Hypomandibular faciocranial dysostosis in consanguineous parents revealed by ultrasound prenatal diagnosis.

We report here the fourth case of hypomandibular faciocranial dysostosis (HFD). The diagnosis was made at birth on the association of severe retrognathia, microstomia, severe hypoglossia with glossoptosis, persistent buccopharyngeal membrane, median cleft palate, bifid uvula, down-slanting palpebral fissures, short nose with anteverted nares, laryngeal hypoplasia, and low-set ears. A severe microstomia and micrognathia were detected by ultrasound at 31 weeks of gestation. Interestingly, even though the present case exhibits many facial dysmorphic features characteristic of HFD, craniosynostosi…

Vagal and hypoglossal Bell's palsy.

A 7-year-old boy was referred because of a sudden change to nasal speech, dysarthria for words with explosive consonants in speech, and nasal regurgitation of fluids. The symptoms arose over 1 week following a capricious episode of acute asthmatic bronchitis. Physical and neurologic examinations were normal except for a left deviation of the uvula, accompanied by a "curtain" movement of the posterior pharyngeal wall against the opposite side, and a left deviation of the protruded tongue. No vascular, traumatic, infectious, neoplastic, or neurologic causes could be identified. No therapy was ad…

Rapp-Hodgkin syndrome with pili canaliculi.

A 20-year-old woman and her 12-year-old brother had hypohidrotic ectodermal dysplasia, cleft lip and palate, midfacial hypoplasia with narrow nose from the nasal bridge to the tip, narrow dysplastic nails, and conical teeth and hypodontia, and hypospadias and hypoplastic uvula in the boy. The woman had major underdevelopment of intellectual capacity. The most important hair anomalies in both siblings were sparse eyebrows, pili torti, and pili canaliculi. Some of the pili canaliculi had two canals (pili bicanaliculi), and the cross section for scanning electron microscopy had a quadrangular asp…

Steinfeld syndrome: report of a second family and further delineation of a rare autosomal dominant disorder.

We report on a fetus with alobar holoprosencephaly, microphthalmia, midline cleft lip and palate, absent nose, dysplastic ears, radial defects, pentalogy of Fallot, unilateral renal aplasia, absent gallbladder, vertebral anomalies, and absence of ribs. The father had a cleft palate, bilateral colobomas of the iris and retina, a bifid uvula, vertebral anomalies, and unilateral congenital hearing loss. His sister had a cleft lip. On the basis of this family and the family reported by Steinfeld [1982], this malformation syndrome can be defined as a rare autosomal dominant syndrome whose main comp…

Results of uvulopalatopharyngoplasty in snoring.

Twenty-one patients (4 females and 17 males) with very annoying snoring were subjected to UPPP with CO2-laser technique. All patients were overweight (bodymass index average 30.3 kg/m2). In 71%, narrowing were found at the naso/oropharyngeal border. All had either abnormal uvula or soft palate. In 17 patients, snoring was reduced to a degree no longer socially annoying. Three were annoying to some extent. In only one patient, snoring was completely unchanged. UPPP is not very difficult to perform and we did not notice any troublesome sequelae. We think that UPPP is an effective operation for s…

The oral manifestations of Apert syndrome.

As part of an ongoing study of 119 patients with the Apert syndrome, extensive data were available for the analysis of oral manifestations, including mouth shape, lip posture, palatal morphology, dental anomalies, and malocclusion. Findings included a characteristic trapezoidal-shaped mouth. Cleft soft palate or bifid uvula was found in approximately 75%. A Byzantine-arch shaped palate was recorded in almost all patients. Dental anomalies included severely delayed eruption, ectopic eruption, and shovel-shaped incisors. Malocclusion tended to be severe with mesial molar occlusion, mandibular ov…

Van der Woude syndrome. A case report.

We describe several members of a family with Van der Woude syndrome, a genetic and congenital malformation syndrome with autosomal dominant inheritance and 70% to 80% penetrance with variable expressivity. It is characterized by clinical signs localized to the face, such as bilateral or unilateral pits on conical elevations in babies or extensive depressions in adults, both in the vermilion border of the lower lip, with cleft lip, with or without cleft palate and uvula. Small accessory or heterotopic salivary glands empty into sinuses or fistulas in the lips. This eight member family had vario…

A Unique Presentation of Velopharyngeal Dysfunction Secondary to Chiari I Malformation.

An 8-year-old female presented with severe hypernasality and suspected velopharyngeal dysfunction (VPD). Physical examination demonstrated a hypoplastic hemi-velum, tongue atrophy, and deviated uvula. Nasopharyngoscopy demonstrated a significant, asymmetric velopharyngeal gap with adynamic hemi-velum and no lateral pharyngeal wall movement. Given the patient's focal neurologic findings, a magnetic resonance imaging was obtained for further evaluation. Magnetic resonance imaging findings were consistent with Chiari malformation type 1. This case illustrates an unusual presentation of VPD likely…

Bilateral peritonsillar abscess: a case study and literature review.

