Incidental finding of lung hernia in a patient with a remote history of empyema status post video-assisted thoracoscopic surgery.

Lung hernia, the protrusion of pulmonary tissue outside of the thoracic cage, is a rare radiologic finding. The exact incidence of this condition is not well documented. The etiology is either secondary to underlying congenital conditions which weaken the supporting structures of the thoracic wall or more commonly, post-surgical changes. In this report, we describe the case of a 58-year-old male trauma patient who was involved in a motor vehicle collision. Computed tomography of the head, neck, body, and upper extremities was negative for acute injury, but an incidental left-sided herniation o

Video-assisted thoracoscopic surgery for the central bronchogenic carcinoma with invasion of the main bronchus and pulmonary artery: A case report.

Surgical treatment for central bronchogenic carcinoma with invasion of the main bronchus and pulmonary artery often include bronchial anastomosis and pulmonary angioplasty. As a technique, video-assisted thoracoscopic surgery (VATS) lobectomy has proven to reduce the rate of occurrence of complications, and thereby obtain improved survival rates. In the present case study, its use in treating central bronchogenic carcinoma with invasion of the main bronchus and pulmonary artery is reported. A case study of a 68-year-old man with a history of smoking 15-20 cigarettes per day for 40 years is des

Single-center experience in combined video-assisted thoracic surgery with transbronchial microwave ablation-a novel approach to multifocal lung nodules.

BACKGROUND: The management of multifocal lung malignancies presents significant challenges, particularly when extensive resection may not be feasible due to limited pulmonary reserve. Thus, a hybrid management approach combining lung surgery with transbronchial local ablation has been developed to control local disease while minimizing lung parenchymal loss. We evaluated the safety, feasibility, and mid-term clinical outcomes of this hybrid approach in managing multifocal lung lesions. METHODS: A retrospective single-center review was conducted of patients who underwent combined video-assiste

Clinical outcomes of video-assisted thoracoscopic surgery, talc pleurodesis and indwelling catheterisation for malignant pleural effusion: A retrospective comparative study.

INTRODUCTION: Malignant pleural effusion (MPE) is a common complication of advanced cancer that impairs quality of life. The present study compared the outcomes of three procedures: tube thoracostomy with talc pleurodesis, video-assisted thoracoscopic surgery (VATS) with talc and indwelling pleural catheter (IPC). PATIENTS AND METHODS: In this retrospective study, 76 MPE patients were assigned to tube + talc ( n = 29), VATS + talc ( n = 27) or IPC ( n = 20). Outcomes included pleurodesis success, complications, hospitalisation, 30-day reintervention, mortality and overall survival. Eastern Co

A Retrospective Comparison of Oncologic and Staging Outcomes Between Surgical Procedures-Video-Assisted Thoracoscopic Surgery Versus Thoracotomy in Pulmonary Adenocarcinoma.

Introduction: Lymph node status is a key prognostic factor of lung cancer. Although video-assisted thoracoscopic surgery (VATS) is widely used for early-stage disease, its consistency in achieving thorough lymph node dissection remains debated. While many studies show outcomes comparable to thoracotomy, others question its reliability for accurate staging in advanced cases. This study compared the oncologic efficacy of VATS and thoracotomy in pulmonary adenocarcinoma, focusing on lymph node dissection and postoperative outcomes. Materials and Methods: A retrospective analysis was conducted on

Effect of Transcranial Alternating Current Stimulation on Prevention of Postoperative Pain After Video-Assisted Thoracic Surgery: A Randomized Controlled Trial.

INTRODUCTION: Transcranial alternating current stimulation (tACS), which can noninvasively entrain oscillatory brain activity, has attracted scientific attention as a possible technique to control pain. However, there is a scarcity of studies investigating the preventive effect of tACS on postoperative pain. METHODS: This double-blind, randomized, sham-controlled trial enrolled 72 patients undergoing elective video-assisted thoracoscopic surgery (VATS). Patients were randomly allocated (1:1) to receive a single 20-min session of α-tACS on the primary somatosensory cortex (S1) or sham stimulat

The limited protective effect of prolonged smoking cessation on postoperative outcomes in uniportal video-assisted thoracic surgery for pulmonary nodules.

