Evaluation of Monaldi's approach with regard to needle decompression of the tension pneumothorax-A cadaver study.

BACKGROUND: Although needle decompression of tension pneumothorax through the second intercostal space in the midclavicular line (Monaldi's approach) is a life-saving procedure, severe complications have been reported after its implementation. We evaluated the procedure by comparing how it was performed on cadavers by study participants with different training levels. METHODS: Six participants including one thoracic surgeon performed bilateral thoracic drainage after Monaldi on 82 torsos. After the thoraces were opened, the distances from the internal thoracic artery (A), the site of the punc

Thoracoamniotic Shunting for Fetal Hydrothorax: Predictors of Intrauterine Course and Postnatal Outcome.

OBJECTIVE: To assess predictors for survival and complications among a relatively large cohort of fetuses with hydrothorax treated by thoracoamniotic shunting. METHODS: All cases with hydrothorax treated by thoracoamniotic shunting in a 10-year period (2002-2011) in two centers were retrospectively reviewed. RESULTS: A total of 78 fetuses with hydrothorax treated with thoracoamniotic shunting were included in the study. Mean gestational age at diagnosis was 25.6 weeks (12-34 weeks). Initial thoracoamniotic shunting was performed at a mean gestational age of 26.5 weeks (16-33 weeks). A mean o

Pediatric bilateral spontaneous pneumothoraces in monozygotic twins.

Primary spontaneous pneumothorax from subpleural bleb disease is an uncommon occurrence in pediatric patients. This is a rare case of monozygotic twins presenting at alternating intervals with a single-sided spontaneous pneumothorax, only to have it surgically corrected, and to present later with a subsequent contralateral pneumothorax. A review of familial spontaneous pneumothoraces occurring in children was queried for congenital or genetic syndromes. We concluded that a vast majority of pneumothoraces in children, like adults, are not spontaneous and not familial linked. While they are rare

Extralobar pulmonary sequestration presenting as hemothorax.

Symptomatic extralobar pulmonary sequestration (EPS) is extremely rare. Herein, we report two male patients (3 and 16 years of age) with EPS presenting as hemothorax. Thoracotomic resections of the sequestrated lungs were uneventful. Pathologic examinations revealed hemothorax caused by circulatory disorders within the EPSs.

Successful factor XIII administration for persistent chylothorax after lung transplantation for lymphangioleiomyomatosis.

Lung transplantation has emerged as a viable treatment option for patients with end-stage lymphangioleiomyomatosis (LAM), and therapeutic outcome results reported thus far have been satisfactory. However, persisting chylothorax after transplantation for LAM remains a challenging problem, and the optimal management has not been decided. We present the case with persistent chylothorax after lung transplantation for LAM, in which the intravenous administration of a tissue repair factor (human factor XIII) resulted in complete resolution of chylous effusion without performing additional invasive t

How to avoid and manage a pneumothorax.

Pneumothorax is one of the most frequent complications during percutaneous central vascular cannulation. When choosing a site for central vascular access, the internal jugular vein is preferable to other vessels, for the lower frequency of related complications, including pneumothorax. This review intends to summarize the current state of the art on how to avoid and, if it occurs, to manage this rare but relevant complication. In order to prevent pneumothorax, as well as other relevant complications of central vein cannulation, it is advisable to use ultrasound guidance whenever possible. If p

[Clinical case of the month. A recurrent chylothorax in Bourneville tuberous sclerosis].

Bourneville's disease, first described in 1862, is a phacomatosis that is either autosomal dominant or sporadic. Its typical clinical signs include mental retardation, epilepsy and cutaneous adenomas. The pulmonary form is rare, less than 1%, and is secondary to occlusion of the bronchus, vascular and lymphatics by immature smooth muscle cells. Chylothorax may appear in more than 50% of all cases. No guidelines currently exist for treatment of recurrent chylothorax. However, several possibilities are described in the literature.

Techniques of valvular reoperation.

Valve replacement is simply the exchange of one disease for another. Degeneration of bioprostheses and thrombotic obstruction of mechanical valves, other forms of prosthetic dysfunction and prosthetic valve endocarditis are the main complications leading to reoperation. Repeat valve surgery is more complex than primary surgery and therefore carries a higher mortality and morbidity, even in the hands of experienced surgeons. Several technical details of valve replacement have contributed to the improved results obtained by some surgical groups. Difficulties start with the opening of the sternum

Double innominate vein in a case with double aortic arch: a case report.

BACKGROUND: Double aortic arch (DAA) is a type of vascular ring, causing stridor and dysphagia owing to compression of the trachea and esoph … ring caused by the DAA, surgical resection of the atretic side of the arch was performed by lateral thoracostomy at 4 months of age, before the appearance of signs of bronchomalacia. The double innominate veins were left untouched, and open-heart surgery was avoided. CONCLUSIONS: DAA should be suspected if airway symptoms are present in early infancy, particu …

Use of a pleural access port for the treatment of pyothorax in a cat.

BACKGROUND: Pyothorax in cats is treated with intravenous fluids and antibiotics, and while thoracotomy and debridement are less commonly necessary, thoracostomy tubes are the treatment of choice when repeated drainage of the pleural cavity is needed. CASE DESCRIPTION: An 11-month-old British short-haired cat was presented for a sudden onset of lethargy, dyspnea, and tachypnea, following an ovariohysterectomy 10 days prior to the treatment process. Pyrexia and muffled cardiac sounds on the left hemithorax were noted. A hemogram indicated the development of anemia and neutrophilia with a left …

A delayed and recurrent pneumothorax complicating a fractured clavicle - A novel presentation.

There are very few reports in the literature about clavicular fractures being associated with a pneumothorax. With this combination of injur … hs showed no evidence of a pneumothorax. The pneumothorax resolved after 5 days of treatment with a thoracostomy tube. After removing the tube, the procedure was repeated later that day as he again developed a pneumothorax. Ten days later, the patient had surgical intervention of the clavicle due to the unresolved pneumothorax. The clavicle is usually managed …

Octreotide for chylous effusions in congenital diaphragmatic hernia.

BACKGROUND/PURPOSE: Chylothorax is a frequent complication in congenital diaphragmatic hernia (CDH) infants and is associated with significant morbidity. The optimal treatment strategy remains unclear. We hypothesize that octreotide decreases chylous effusions in infants with CDH. METHODS: This is a retrospective study of all infants with CDH admitted to our institution from October 2006 to October 2011. RESULTS: Eleven (12%) infants developed a chylothorax. Five infants were managed conservatively with thoracostomy and total parenteral nutrition. Six infants were started on octreotide thera…

Traumatic asphyxia due to blunt chest trauma: a case report and literature review.

INTRODUCTION: Crush asphyxia is different from positional asphyxia, as respiratory compromise in the latter is caused by splinting of the chest and/or diaphragm, thus preventing normal chest expansion. There are only a few cases or small case series of crush asphyxia in the literature, reporting usually poor outcomes. CASE PRESENTATION: We present the case of a 44-year-old Caucasian man who developed traumatic asphyxia with severe thoracic injury and mild brain edema after being crushed under heavy auto vehicle mechanical parts. He remained unconscious for an unknown time. The treatment inclu
