What are relevant predictors of physical activity in older adults with lower limb loss (LLL)? Results of a retrospective analysis.

BACKGROUND: People with lower limb loss (LLL) have reduced physical activity (PA). There is evidence of physical and psychosocial predictors of PA in older adults with limb loss. However, these 2 areas (physical/psychosocial) have not been evaluated in the same analysis. OBJECTIVES: To describe and identify predictors of PA in individuals with LLL. STUDY DESIGN: Cross-sectional study. METHODS: Secondary analysis of data from a multisite Canadian randomized control trial involving community-dwelling prosthetic ambulators with unilateral transtibial or transfemoral amputation (N = 72). Influence of Depression on Pain and Disability in Patients with Chronic Low Back Pain after Physical Therapy: A Secondary Analysis of a Randomized Controlled Trial.

BACKGROUND: Depressive complications in chronic pain are detrimental to rehabilitation. This study was aimed at determining the influence of the presence of depressive symptoms on the efficacy of physical therapy among participants with chronic low back pain (CLBP). METHODS: Data was collected from a randomized controlled trial on 113 participants with CLBP. Participants were reallocated into the depressed or nondepressed groups based on the 50-cutoff point of the self-rating depression scale. All patients received 60 min sessions of physical therapy twice a week for 12 weeks. The primary out

Association of surgical timing with 30-day independent ambulation and postoperative sleep disorder in older adult patients undergoing joint replacement: an exploratory secondary analysis of a prospective cohort.

OBJECTIVE: This exploratory study investigated the observational association between surgical timing (morning versus afternoon) and 30-day independent ambulation in older adult joint replacement patients, and whether postoperative sleep disorder (PSD) mediates this association. METHODS: This was an exploratory secondary analysis of a prospectively registered cohort. Patients aged ≥ 65 years undergoing joint replacement were included. Surgical timing was the exposure, 30-day independent ambulation the primary outcome, and PSD the potential mediator. Propensity score matching (PSM) was used to

The prevalence and characteristics of unrecorded alcohol consumption: a secondary analysis of cross-sectional surveys in 11 countries in sub-Saharan Africa.

OBJECTIVE: Empirical evidence on unrecorded alcohol (alcohol produced, distributed and sold outside government oversight) remains limited in sub-Saharan Africa, despite its contribution to total alcohol consumption in many low- and middle-income countries. We estimated the prevalence and main types of unrecorded alcohol consumption; investigated sociodemographic characteristics of its use; and examined differences in drinking patterns between consumers of recorded and unrecorded alcohol across 11 sub-Saharan African countries. METHODS: We conducted a cross-sectional analysis of nationally rep

The effect of metformin on perinatal outcomes in pregnant women with type 2 diabetes differs by baseline insulin requirements.

BACKGROUND: Metformin works by increasing insulin sensitivity, which may aid in the management of insulin-treated type 2 diabetes in pregnancy. However, large trials have found that adjunctive metformin did not reduce composite adverse neonatal outcomes in this population. OBJECTIVE: Our objective was to determine if baseline insulin requirements modified the effect of metformin on adverse neonatal and maternal outcomes. STUDY DESIGN: We performed a secondary analysis of the Medical Optimization and Management of Pregnancies with Overt Type 2 Diabetes trial, a randomized controlled trial of

The Azygos Esophageal Recess Is Not to Be Missed in Screening Lung Cancer With LDCT.

PURPOSE: Lesion overlooking and late diagnostic workup can compromise the efficacy of low-dose CT (LDCT) screening of lung cancer (LC), implying more advanced and less curable disease stages. We hypothesized that the azygos esophageal recess (AER) of the right lower lobe (RLL) might be an area prone to lesion overlooking in LC screening. MATERIALS AND METHODS: Two radiologists reviewed the LDCT examinations of all the screen-detected incident LCs observed in the active arm of 2 randomized clinical trials: ITALUNG and national lung screening trial. Those in the AER were compared with those in

The Lioness™ device: a prospective non-randomized safety and feasibility study of a novel device for preterm birth delay and prevention.

BACKGROUND: Preterm birth is a leading cause of neonatal mortality and morbidity. Existing interventions to reduce preterm birth risk, such as progesterone therapy, cervical cerclage, and the Arabin pessary, demonstrate limited efficacy, particularly in women with twin pregnancies or a history of preterm birth. The Lioness™ is an elastic silicone ring, with six supporting arms and an encompassing band placed around the cervix at the site of the Shirodkar cerclage, promoting cervical elongation and helping to maintain it throughout pregnancy. It aims to reduce the risk of preterm birth by provi

Assessing immunization coverage in Southern togo: implications for perennial malaria chemoprevention delivery.

