Sagittal range of motion after extensive cervical fusion.

BACKGROUND CONTEXT: Complicated cervical spine revision and deformity correction surgeries are becoming increasingly common. These challenging operations often necessitate fusion of the entire cervical spine. Patients frequently express concern over the likely loss of range of motion (ROM) of the neck postoperatively. However, we are aware of no study that specifically examines the sagittal cervical ROM after extensive cervical fusion. PURPOSE: To characterize sagittal ROM after extensive cervical fusion. STUDY DESIGN: Retrospective case series. PATIENT SAMPLE: Thirty patients were included

Cervical Dermal Sinus Tract: A Case Report and Comprehensive Literature Review.

The cervical and thoracic dermal sinuses are rare entities, conforming epithelium-lined tracts that extend from an opening in the skin through a corridor to the layers of the spinal cord. They are commonly detected in early childhood; however, adult reports are singularly rare, especially in cervical regions. We report a very unusual case of a 45-year-old Mexican female who developed progressive left-side weakness and dexterity suffered from childhood, getting worse in the last year. Physical examination revealed a soft, congenital round cystic lesion in the dorsal-midline skin at the level of

Extraoral anatomy in CBCT – a literature review. Part 4: Pharyngocervical region.

This review about extraoral anatomy depicted in cone beam computed tomography describes the pharyngocervical region. Large (≥ 8 × 8 cm) field of views of the maxilla and/or mandible will inevita-bly depict the pharyngocervical region that com-prises the posterior upper airway, the pharyngeal part of the digestive tract, as well as the cervical segment of the spine. The latter consists of seven cervical vertebrae (C1-C7) with corresponding distinctive features, i.e., the atlas (C1) and the axis (C2). In addition, cervical vertebrae serve as ref-erences for the vertical position of anatomical st…

The orientation of the articular facets of the zygapophyseal joints at the cervical and upper thoracic region.

Knowledge of the orientation of facet joints in the cervical and upper thoracic region is important for understanding the biomechanical properties and clinical conditions relevant to the neck. The study was undertaken on dry macerated bones from 30 adult male human vertebral columns. The orientation of the superior articular facets in relation to their inclination with the sagittal and transverse planes was examined between C3 and T3 vertebrae in each column. The linear dimensions of the superior articular facets and the width/height ratio were also calculated. The results show that all verteb

A Specimen-Based Comparative MicroCT-FEA Analysis of Vertebral Trabecular Bone Microarchitecture and Mechanical Response in Two South American Cervids: The Patagonian Huemul (Hippocamelus bisulcus) and the Southern Pudu (Pudu puda).

The Patagonian huemul (Hippocamelus bisulcus) and the Southern pudu (Pudu puda) are native South American cervids that differ in body size, … under standardized axial compression. Both specimens exhibited consistent regional variation, with cervical vertebrae showing lower density and organization, and thoracic-lumbar vertebrae displaying denser trabecular networks. The Southern pudu specimen appeared to present a more homogeneous microarchitecture and a relatively uniform mechanical response along the v …

Pediatric cervical spine: normal anatomy, variants, and trauma.

Emergency radiologic evaluation of the pediatric cervical spine can be challenging because of the confusing appearance of synchondroses, normal anatomic variants, and injuries that are unique to children. Cervical spine injuries in children are usually seen in the upper cervical region owing to the unique biomechanics and anatomy of the pediatric cervical spine. Knowledge of the normal embryologic development and anatomy of the cervical spine is important to avoid mistaking synchondroses for fractures in the setting of trauma. Familiarity with anatomic variants is also important for correct im

Isolated atlas-duplication as a manifestation of persistent proatlas: a case report.

PURPOSE: Atlas-duplication is an exceedingly rare dysplasia of the craniocervical junction. To the best of our knowledge, only two cases of atlas-duplication have been reported and these were associated with complete anterior rachischisis and os odontoideum. We aimed to report a case of isolated atlas-duplication of incidental finding and without attributable symptoms which makes it unique. METHODS: Following a normal coronarography for a suspected myocardial infarction, a 60-year-old-man with no significant medical history developed a transient ischemic attack that justified brain computed-t

[Decapitation due to early post-mortem canine gnawing].

