Direct and indirect effects of transforming growth factor-beta on osteoclast-mediated bone remodeling using a new in vitro bone matrix model.

Transforming growth factor-beta (TGF-β), a cytokine embedded in the bone matrix, is released during bone resorption, influencing osteoclast differentiation and coupling factor production, which affect osteoblasts and osteocytes. This study investigates the role of TGF-β in bone remodeling using an in vitro model with calcium phosphate-coated plates covalently bonded to latent TGF-β (LTGF-β(+)-CaP plates). This model replicates the natural release of TGF-β and its effects on RAW264 macrophage-like cells, which differentiate into osteoclasts upon stimulation of RANKL. Cells cultured on LTGF-β(+)

Biomechanical Evaluation and Factorial Analysis of the 3-Dimensional Printing Self-Designed Metallic Reconstruction Plate for Mandibular Segmental Defect.

PURPOSE: Reconstruction plates are frequently used to treat mandibular segmental defects. The aim of this study is to compare the biomechanical performance of a 3-dimensional-printed self-designed titanium alloy reconstruction plate with that of the traditional reconstruction plate in mandible reconstruction. The analyzed parameters of the self-designed reconstruction plate, including plate length (100 mm and 125 mm), plate thickness (2.1, 2.4, and 2.7 mm), and bone mass (100, 75, and 50%), were also evaluated. METHODS: An artificial mandible with anatomical geometry was used to develop the s

Biomechanical design of a new proximal humerus fracture plate using alternative materials.

Comminuted proximal humerus fractures are often repaired by metal plates, but potentially still experience bone refracture, bone "stress shielding," screw perforation, delayed healing, and so forth. This "proof of principle" investigation is the initial step towards the design of a new plate using alternative materials to address some of these problems. Finite element modeling was used to create design graphs for bone stress, plate stress, screw stress, and interfragmentary motion via three different fixations (no, 1, or 2 "kickstand" [KS] screws across the fracture) using a wide range of plat

Preclinical study of additive manufactured plates with shortened lengths for complete mandible reconstruction: Design, biomechanics simulation, and fixation stability assessment.

BACKGROUND: A combination of short titanium plates fabricated using additive manufacturing (AM) provides multiple advantages for complete mandible reconstruction, such as the minimisation of inherent implant deformation formed during AM and the resulting clinical impact, as well as greater flexibility for surgical operation. However, the biomechanical feasibility of this strategy is still unclear, and therefore needs to be explored. METHOD: Three different combinations of short mandible reconstruction plates (MRPs) were customised considering implant deformation during the AM process. The res

Expansion-Floating Craniotomy as a Novel Alternative Technique for Decompressive Craniectomy: A Technical Note.

BACKGROUND: Decompressive craniectomy and subsequent cranioplasty are associated with significant morbidity and mortality. This study presents a novel expansion-floating craniotomy designed to provide a decompressive volume similar to decompressive craniectomy and allows the bone flap repositioning later in a simple way. METHODS: In expansion-floating craniotomy, the bone flap was elevated with 3 plates to the desired position to ensure sufficient decompression and preserve capacity for subsequent outward displacement. Subsequent plates were released to permit spontaneous bone flap reposition

Biomechanical study of different plate configurations for distal humerus osteosynthesis.

Fractures of the distal humerus are most commonly fixed by open reduction and internal fixation, using plates and screws, either in a locking or in a non-locking construct. Three different plating systems are commonly used in practice. The most important differences between them are in plate orientation, which affects both the rigidity of the osteosynthesis and invasiveness of the surgical procedure. Unfortunately, there is no common agreement between surgeons about which plate configuration brings the best clinical outcome. In this study, we investigate the theoretical rigidity of plate osteo

A Cone Bean Computer Tomography Investigation of the Newly Formed Mandibular Anterior Ridge following the Treatment of an Extended Comminuted Fracture: A 12-Year Follow-Up.

INTRODUCTION: Extensive comminuted fractures are associated with tooth loss that ultimately leads to dimensional changes in the hard and soft tissues of the alveolar ridge. Reconstruction of the lost mandibular anterior ridge is very complex due to the natural curvature of the region. Case Presentation. In this case report, the combination of the modified shell technique with autologous bone plates and the guided bone regeneration (GBR) technique was performed on an 18-year-old patient after a comminuted fracture, to ensure new bone formation in the anterior ridge with a natural curvature. Aft…

Biomechanical and biological aspects of defect treatment in fractures using helical plates.