BACKGROUND: Cigarette smoking significantly increases both lung cancer risk and postoperative pulmonary complications. While smoking cessation before surgery is recommended, the optimal duration remains controversial, particularly for uniportal video-assisted thoracoscopic surgery (VATS). This study aimed to clarify the relationship between smoking cessation duration and postoperative outcomes following uniportal VATS for pulmonary nodules. METHODS: This retrospective observational study examined the relationship between smoking cessation duration and complications in patients receiving unipo

Effectiveness of chest tube placement via the utility incision in uniportal video-assisted thoracoscopic surgery lobectomy on residual pleural effusion.

INTRODUCTION: The optimal placement of chest drains after uniportal lobectomy remains a subject of debate, despite the widespread adoption of this minimally invasive approach. This study aimed to compare post-operative pleural effusion between two different chest drain placements: through the utility incision (uniportal video-assisted thoracoscopic surgery [U-VATS]) and through a separate port (triportal VATS [T-VATS]) in patients undergoing pulmonary lobectomy. PATIENTS AND METHODS: We conducted a retrospective, single-center study including 214 consecutive patients who underwent lobectomy f

Effect of Thoracic Paravertebral Block on Postoperative Pulmonary Complications After Video-Assisted Thoracoscopic Surgery: A Dual-Center Randomized Clinical Trial.

PURPOSE: TPVB can provide effective postoperative analgesia in lung cancer patients undergoing video-assisted thoracoscopic surgery (VATS), which may enhance respiratory mechanics and postoperative expectoration, as well as relieve inflammation and stress. These mechanisms may reduce the incidence of postoperative pulmonary complications (PPCs). This study explored whether TPVB reduces the risk of PPCs in patients undergoing VATS for lung cancer. PATIENTS AND METHODS: In this dual-center trial, patients who underwent VATS for lung cancer were randomly divided into the PV group (n = 151, gener

Curved Axillary Incision with Video-Assisted Thoracoscopic Surgery: An Alternative Approach for Teenage Female with Large Apical Chest Wall Tumor.

Primary chest wall tumors are rare, their common clinical features are not well known, and surgical resection remains the main treatment. Apical chest wall tumors require large skin incisions and dissection of the chest wall muscles, making it difficult to maintain cosmetic appearance, respiratory function, and support of the upper extremity. There are few treatment options and no studies have reported on thoracotomy that spares muscles and preserves cosmetic superiority. However, in benign chest wall tumors in young patients, it is necessary to consider radicality, cosmetic superiority, and m

Decortication with uniport video-assisted thoracoscopic surgery for empyema due to postoperative esophageal leakage: a report of two pediatric cases.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) is considered useful for the treatment of parapneumonic empyema in children. However, thoracoscopic management of empyema due to esophageal leakage as an operative complication has not been well described in the literature. CASE PRESENTATION: We successfully decorticated severe empyema using uniport VATS in 2 children (a 2-year-old boy who suffered esophageal perforation after laparoscopic anti-reflux surgery, and a 7-month-old girl who had anastomotic leakage after thoracoscopic repair of esophageal atresia). In these patients, we notic

The effectiveness and safety of computed tomography-guided hook-wire localization for secondary video-assisted thoracoscopic surgery: a retrospective study.

BACKGROUND: Computed tomography (CT)-guided hook-wire localization is currently the most commonly used technique for preoperative localization of pulmonary nodules in clinical practice. With the increasing incidence of multiple primary lung cancers and the increasing occurrence of second primary lung cancers or local recurrences after lung cancer surgery, some patients need to undergo an ipsilateral or contralateral second pulmonary resection. In order to reduce the surgical difficulty of the second operation and accurately guide the surgical resection, preoperative localization of some nodule

Late video-assisted thoracoscopic surgery versus thoracostomy tube reinsertion for retained hemothorax after penetrating trauma, a prospective randomized control study.

BACKGROUND: Early video-assisted thoracoscopic surgery (VATS) is the recommended treatment of choice for retained hemothorax (RH). A prospective single-center randomized control study was conducted to compare outcomes between VATS and thoracostomy tube (TT) reinsertion for patients with RH after penetrating trauma in a resource constrained unit. Our hypothesis was that patients with a RH receiving VATS instead of TT reinsertion would have a shorter hospital stay and lesser complications. MATERIALS AND METHODS: From January 2014 to November 2019, stable patients with thoracic penetrating traum
