BACKGROUND: in this report of three new cases of superficial acral fibromyxoma (SAF), we discuss the clinicopathological features of this acral fibrous tumour as well as the differential diagnoses for hard nodules affecting the extremities.

PURPOSE: Although rarely indicated, surgical treatment of severe megaureter can pose a formidable technical challenge, especially in the small infant. We present our experience and outcomes with end cutaneous ureterostomy as a temporizing adjunct to future ureteral reimplantation.

Dupuytren's disease may present with well-defined subcutaneous cords or as more diffuse disease with involvement of the skin. Fasciectomy is the procedure commonly carried out for the full range of disease, but is associated with rates of recurrence of up to 66%. We reviewed 143 rays in 103 patients undergoing dermofasciectomy for diffuse disease with involvement of the skin. We found recurrence in 12 rays (8.4% of rays; 11.6% of patients) during a mean follow-up of 5.8 years, eight as cords and four as nodules. Described is a case of laparoscopically assisted continent catheterizable cutaneous appendicovesicostomy in a 15-year-old female patient with bilateral ectopic ureteroceles and a totally obliterated bladder neck. The appendix and cecum were mobilized and dissected entirely via laparoendoscopic techniques. The nonrefluxing appendiconeocystostomy required an open cystotomy. Tumescent local anesthesia (TLA) offers many advantages in major and minor dermatosurgical procedures, especially in elderly and multimorbid patients in whom general anesthesia is sometimes associated with major risks. LA intolerances are rare. In our experience, TLA is a safe method despite the mixture of different LA. Monitoring of the patient during TLA is necessary. Clarification of a history of LA intolerance prior to surgical treatment is useful; however, a negative test does not reliably rule out an allergic reaction.

The irritating effects of concrete (calcium oxide) on skin have long been known. The effect by long-term skin exposure is not only irritating but also erosive and alkali skin erosion may result due to colliquative skin necrosis. A complicated course may ensue potentially necessitating plastic reconstructive treatment due to the development of unstable scars and defects. Correct interpretation of the skin trauma and adequate treatment are mandatory for functional restitution.
