Intramedullary brain stem cyst and trapped IV ventricle after infection with Listeria monocytogenes.

We present the rare case of a girl surviving intrauterine listeria brain stem meningoencephalitis, who subsequently developed hydrocephalus, a trapped IV ventricle and an intramedullary cyst. Such cases have been reported only infrequently, and in earlier cases modern imaging studies were not available. Magnetic resonance imaging has been helpful in our patient to delineate the lesions and plan further treatment.

Nucleus 20-channel and 21-channel auditory brain stem implants: first European experiences.

Central electrical stimulation of the auditory pathway can allow hearing in patients suffering from deafness localized in the auditory nerve. Developments in a multichannel auditory brain stem implant based on the Nucleus Mini 22 cochlear implant with transcutaneous signal transmission is discussed. The device has been implanted in seven European patients who suffered from neurofibromatosis type 2. Preliminary speech perception results and patient satisfaction are encouraging, and the data presented include some limited open-set speech recognition.

Auditory-evoked brain-stem responses and auditory disorders in patients with Bell's palsy.

Out of 121 patients examined with acute unilateral facial paralysis, 93 were determined to have idiopathic facial palsy (Bell's palsy). The examination included pure-tone and speech audiometry, stapedial reflex recordings, temporal bone radiography and auditory-evoked brain-stem response testing (ABR). If a retrocochlear lesion was suspected, computed tomography or magnetic resonance imaging was performed. Patients with sensorineural hearing loss affecting all frequencies were compared to one group with hearing loss affecting only high frequencies and to another group with ABR findings suggest

Brain stem auditory-evoked potentials in meningomyelocele. Natural history of Chiari II malformations.

Brain stem auditory-evoked potentials (BAEPs) were recorded in 30 patients with meningomyelocele and shunted hydrocephalus ranging in age from birth to 33 years. Twenty-two of them had Chiari II malformation, one of which was symptomatic. In 22 cases, including 15 with Chiari II malformation, BAEPs were recorded repeatedly after periods ranging from 18 months to 7 years. The results were as follows: (1) III-V interpeak latency (IPL) decreased progressively from prolonged to the normal range with growth until 25 years of age; (2) I-III IPL tended to prolong gradually from the normal range; (3) …

GABAA receptor mediated fast synaptic inhibition in the rabbit brain-stem respiratory system.

The involvement of GABA mediated neurotransmission in the central control of respiration was investigated by administration of the specific GABAA receptor agonist muscimol and the specific GABAA receptor antagonist biculline into the fourth cerebral ventricle of the rabbit. Cycle-triggered averaging of the phrenic nerve activity (PNA) was used to quantify drug-induced changes of the central respiratory pattern. Muscimol reduced the peak amplitude of PNA and increased the duration of the respiratory phases. High amounts of muscimol led to a long-lasting but reversible central apnea. Bicuculline

Availability of transplantable organs from brain stem dead donors in intensive care units.

OBJECTIVE: By audit from January to June 1989 to quantify, separately for hearts, kidneys, liver, lungs and corneas, the possible increases in transplantable organs from brain stem dead potential donors in intensive care units and to compare them with the increases achieved in October-November 1989, during intense, national publicity about transplantation. DESIGN: Prospective audit of all deaths in intensive care units in England from 1 January to 30 June 1989 and subsequent case study of the impact of publicity on offers and donations during October-November 1989. SETTING: 15 regional and s…

Pressure-induced modifications of the acoustic nerve. Part II: Auditory brain stem responses.

The auditory brain stem response is a diagnostic tool commonly used in the evaluation of patients suspected of having an acoustic neuroma. The mechanism by which the auditory brain stem response is altered in the presence of an acoustic neuroma is unclear. In this report, an experimental model is presented as an investigative tool for future studies of this mechanism: acute compression of the nerves of the internal auditory canal in cats stimulates the changes observed in patients with acoustic neuromas.

Auditory brain-stem responses in guinea pigs following middle ear implantation of Ceravital.