While peritonsillar abscesses are the most common deep neck infections, bilateral forms are rare. A peritonsillar abscess occurs when pus accumulates in the peritonsillar space, located between the tonsils and superior constrictor muscle, causing medial displacement of the uvula, trismus, odynophagia or even upper airway obstruction. High clinical suspicion is needed to diagnose bilateral peritonsillar abscess due to frequent history, computerized tomography scan of the neck with IV contrast facilitates accurate diagnose and a full assessment of the patient. Incision and drainage are needed to…

Ascher's syndrome: A rare case report.

An 18-year-old Indian girl with upper lip deformity presented with on and off painless swelling of her both upper eyelids for 3 years. Clinical evaluation revealed bilateral blepharochalasis, narrowing of horizontal palpebral fissure, decreased outer intercanthal distance, iris coloboma, cleft soft palate, bifid uvula, sensorineural deafness and double upper lip. Clinical examination of the thyroid, thyroid hormone assay and ultrasonography revealed normal thyroid gland structure and function. Ascher's syndrome was diagnosed. To our knowledge, this is the first reported case of Ascher's syndro…

Loeys-Dietz syndrome.

Loeys-Dietz syndrome is an autosomal dominant aortic aneurysm syndrome characterized by multisystemic involvement. The most typical clinical triad includes hypertelorism, bifid uvula or cleft palate and aortic aneurysm with tortuosity. Natural history is significant for aortic dissection at smaller aortic diameter and arterial aneurysms throughout the arterial tree. The genetic cause is heterogeneous and includes mutations in genes encoding for components of the transforming growth factor beta (TGFβ) signalling pathway: TGFBR1, TGFBR2, SMAD3 and TGFB2. Despite the loss of function nature of th…

Annuloaortic ectasia in a 16 year-old boy with Loeys-Dietz syndrome.

Loeys-Dietz syndrome (LDS) is a rare genetic disorder with an autosomal dominant inheritance due to mutations in the transforming growth factor beta-receptor type 1 or type 2. The disease is characterised by the triad of hypertelorism, bifid uvula or cleft palate, arterial tortuosity and aortic aneurysms. These phenotypic characteristics distinguish LDS from other connective tissue disorders related to transforming growth factor beta-receptor. Patients with LDS have a high risk of aortic dissection or rupture at a younger age and smaller aortic diameters. So, clinical suspicion of LDS followed…

Elevoplasty®: An Innovative, Minimally Invasive Procedure for the Treatment of Snoring.

For non-obstructive sleep apnea diagnosed patients with predominantly palatal snoring, Elevoplasty® is an efficient, minimally invasive treatment option. Aimed at reducing snoring severity, the innovative procedure involves the placement of three to four small resorbable polydioxanone barbed sutures, which are buried in the tissues of the soft palate. After placement, the sutures are "activated" by a gentle pull, which provides a "lift" of the soft palatal tissues and uvula. The soft palate, thus, is moved off of the posterior pharyngeal tissues at the back of the throat, providing an increase…

Hair-thread tourniquet syndrome in a preterm baby.

Hair-thread tourniquet syndrome is a rare disorder characterized by the encircling of an appendage by a hair or thread. It usually occurs in children under the age of one year. The tourniquet syndrome may occur in different parts of the body, particularly in toes, fingers, penis, clitoris, labia, neck and uvula. It is an emergency condition that induces progressive edema, ischaemia and tissue necrosis and can lead to autoamputation of digits or other strangulated structures. Emergency treatment is careful removal of the constricting fiber. We report a preterm newborn with hair-thread tournique…

A new syndrome of tufting enteropathy and choanal atresia, with ophthalmologic, hematologic and hair abnormalities.

Three siblings are reported with a syndrome of intractable diarrhea of infancy (owing to tufting enteropathy) and choanal atresia/stenosis. Additional components of this condition are a mild shortness of stature, a prominent and broad nasal bridge, micrognathia, single palmar creases, chronic corneal inflammation, episodic cytopenia, and abnormal hair texture. Intelligence is normal, and there is no immunodeficiency distinguishing this syndrome from that reported by Girault et al. (1994). Additional features that might occur in this syndrome include bifid uvula, preauricular pits, and 2/3 toe …

Tilt suppression of vestibulo-ocular reflex in patients with cerebellar lesions.

The effect of tilt on the time constant of post-rotatory nystagmus was determined in a group of normal subjects and patients with cerebellar lesions. The normal subjects showed a vestibular response that decayed with a time constant of 19.6 s when upright and 7.2 s after tilt prone. Patients with midline cerebellar lesions near the uvula and nodulus had time constants that were normal in the upright position but were unaffected by head tilt. Patients with cerebellar lesions due to the Arnold-Chiari malformation showed behavior that was intermediate between that of normal subjects and the patie…