BACKGROUND: Malaria remains a major health challenge in sub-Saharan Africa, especially for children under five. The World Health Organization (WHO) recommends perennial malaria chemoprevention (PMC) to children in areas of medium to high perennial transmission. In Togo, since 2022, a pilot project has leveraged the Expanded Program on Immunization (EPI) to deliver PMC to children under two years; however, the extent to which PMC achieves its desired outcome may depend on EPI performance. This study assessed EPI coverage and factors associated with zero-dose status in southern Togo. METHODS: W

Topological and mutational analysis of Saccharomyces cerevisiae Fks1.

Fks1, with orthologs in nearly all fungi as well as plants and many protists, plays a central role in fungal cell wall formation as the putative catalytic component of β-1,3-glucan synthase. It is also the target for an important new antifungal group, the echinocandins, as evidenced by the localization of resistance-conferring mutations to Fks1 hot spots 1, 2, and 3 (residues 635 to 649, 1354 to 1361, and 690 to 700, respectively). Since Fks1 is an integral membrane protein and echinocandins are cyclic peptides with lipid tails, Fks1 topology is key to understanding its function and interactio

Amino acid sequence of fibrolase, a direct-acting fibrinolytic enzyme from Agkistrodon contortrix contortrix venom.

The complete amino acid sequence of fibrolase, a fibrinolytic enzyme from southern copperhead (Agkistrodon contortrix contortrix) venom, has been determined. This is the first report of the sequence of a direct-acting, nonhemorrhagic fibrinolytic enzyme found in snake venom. The majority of the sequence was established by automated Edman degradation of overlapping peptides generated by a variety of selective cleavage procedures. The amino-terminus is blocked by a cyclized glutamine (pyroglutamic acid) residue, and the sequence of this region of the molecule was determined by mass spectrometry.

Prevalence, Trends, and Associated Factors of Malnutrition Among Under Six-Month-Old Infants in Bangladesh: A Pooled Analysis of DHS Data (2011-2022).

Malnutrition among infants aged under 6 months (u6m) is a global public health problem. As countries begin implementing 2023 WHO malnutrition guidelines, local prevalence and epidemiology must be well understood. We analysed Bangladesh Demographic Health Survey data (2011-2022), describing the prevalence and time trends of infant u6m wasting (weight-for-length z-score [WLZ] < -2), stunting (length-for-age z-score [LAZ] < -2), and underweight (weight-for-age z-score [WAZ] < -2). In bivariate and multivariate analyses, we tested the association between wasting and underweight and established ris

Impact of a Reproductive Health Survivorship Care Plan on Fertility, Pregnancy Concerns, and Accessing Reproductive Healthcare Among Young Nulliparous Breast Cancer Survivors.

BACKGROUND: Young patients with breast cancer frequently receive chemotherapy and/or endocrine therapy that adversely affect ovarian function, leading to fertility, pregnancy, and other reproductive health concerns. Despite available evidence-based management strategies, dissemination to survivors and healthcare providers remains limited, resulting in substantial unmet informational and care needs. Web-based survivorship care plans may offer an effective approach to address these gaps. AIMS: To evaluate the effect of a web-based Reproductive Health Survivorship Care Plan (SCP-R) on reproducti

Treatment for co-occurring posttraumatic stress disorder and substance use disorders among veterans and civilians: A test of causally moderated comparative effectiveness.

OBJECTIVE: The prevalence and negative sequelae of co-occurring posttraumatic stress disorder and substance use disorder (PTSD + SUD) are well documented. Findings from evidence syntheses regarding which PTSD + SUD treatments work best overall have been mixed, potentially because these treatments are not equally effective for all PTSD + SUD patient subgroups. Epidemiological and qualitative research suggest that veteran status may be related to differential PTSD + SUD treatment response. This study, applying recent quantitative methodological advances in evidence synthesis, is the first to exa

Expanding the scope of the withdrawal syndrome: Anhedonia as a core nicotine withdrawal symptom.

Some evidence suggests that anhedonia is a component of nicotine withdrawal, but additional research is needed to support this conclusion and establish its clinical relevance. Secondary analyses were conducted for a comparative effectiveness smoking cessation clinical trial of combination nicotine replacement therapy, nicotine patches, and varenicline (N = 1,084). Self-reported consummatory anhedonia was assessed at multiple time points pretarget quit day (TQD) and post-TQD, and biochemically confirmed 7-day point-prevalence smoking abstinence was assessed at 4, 12, and 26 weeks post-TQD.