The effects on auditory function caused by implantation of the bioactive glass ceramic Ceravital in the middle ear of guinea pigs was investigated. Auditory brain-stem responses (ABR) were used to measure hearing function. A threshold shift due to a conductive hearing loss was observed immediately post-operatively. This threshold shift increased towards the 20th post-operative day and improved thereafter. Surgical manipulation of the middle ear and biochemical reactions between the Ceravital granules and the middle ear wall may have contributed respectively to the immediate and late temporary

[Response latency characteristics for ENT medical assessment of auditory brain stem evoked response].

A schematic description of the correlation between the various types of hearing disorders and the behaviour of auditory brain stem responses (ABR) is presented. Conductive pathology and high-frequency cochlear hearing loss prolong wave component latency due to energy loss and hair cell dysfunction. Latency is not affected in flat cochlear hearing loss. Prolonged interwave latencies between wave I and wave V indicate eighth nerve and brain stem disorders. An algorithm in the form of a flow chart was developed for location of the malfunction. Families of characteristics of wave V intensity-laten…

A quantitative assessment of the brain stem auditory evoked response during intraoperative monitoring.

The brain stem auditory evoked response (BAER) was monitored intraoperatively in 11 patients with tumors of the posterior fossa. A technique for the objective quantitative assessment of the BAER based on the variance of the phase angle between small group averages is described. The resulting component synchrony measure corroborated standard peak latency and amplitude changes of the BAER during operative manipulation. A discussion of the replication and use of the objective measure is presented.

Measurements of brain-stem auditory evoked potentials in patients with spontaneous intracerebral hemorrhage.

Brain-stem auditory evoked potentials (BAEP's) were recorded in 19 patients with spontaneous intracerebral hemorrhage. More than half of the patients were deeply comatose. There was no correlation between BAEP changes and different types of spontaneous intracerebral hemorrhage or between BAEP's and coma grading by the Glasgow Coma Scale. However, BAEP's were a significant prognostic aid in these cases and useful in indicating the level of the brain-stem lesion.

Focal necrotizing brain stem encephalopathy and cranial radiculopathy in a kidney transplant recipient.

A 48-year-old female patient developed sensory and motor palsy of the V-Xth cranial nerves immediately after kidney transplantation due to terminal uremia. A second exacerbation followed about 4 weeks later. After 46 days post transplantation she died from bronchopneumonia. Autopsy showed multiple acute and subacute necrotic foci in proximal portions of the cranial nerves as well as a discontinuous necrotizing encephalopathy localized mainly in the subpial zone of the brain stem. The mechanism by which lesions of the nervous system were produced remains speculative. A possible neurotoxic role …

Alteration of brain stem auditory evoked responses following cardio-respiratory arrest and resuscitation.

BAERs were recorded in a patient who suffered spontaneous cardio-respiratory arrest after testing had begun. During the arrest the BAERs initially decreased in amplitude and then could not be recorded. After restoration of cardiac activity there was immediate recovery of waves I and II. Subsequent potentials also immediately reappeared but they did not recover their original peak latency values during the 50 min following resuscitation.

Modifications of the stapedius muscle reflex under spontaneous and experimental brain-stem impairment.

An oscilloscopic plus a graphic method of recording of the stapedius muscle reflex has revealed peculiar modifications in the latency, threshold, and shape of development of the reflex in cases of brain-stem impairment. Many subjects have been examined who were affected by nucleo-reticular vestibular syndrome, vascular insufficiency, disseminated sclerosis, tumours, etc. and a comparison has also been made in normal subjects and in the same subjects during a temporary impairment of the brain-stem structures following barbiturate administration. The greatest semeiological value is attributed to

Stem Cell Therapy for Traumatic Brain Injury: Translational Mechanisms, Biomarkers, and Clinical Trials.

Traumatic brain injury (TBI) is a leading cause of persistent cognitive, motor, and neuropsychiatric impairment, arising from both the initial mechanical insult and a prolonged cascade of secondary injury processes. While primary injury reflects structural damage, secondary mechanisms, including neuroinflammation, oxidative stress, apoptosis, and blood-brain barrier disruption, evolve over time and critically influence long-term outcomes. Despite extensive investigation, therapies targeting isolated components of secondary injury have demonstrated limited clinical success, highlighting the nee
