The proton nuclear magnetic resonance spectrum of sulfmyoglobin prepared in standard fashion reveals the presence of three forms, A, B, and C, with different chemical reactivity. Conditions for some interconversions of these forms are given. The 1H NMR spectra of the different forms show similar patterns. It appears that the differences between forms involve chemical modification on the porphyrin periphery. The altered heme can be extracted from FeIII(CN) sulfmyoglobin C to give a stable green substance.

Recent research has contributed to the understanding of nontraumatic rhabdomyolysis. In cases associated with coma, the pathophysiology is secondary to local pressure necrosis caused by compression from the patient's own body. The local pressure results in an edema-ischemia cycle concluding with compartmental tamponade and muscle breakdown. Rhabdomyolysis is well known in adults and can be either traumatic (crush syndrome) or ischemic in origin. Hereditary, metabolic, or toxic causes have also been described. Quantitation of cytochrome c oxidase in complex systems such as tissue homogenates is often hampered by the presence of other hemoproteins. Cyanide can bind to reduced cytochrome c oxidase from diverse sources with a dissociation constant in the range of 0.1-0.5 mM and induces a characteristic optical change. This contrasts with the very weak binding of cyanide to reduced forms of many other hemoproteins, including hemoglobin and myoglobin. Acute aortic occlusion is a rare but catastrophic disease with a high mortality rate. Severe perioperative complications could result from revascularization of infarcted muscles. Muscle cell ischaemia and massive volume cell death lead to the release of myoglobin, potassium, and lactic acid, which could be fatal if not recognised or treated early.
