Silent Pain or Silent Records? Pain Visibility, Documentation Ethics, and Nursing Management in Neurocritical Care.

Pain assessment in critical care is organised primarily around patient self-report and numerical scoring, an arrangement that protects the patient's first-person authority. In neurocritical care, however, critically ill patients with stroke may be unable to communicate because of aphasia, impaired consciousness, sedation, mechanical ventilation, or delirium. Under these conditions, the absence of a numerical score may reflect limitations in the systems through which pain is made visible rather than the absence of pain. This paper offers an empirically informed philosophical analysis of what a

Predicting ki-67 expression in breast cancer via transformer and multiple instance learning on DCE-MRI.

BACKGROUND: Accurate assessment of Ki-67 expression levels in breast cancer is crucial for determining prognosis and making informed treatment decisions. Current immunohistochemical methods relying on needle biopsy introduce sampling errors due to tumor spatial heterogeneity, making the development of non-invasive, precise preoperative prediction methods of significant clinical importance. This study aims to explore and compare advanced deep learning models based on dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) for noninvasive assessment of Ki-67 expression. METHODS: This ret

Predictors of adherence to a digital health application in patients with inflammatory arthritis: retrospective analysis.

Digital health applications (DHAs) can support self-management in inflammatory arthritis (IA), but sustained adherence remains challenging in real-world use. To determine demographic and clinical predictors of 12-week adherence to a rheumatology-specific mobile health application in patients with IA. We retrospectively analyzed 2,036 adult users of the CE-certified Mida Rheuma App (2022–2024). Adherence was defined as self-reported body weight entry at week 12. Candidate predictors included age, gender, time since diagnosis, baseline body weight, and two patient-reported outcomes (PROs): Patie

Using a mixed-reality headset to elicit and track clinically relevant movement in the clinic.

BACKGROUND: 21st century neurology will require scalable and quantitative tools that can improve neurologic evaluations over telehealth and expand access to care. Commercially available mixed-reality headsets allow for simultaneous presentation of stimuli via holograms projected into the real world and objective and quantitative measurement of hand movement, eye movement, and phonation. NEW METHOD: We created 6 tasks designed to mimic standard neurologic assessments and administered them to a single participant via the Microsoft HoloLens 2 mixed-reality headset. The tasks assessed postural ha

Impact of Genital and Non-Genital Psoriasis on Genital Self-Image, Quality of Life, and Sexual Dysfunction in Female Patients.

INTRODUCTION: Psoriasis is a chronic systemic inflammatory disease that affects millions of people worldwide. While its physical manifestations are well-documented, the psychosocial impact of psoriasis, particularly on female patients, is an area of growing concern. Feelings of shame, inferiority, and stigma are common among individuals with psoriasis, and these emotional burdens can be especially pronounced in females, particularly when the genital area is affected. OBJECTIVES: We aimed to shed light on the significance of genital psoriasis on the overall well-being of female patients. METH

Losing sight of Luck: Automatic approach tendencies toward gambling cues in Canadian moderate- to high-risk gamblers - A replication study.

Evidence for approach bias tendencies to underly automatic behavioural impulses towards seeking out gambling activities in the presence of appetitive salient cues was first shown by Boffo et al. (2018) in a Dutch sample. Relative to non-problem gamblers, moderate-to-high-risk gamblers demonstrated stronger approach tendencies towards gambling-related stimuli compared with neutral ones. Moreover, gambling approach bias was associated with past-month gambling behaviour and predictive of gambling activity persistence over time. The current study aimed to replicate these findings within a Canadian

Clinical Characteristics of Adult Functional Constipation Patients with Rectoanal Areflexia and Their Response to Biofeedback Therapy.

Background: The London Classification for anorectal physiological dysfunction specifically proposes rectoanal areflexia (RA), which means the absence of a rectoanal inhibitory reflex (RAIR) based on a manometric diagnosis. Although RA is not observed in healthy people, it can be found in adult patients with functional constipation (FC). This study describes the clinical manifestations of adult patients with FC and RA and their response to biofeedback therapy (BFT). Methods: This retrospective study reviewed the reports of high-resolution anorectal manometry (HR-ARM) and the efficacy of BFT in

Utility of St. George's respiratory questionnaire in predicting clinical recurrence in chronic pulmonary aspergillosis.

BACKGROUND AND AIMS: Patients with chronic pulmonary aspergillosis (CPA) who discontinue antifungal therapy commonly exhibit disease recurrence. We aimed to evaluate the utility of the St. George's respiratory questionnaire (SGRQ) in predicting the likelihood of clinical recurrence of CPA in patients who come off antifungal therapy. METHODS: This audit included CPA patients for whom antifungal therapy was discontinued for at least 1 month. Comparisons were made between the quality of life scores at the time of discontinuation of treatment and at the time of diagnosis of clinical recurrence. T

Systematic Development of an Intervention to Promote Self-Sampling for HIV and Sexually Transmitted Infections for Men Who Have Sex With Men: An Intervention Mapping Approach.

Sexual healthcare aims to reduce HIV and sexually transmitted infections (STIs) by promoting testing and prevention. To better reach men who have sex with men (MSM), additional strategies are needed. Here, we describe development of an intervention, which is part of a broader HIV/STI home-care program, targeted to reach MSM and motivate them to use self-sampling tests. Self-sampling includes blood sampling (finger prick) for HIV, hepatitis B, and syphilis, and a urine sample and oral and anorectal swab samples for chlamydia and gonorrhea. Intervention mapping, a systematic six-step approach, w