The clinical case of figure 1 through figure 11 shows a series of impressive failures of plate fixation. The plates were repeatedly applied bridging a comminuted bone segment in a heavy patient. The biomechanical analysis elaborates why this happened and proposes an unconventional procedure to prevent this failure with a minimally invasive procedure. A plate bridging an open gap or a defect in a long bone diaphysis is exposed to full functional load. According to clinical observations such plate application often fails even without external load such as weight bearing. The plate risks to break

Biomaterials for orbital fractures repair.

The unique and complex anatomy of the orbit requires significant contouring of the implants to restore the proper anatomy. Fractures of the orbital region have an incidence of 10-25% from total facial fractures and the most common age group was the third decade of life. The majority of cases require reconstruction of the orbital floor to support the globe position and restore the shape of the orbit. The reason for this is that the bony walls are comminuted and/or bone fragments are missing. Therefore, the reconstruction of missing bone is important rather than reducing bone fragments. This can

A Rare Complete Metacarpal Pseudoepiphysis in a Teenager With a Scaphoid Fracture.

A 13-yr-old adolescent boy presented with wrist pain after falling off a scooter onto his outstretched hand. Radiographs revealed a nondisplaced hairline fracture of the scaphoid bone and an irregular radiolucent line in the proximal metaphysis of the second metacarpal bone, consistent with an anomalous growth plate, or complete pseudoepiphysis. Complete pseudoepiphysis is a rare finding, with only a few cases reported. Learning about the common locations of growth plates and the radiographic differences between normal, injured, and anomalous growth plates can help decrease physician error and

Anatomical Fit of the Volar Locking Plate for Distal Radius and Influence of the Plate Size.

PURPOSE: The volar locking plates (VLPs) for distal radius fractures are currently available in different shapes and size options. Therefore, we investigated whether the anatomical fit and plate size of VLPs affect their proper placement. MATERIALS AND METHODS: Two sizes of Acu-Loc2 Proximal Plates (AcuLoc), three sizes of Variable Angle LCP Two-Column Volar Distal Radius Plate 2.4 (VALCP), and 16 formalin-fixed cadaver specimens were studied. Computed tomography scans were performed on both the plates and forearms. The plate was fixed to a radius, and radiographs were obtained using fluorosc

General principles for the clinical use of the LCP.

The basic principles of an internal fixation procedure using a conventional plate and screw system (compression method) are direct, anatomical reduction and stable internal fixation of the fracture. Wide exposure of the bone is usually necessary to gain access to and provide good visibility of the fracture zone to allow reduction and plate fixation to be performed. This procedure requires pre-contouring of the plate to match the anatomy of the bone. The screws are tightened to fix the plate onto the bone, which then compresses the plate onto the bone. The actual stability results from the fric

[Prevalence of metatarsal growth plate lesions in Swiss fattening bulls].

The prevalence of osteochondral changes in the metatarsal growth plates of fattening bulls was investigated. Plantarodorsal radiographic views of the metatarsal region of 204 hind legs from 102 slaughtered fattening bulls were taken. Radiographic lesions in metatarsal growth plates were confirmed in selected cases using computed tomography (n=14) and histology (n=6). Radiographic criteria for growth plate lesions were axis deviations between the metatarsal bones and the digit, radiolucencies in the growth plate, increased bone density in the metaphysis, irregularities of the growth plate and m

A biomechanical comparison of a biodegradable volar locked plate with two titanium volar locked plates in a distal radius fracture model.

BACKGROUND: Volar plating is commonly used in the management of distal radius fractures; bioresorbable plates have attractive features. We compared a bioresorbable plate with a latest generation and an established locked titanium plate. METHODS: Twenty-four fresh-frozen radii (12 pairs) were assigned to three mean bone mineral density-matched groups of eight radii each. A standardized extraarticular distal radius fracture was created and plated using one implant type per group. Postplating stiffness and displacement were studied in a first axial-loading test (15 cycles at 250 N). Next, biodeg

Biomechanical comparison of three-dimensional titanium and CFR-PEEK plates for mandibular subcondylar fracture fixation: a three-dimensional finite element analysis.

BACKGROUND: Fixation of mandibular subcondylar fractures presents challenges due to limited surgical access and the small size of the proximal fragment; therefore, single-piece and easily applicable three-dimensional (3D) plates may be considered as an alternative. Although titanium is frequently used as a plate material, it has been reported to have certain clinical and imaging-related limitations. In comparison, Carbon Fiber-Reinforced Polyetheretherketone (CFR-PEEK) has emerged as a favorable alternative for mandibular fracture fixation due to its bone-like elastic properties, radiolucency,
