Neurological sequelae of cervical spine trauma.

It is estimated that there will be 5,000 new spinal cord injured patients each year. Prolonged survival has resulted from better understanding of the pathophysiology of cord damage, as well as from the advances in antibiotic and ventilatory therapy. Most well-motivated patients with lesions at or below C5 can attain a level of independence which obviates institutional care. Regional spinal injury centers have done much to increase the survival of and quality of life of cord injured patients. These realities make initial care of patients with acute traumatic lesions of the cervical spine of par…

Cervical spine injuries. Diagnosis and classification.

Roentogenographic techniques in the evaluation of the patient with a cervical spine injury entail several considerations that should be respected in defining and classifying the lesion according to the mechanism of injury. Unilateral or bilateral facet dislocation with separation of posterior elements usually implies disruptive flexion injury. Anterior wedging of a vertebral body indicates flexion with some degree of compression. Comminution of the vertebral body indicates a predominant compressive element to the injury. An anterior-inferior marginal fracture indicates extension injury. Impact…

Multiple spine fractures in ankylosing spondylitis.

1. Concomitant cervical and thoracic spine fractures are reported in a patient with advanced ankylosing spondylitis (second reported case). 2. The thoracic T9-10 interspace fracture-dislocation was grossly unstable, as well as the C7-T1 fracture-dislocation. 3. The higher incidence of cord injury following spinal fracture-dislocations in these patients is discussed. 4. The dangers of inadequate postinjury immobilization in those patients fortunate enough to be neurologically intact following spinal fracture are stressed.

Dendritic spine "dysgenesis" and mental retardation.

Golgi studies reveal abnormally long, thin spines and the absence of short, thick spines on dendrites of cortical neurons in retarded children with normal karyotypes. The degree of dendritic spine loss and abnormality appears to be related to age and the severity of developmental retardation. Dendritic spine "dysgenesis" is a common feature of the microstructural pathology that occurs in profound mental retardation of unknown etiology.

Symptom Correlation in Spine MRI Interpretation.

ABSTRACT: In spine magnetic resonance imaging (MRI), incidental abnormalities are difficult to distinguish from actual pain generators (the cause of symptoms). Spine specialists see patients face to face and take clinical histories to learn symptoms. They use this symptom information to focus on only the most relevant MRI abnormalities, diagnose pain generators, and make treatment decisions. In the absence of symptom information, radiologists seek to identify all MRI abnormalities and list them in dictated reports without ranking their relative importance. ABSTRACT: Like spine specialists, ra…

Thyroid incidentalomas in cervical spine MRI.

BackgroundThe increasing prevalence of thyroid incidentalomas and their uncertain malignancy risk necessitates population-specific analyses. This study aimed to investigate the prevalence and characteristics of thyroid incidentalomas in patients at the University of Jordan.MethodsConducted at the Jordan University Hospital (JUH), the retrospective study reviewed Magnetic Resonance Imaging (MRI) reports. Patients aged 18-83 were included, and MRI findings were screened for thyroid incidentalomas. Further assessments included neck ultrasound (US) and serum thyrotropin (TSH) analysis to evaluate

[Thoracolumbar spine fractures : Classification and stability].

BACKGROUND: Fractures of the thoracolumbar spine are among the most common spinal injuries and are associated with a high risk of instability and neurological deficits. Accurate classification is crucial to correctly assess the severity of injury and initiate the correct treatment. OBJECTIVES: Presentation and classification of current classification systems, in particular the AO Spine classification, for the structured analysis of thoracolumbar spine fractures, taking into account fracture morphology, ligament stability, and neurological status. METHODS: Literature-based evaluation of epide…

Perioperative Nutritional Optimization in Spine Surgery.

Although spine surgery has been shown to be an effective treatment for many spinal disorders, perioperative complications can increase patient morbidity and lead to poorer surgical outcomes. Nutritional status is a modifiable factor that affects spine surgery results. Malnutrition can negatively influence inflammatory pathways and can change circulating stress hormones. Perioperative nutrition status can be evaluated by both clinical and laboratory parameters, with nutritional indices allowing for determination of perioperative risk. Perioperative nutritional optimization has been attempted an…

Radiological assessment in cervical spine myelopathy.

Degenerative cervical myelopathy (DCM) is the most common cause of non-traumatic spinal cord dysfunction in adults, resulting from conditions such as cervical spondylosis, ossification of the posterior longitudinal ligament (OPLL), and disc degeneration. Radiological evaluation is essential to diagnosis, surgical planning, and prognosis. This review outlines the current role of imaging modalities, ranging from plain radiographs and computed tomography (CT) to magnetic resonance imaging (MRI), in the assessment of DCM. Plain radiographs and CT scans provide valuable information on cervical alig

Pediatric cervical spine instability: evolving concepts.

The pediatric cervical spine is structurally and biomechanically unique in comparison to adults. Guidelines to assess for cervical spine instability and standard of care treatments in the pediatric population have yet to be delineated. This is due to the rarity of the condition and the lack of multicenter data published on the topic. Our review explores the biomechanics of the pediatric cervical spine and highlights evolving concepts/research over the last several decades, with special attention to the Down syndrome and complex Chiari malformation cohorts.

Robotic-navigated assistance in spine surgery.

The aim of this study was to assess the accuracy of pedicle screw placement, as well as intraoperative factors, radiation exposure, and complication rates in adult patients with degenerative disorders of the thoracic and lumbar spines who have undergone robotic-navigated spinal surgery using a contemporary system. The authors reviewed the prospectively collected data on 196 adult patients who had pedicle screws implanted with robot-navigated assistance (RNA) using the Mazor X Stealth system between June 2019 and March 2022. Pedicle screws were implanted by one experienced spinal surgeon after …

Active tuberculosis of spine: Current updates.

BACKGROUND: Spinal tuberculosis (TB) is the most common extrapulmonary form of tuberculosis. In both developing and developed countries, TB has been on the rising trend due to factors such as increasing HIV coinfection, multidrug resistance of the organism, and global migration. Spinal TB, which most often affects the lower thoracic and thoracolumbar area, accounts for 50% of all musculoskeletal tuberculosis. METHODS: Using the Cochrane Database of Systematic Reviews, EMBASE, and PubMed, a systematic computerized literature search was performed. Analyses of studies published within the past 1

Tibial Spine Fractures in Young Athletes.

Tibial spine fractures are a relatively rare injury in the young athlete. Previously thought to be the equivalent of a "pediatric anterior cruciate ligament (ACL) tear," contemporary understanding of these injuries classifies them as distinct from ACL injuries in this patient population. Successful treatment hinges on accurate diagnosis paying special attention to fracture displacement and the presence of concomitant intraarticular injury. Surgery can be performed using open or arthroscopic techniques and a variety of fixation options. The most common complication after surgical treatment is a…

Degenerative Spine Disorders and Multiple Sclerosis.

Multiple sclerosis (MS) is an autoimmune inflammatory disease that results in demyelination of the central nervous system (CNS). MS affects as many as 350,000 individuals in the United States and commonly presents before the age of 45 years. Patients with MS, as the general population, are likely to encounter degenerative changes of the spine as they age, and this can pose a unique challenge to both patients with MS and physicians, as both conditions can have a great deal of symptomatic overlap despite stark differences in management. Currently there is no definitive approach that allows physi…
