The Opioid Epidemic and Perinatal Health.

Opioid misuse, opioid use disorder (OUD), and opioid overdose continue to be significant contributors to perinatal morbidity and mortality, including in the postpartum period. Medications for OUD including methadone and buprenorphine reduce the risk of pregnancy-associated overdose and improve perinatal outcomes. Newer medications and medication formulations are currently being investigated to provide additional treatment options. Modifications to obstetric and pediatric management may be required to best support maternal, fetal, and neonatal outcomes. Harm reduction tools, including Narcan an

Is England facing an opioid epidemic?

The opioid crisis in the United States (US) is one of the most high-profile public health scandals of the 21st century with millions of people unknowingly becoming dependent on opioids. The United Kingdom (UK) had the world's highest rate of opioid consumption in 2019, and opiate-related drug poisoning deaths have increased by 388% since 1993 in England and Wales. This article explores the epidemiological definitions of public health emergencies and epidemics in the context of opioid use, misuse, and mortality in England, to establish whether England is facing an opioid crisis.

Epidemic retinitis during pregnancy.

PURPOSE: To study the clinical presentation and treatment outcome of epidemic retinitis (ER) during pregnancy. METHODS: This is a retrospective, observational chart review of pregnant patients diagnosed with ER from January 2014 to February 2023. Demographic details, month of pregnancy at the onset of ocular symptoms, history of present illness, clinical manifestations, and treatment outcomes were studied. RESULTS: In 9 years, ER was seen in 86 females, of whom 12 (13.9%) were pregnant. Twenty-one eyes of those 12 patients were studied. Most of the patients presented in the sixth month of pr

Epidemic retinitis and community outbreaks.

PURPOSE: The objective of this study was to correlate seasonal variation of epidemic retinitis (ER) with concurrent community outbreaks. METHODS: This is a retrospective, observational, comparative study conducted in a tertiary care eye hospital in south India. Monthly variation in number of ER cases in comparison with reported community outbreaks by Integrated Disease Surveillance Program (IDSP) from 2009 to 2020 in the same region were studied. Month-wise graphs against number of patients were plotted for ER and for each community outbreak. RESULTS: ER was diagnosed in 163 patients. Diagno

The Global Epidemic of the Metabolic Syndrome.

Metabolic syndrome, variously known also as syndrome X, insulin resistance, etc., is defined by WHO as a pathologic condition characterized by abdominal obesity, insulin resistance, hypertension, and hyperlipidemia. Though there is some variation in the definition by other health care organization, the differences are minor. With the successful conquest of communicable infectious diseases in most of the world, this new non-communicable disease (NCD) has become the major health hazard of modern world. Though it started in the Western world, with the spread of the Western lifestyle across the gl

The Opioid Epidemic: Crisis and Solutions.

The widespread abuse of prescription opioids and a dramatic increase in the availability of illicit opioids have created what is commonly referred to as the opioid epidemic. The magnitude of this epidemic is startling: About 4% of the adult US population misuses prescription opioids, and in 2015, more than 33,000 deaths were attributable to overdose with licit and illicit opioids. Increasing the availability of medication-assisted treatments (such as buprenorphine and naltrexone), the use of abuse-deterrent formulations, and the adoption of US Centers for Disease Control and Prevention prescri

The Opioid Epidemic in the United States.

There is an epidemic of opioid abuse. This article discusses the history of opioid use. Abusers of opioids are at great risk of harm. There have been increasing legislative efforts to curb this abuse and we present a review of the current state of these laws. Naloxone has made a profound impact in the care of these patients if they present for medical care early enough. This paper discusses naloxone pharmacodynamics, its use in the medical setting, and how its use is now being expanded to include nontraditional providers with take home naloxone programs.

Chikungunya in the Caribbean: An Epidemic in the Making.

Chikungunya is a mosquito-borne virus that has shown increased prevalence in the Caribbean since October 2013. There have been several outbreaks throughout Asian and African countries over the past few decades with global travel and tourism having a major impact on the further spread of this disease. Improved policies and practices for preventative measures and epidemiological surveillance must be implemented to prevent the continued transmission of chikungunya.

Opioid epidemic in the United States.

Over the past two decades, as the prevalence of chronic pain and health care costs have exploded, an opioid epidemic with adverse consequences has escalated. Efforts to increase opioid use and a campaign touting the alleged undertreatment of pain continue to be significant factors in the escalation. Many arguments in favor of opioids are based solely on traditions, expert opinion, practical experience and uncontrolled anecdotal observations. Over the past 20 years, the liberalization of laws governing the prescribing of opioids for the treatment of chronic non-cancer pain by the state medical