A 53-year-old male was found dead in his home. The deceased's head, almost totally skeletonized, was lying at a distance of about 150 cm from the thorax inlet. The other occupant of the flat was a mongrel sheepdog. The autopsy conducted for the inquest established extensive damage to soft tissue in the head, neck and chest. The second to sixth cervical vertebrae were missing. The seventh cervical vertebra and the right first rib displayed bone lesions. The tissue injuries were attributed in the diagnosis to post-mortem canine gnawing. Cause of death was given as intermittent haemorrhaging of t…

Clinical and radiological outcomes of posterior cervical decompression and fusion for severe cervical compressive-extension injury: A case series.

BACKGROUND: Although severe cervical compressive-extension (CE) injuries are usually repaired using a combined anterior-posterior approach, the repair is possible using a posterior approach alone with reliable anchors. This study aimed to present the outcomes and imaging analysis results of posterior cervical decompression and fusion (PCDF) for severe CE injuries. METHODS: We retrospectively reviewed 16 patients who underwent PCDF surgery for severe CE injuries (>50% subluxation) between January 2012 and December 2018. All patients completed 1-year follow-up, and their mean age at the time of

Micro-CT Anatomy of the Vertebral Column of the Luristan Newt (Neurergus kaiseri).

The Neurergus kaiseri is one of the native and endangered species of the Salamandridae family, which is restricted to the mountainous habitats of southern Lorestan and northern Khuzestan. The presence of this species in the environment and the risks surrounding the extinction and health of this species make it necessary to produce basic anatomical information. The present study investigated the typical morphological characteristics of normal, mature, and healthy Luristan newt (Neurergus kaiseri) vertebral column using a micro-CT scan. The samples were entered into the micro-CT scan machine one

Congenital variations of the upper cervical spine and their importance in preoperative diagnosis. A case report and a review of the literature.

Several variations of the bony and vascular anatomy around the first and second cervical vertebrae have been reported. Failure to recognise these variations can complicate operations on the upper cervical spine. We present a patient with recent onset of cervical myelopathy due to stenosis at the C3-4 level. Preoperative evaluation identified Klippel-Feil syndrome with cervical fusion of C2-3, aplasia of posterior arch of C1, anomalous vertebral artery course and a "ponticulus posticus" of C2. The combination of these variations in a Klippel-Feil syndrome patient has never been reported. Thus, …

Vertebral arteriovenous fistula due to blunt neck trauma: A case report.

The arteriovenous fistulas (AVFs) of the vertebral artery are usually caused by iatrogenic and penetrating traumas. Vertebral AVF is rarely seen after blunt cervical trauma. A 65-year-old male patient applied to the emergency clinic due to falling from a height of about 3 m. In his neurological examination, he had weakness in the left upper limb and tinnitus in the left ear. The cervical computed tomog-raphy examination showed a Hangman's fracture, a C3 vertebra corpus compression fracture, and a C1 anterior arch fracture. There was a separation on the C1-2 transverse foramen due to the fractu

Median labiomandibular glossotomy approach to the craniocervical region.

INTRODUCTION: In children as well as adults, adequate access to the craniocervical junction and upper cervical vertebra can usually be achieved with a transoral-transpalatopharyngeal route. However, when access is necessary to achieve the C5 level and the upper cervical spine in children, this is very difficult. This is particularly so when the incisor opening is less than 2.5 cm. The median labiomandibular glossotomy provides such an approach. MATERIALS AND METHODS: Our experience with five children is presented in a representative case: a 4-year-old male with a family history of spondyloepi

Regionalization of the vertebral column and its correlation with heart position in snakes: Implications for evolutionary pathways and morphological diversification.

Spinal regionalization has important implications for the evolution of vertebrate body plans. We determined the variation in the number and morphology of vertebrae across the vertebral column (i.e., vertebral formula) for 63 snake species representing 13 families using intracolumnar variation in vertebral shape. Vertebral counts were used to determine the position of the heart, pylorus, and left kidney for each species. Across all species we observed a conspicuous midthoracic transition in vertebral shape, indicating four developmental domains of the precloacal vertebral column (cervical, ante

Extracranial Segments of the Vertebral Artery: Insight in the Developmental Changes up to the 21st Year of Life.

The vertebral artery (VA) bilaterally arises usually from the subclavian artery and courses within the bony canals of cervical vertebrae, and then it reverses directions on the first vertebra before piercing the dura to enter the cranium. The aim was to follow (ab)normal developmental changes of extracranial VA from prenatal status to age 21. This chapter included a brief description of the arterial embryology and morphofunctional specificity of the VA in prevertebral, cervical, and atlantic parts, during prenatal and postnatal period. The authors concluded that the subclavian origin of the VA w…
