Reverse total shoulder arthroplasty for cuff tear arthropathy: the clinical effect of deltoid lengthening and center of rotation medialization.

BACKGROUND: Reverse total shoulder arthroplasty (RSA) for cuff tear arthropathy improves shoulder function and reduces pain. Implant position and soft tissue balancing are important factors to optimize outcome. Tensioning the deltoid and increasing the deltoid moment arm by medializing the center of rotation are biomechanically advantageous. The purpose of this study was to correlate RSA functional outcomes with deltoid lengthening and center of rotation medialization. MATERIALS AND METHODS: This prospective cohort study enrolled 49 consecutive patients who underwent RSA for cuff tear arthrop

Total shoulder arthroplasty with an uncemented soft-metal-backed glenoid component.

BACKGROUND: Loosening associated with cemented polyethylene glenoid components is a major concern following total shoulder arthroplasty (TSA). The purpose of this study was to investigate the clinical and radiographic results associated with use of a novel uncemented soft-metal-backed glenoid component (SMBG), with a minimum follow-up of 2 years. MATERIALS AND METHODS: Twenty-two patients (19 women) underwent TSA using a uncemented SMBG. The mean age was 68.5 years (range, 49-84). Mean follow-up was 50 months (range, 24-89). Indications for TSA were primary osteoarthritis (10), post-traumatic

Fracture of a reverse total shoulder arthroplasty retentive liner.

Reverse shoulder arthroplasty may be indicated in shoulders that cannot be effectively managed by conventional reconstruction procedures. Reported complications include instability, infection, hematoma, scapular notching, and premature component loosening. To our knowledge, however, fracture of a humeral component polyethylene inlay has not been described. A 72-year-old man presented with unstable, painful shoulders after undergoing unsuccessful open repairs and acromioplasties at an outside institution for bilateral massive rotator cuff tears. We performed a left reversed shoulder arthroplast

Salvage of major joint prostheses in aggressive non-specific panniculitis.

Panniculitis is a relatively rare condition, which is usually treated non-operatively. We present a case of panniculitis causing an acute septicaemic episode and threatening the integrity of underlying joint prostheses. The situation was salvaged by radical debridement of the affected areas, and reconstruction using split-skin grafts. Considerations in the management of patients with this condition are discussed.

Central screw use delays implant dislodgement in osteopenic bone but not synthetic surrogates: A comparison of reverse total shoulder models.

Adequate glenoid baseplate fixation in reverse total shoulder arthroplasty (rTSA) is important to achieve, but may prove challenging in the context of glenoid bone loss or osteopenia. Current rTSA testing standards rely upon synthetic bone surrogates, but it is unclear if these models accurately recapitulate the mechanics of osteoporotic bone. Additionally, it also unknown if the use of a central screw effectively provides resistance to micromotion in the milieu of poor quality bone. The purpose of this experiment was to create a novel cyclic load test protocol that elicited clinically relevan

Bony increased-offset-reverse shoulder arthroplasty: 5 to 10 years' follow-up.

BACKGROUND: Glenoid lateralization has been shown to be the most imortant factor in maximizing passive range of motion and shoulder stability while preventing scapular impingement and notching. We aimed to evaluate mid- to long-term functional and radiologic outcomes after bony increased-offset-reverse shoulder arthroplasty (BIO-RSA) using a Grammont-style (medialized) humeral implant. METHODS: The study included 143 consecutive shoulders (140 patients; mean age, 72 years) treated with a BIO-RSA for rotator cuff deficiency. A cylinder of autologous cancellous bone graft, harvested from the hu

Short- or long-stem prosthesis for intramedullary bypass of proximal humeral fractures with severe metaphyseal bone loss: evaluation of primary stability in a biomechanical model.

BACKGROUND: Proximal humeral fractures with substantial metaphyseal comminution are challenging to treat. In the elderly with osteoporotic bone, arthroplasty sometimes remains the only valuable option; however, the minimally required length of stem fixation is not known. The aim of this study was to investigate the primary stability of cemented short- and long-stem prostheses with different intramedullary fracture bypass lengths. MATERIALS AND METHODS: Osteoporotic composite bone models of the humerus (Synbone, Malans, Switzerland) with 3 different fracture levels (group A, 6 cm distal to sur

[Treatment outcome after primary hemi-alloarthroplasty in dislocated humeral head fractures].

Humeral head fractures often lead to a high complication rate because of vulnerable vascularization of the humeral head, pretraumatical soft tissue lesions and osteoporotic bone. Compared with open reduction and internal fixation primary hemiarthroplasty seems to be an encouraging alternative treatment in case of dislocated and comminuted fractures. To evaluate our results following primary hemiarthroplasty we examined the outcome of 27 patients treated between November 1993 and May 1997 by primary hemiarthroplasty (15 Neer II, 12 Aequalis) in our institution within two weeks after trauma. The

The lever arm in glenohumeral abduction after hemiarthroplasty.

Fourteen cases of hemiarthroplasty for four-part fractures of the proximal humerus were reviewed. Pain relief was satisfactory, but function was limited, mainly due to loss of glenohumeral abduction despite electromyographic proof of actively contracting abductors in all cases. Analysis of special radiographs of nine cases showed a direct relationship between the clinical results and the "humeral offset", or distance between the geometric centre of the humeral head and the lateral aspect of the greater tuberosity. This offset affects the lever arms of the glenohumeral abductor muscles. The imp

Mid- and long-term results of primary vs. secondary total elbow arthroplasty after index intra-articular fracture of the distal humerus in the elderly.

