbness and claw hand deformity were demonstrated. Intra-operatively the ulnar nerve was found to be embedded between fragments of the fractured ulna, which showed lack of callus formation on the preoperative radiograph. The patient achieved complete recovery of sensory and motor functions 4 months after the surgery.