BACKGROUND: Geriatric distal humerus fractures are difficult to manage. Open reduction and internal fixation remain the standard, but total elbow arthroplasty (TEA) appears to be a viable alternative. This study evaluated the long-term outcome after primary TEA in comparison to secondary TEA. METHODS: In this retrospective cohort study with prospective follow-up (FU), patients undergoing TEA after complex distal humerus fractures were analyzed as either primary TEA (index treatment; n = 21) or secondary TEA (salvage procedure after failed fracture treatment; n = 13). Data extracted from the m

Patient-specific BIO-RSA (bony increased-offset reverse shoulder arthroplasty) for glenoid dysplasia (type C) osteoarthritis.

BACKGROUND: Glenoid dysplasia (Walch type-C) is characterized by a retroversion > 25° and predisposes the shoulder to degenerative osteoarthritis. Our purpose is to report the mid- to long-term clinical and radiologic outcomes of reverse shoulder arthroplasty (RSA) with a patient-specific structural humeral head autograft (bony increased-offset RSA [BIO-RSA] technique) for the treatment of glenoid dysplasia (Walch type C) osteoarthritis (OA). Free muscle transplantation combined with intercostal nerve crossing for reconstruction of elbow flexion and wrist extension in brachial plexus injuries.

Complete paralysis due to traumatic brachial plexus injury is extremely difficult to treat when the injury affects whole nerve roots and when motor function fails to show any signs of recovery. Seddon has suggested that arthrodesis of the shoulder and amputation at the humerus, combined with the use of a functional upper extremity prosthesis, was the most practical procedure available. Since 1965, in cases of irreparable lesions such as complete root avulsion type injuries, we have performed direct intercostal nerve crossing to the musculocutaneous nerve without free nerve graft to achieve elb

Custom-made wrist prothesis (UNI-2™) in a patient with giant cell tumor of the distal radius: 10-year follow-up.

Wrist endoprosthesis is still an exceptional indication compared to the prosthesis of large joints, especially when a rescue surgery procedure of the wrist is no longer possible and the complete wrist arthrodesis is to be considered the ultima ratio. However, a suspended function in the wrist is accompanied by a significant restriction in the patient`s daily life. Using the fourth-generation endoprosthesis, the situation regarding long-term results for the hand has already improved. This means that a durability of more than 6 years is no longer a rarity. Defect situations in joints, in particu

Preliminary results after reconstruction of bony defects of the proximal humerus with an allograft-resurfacing composite.

We retrospectively studied 14 patients with proximal and diaphyseal tumours and disappearing bone (Gorham's) disease of the humerus treated with wide resection and reconstruction using an allograft-resurfacing composite (ARC). There were ten women and four men, with a mean age of 35 years (8 to 69). At a mean follow-up of 25 months (10 to 89), two patients had a fracture of the allograft. In one of these it was revised with a similar ARC and in the other with an intercalary prosthesis. A further patient had an infection and a fracture of the allograft that was revised with a megaprosthesis. In

[How to treat massive rotator cuff tears].

The treatment of massive rotator cuff tears must be adapted to the patient's individual needs and preoperative parameters to achieve the best outcome. First, the shoulder surgeon has to determine whether a direct transosseous repair is possible. If there is not enough remaining tissue, the tissue is atrophic, and the tendon stump can be reduced only with great tension, one can use a margin convergence technique for partial closure, perform a biceps tendoplasty, or perform local tendon transfers with the subscapularis or infraspinatus muscle. If the defect cannot be sufficiently closed, elderly

Lateralization of the glenosphere in reverse shoulder arthroplasty decreases arm lengthening and demonstrates comparable risk of nerve injury compared with anatomic arthroplasty: a prospective cohort study.

HYPOTHESIS: Grammont-style reverse shoulder arthroplasty (RSA) has an increased risk of nerve injury compared with anatomic total shoulder arthroplasty (TSA) due to arm lengthening. We hypothesized that an RSA with a lateralized glenosphere and 135° neck-shaft angle would reduce humeral lengthening and decrease the risk of nerve injury to the level of a TSA. METHODS: The study prospectively enrolled 50 consecutive patients undergoing RSA (n = 30) or TSA (n = 20) as determined by a power analysis based on previous research for our institution. Intraoperative neuromonitoring was used to detect

The TESS reverse shoulder arthroplasty without a stem in the treatment of cuff-deficient shoulder conditions: clinical and radiographic results.

BACKGROUND: Reverse total shoulder arthroplasty (RSA) is a recent concept that enables good functional outcomes in cases of massive rotator cuff tear and cuff tear arthropathy. Design parameters influence the functional results and complications. The purpose of this study is to present the results of a novel RSA, the Total Evolutive Shoulder System (TESS; Biomet, Warsaw, IN, USA), based on a reverse corolla without a stem. METHODS: We enrolled 101 patients with 105 RSAs in a prospective study, with a minimum follow-up period of 24 months. The analysis concerned 91 RSAs in 87 patients (61 men
