Discreet discrete commands for assistive and neuroprosthetic devices.

Many new assistive devices are available for individuals paralyzed below the neck due to spinal cord injury. Severely paralyzed individuals must be able to command their complex assistive devices using remaining activity from the neck up. Electromyographic (EMG) sensors enable people to use contractions of head and neck muscles to generate multiple proportional command signals. Electroencephalographic (EEG) signals can also be used to generate commands for assistive device control by conveying information about imagined or attempted movements. Fully-implanted wireless biopotential detection sy

Implantation of anterior sacral root stimulators combined with posterior sacral rhizotomy in spinal injury patients.

Brindley-Finetech sacral anterior root stimulators combined with posterior sacral rhizotomy were implanted in 68 males and 28 females with spinal cord lesions. In 9 patients the electrodes were implanted extradurally in the sacrum, and in 90 patients they were implanted intradurally (3 patients had a second extradural implant after a first intradural implant). Three patients died from causes unrelated to the implant. Of the 93 surviving patients, 83 used their implants for micturition and 82 were fully continent. The mean bladder capacity increased from 206 ml preoperatively to 564 ml after th

Safety and performance of a novel subxiphoidal pacemaker system.

BACKGROUND: Traditional cardiac pacemakers commonly have a range of complications related to the presence of intracardiac leads. A new class of extravascular and leadless pacemakers has recently emerged with the potential to mitigate these complications and expand access to cardiac pacing. The objective of this study is to evaluate the implantation, short-term chronic safety, and performance of a novel subxiphoidal extracardiac pacemaker. METHODS: Normal Yorkshire Cross swine (n = 16) were implanted with the subxiphoidal pacemaker. The pacemaker was inserted through a midline chest incision a

Biomedical studies on temporal bones of the first multi-channel cochlear implant patient at the University of Melbourne.

OBJECTIVE: To analyse the temporal bones and implant of the first University of Melbourne's (UOM) patient (MC-1) to receive the multi-channel cochlear prosthesis. METHODS: The left cochlea was implanted with the prototype multi-channel cochlear prosthesis on 1 August 1978, and the Cochlear versions CI-22 and CI-24 on 22 June 1983 and 10 November 1998, respectively. MC-1 died in 2007. RESULTS: Plain X-rays of the temporal bones showed that after the CI-22 had been explanted seven electrode bands remained in situ. Micro-CT scans also revealed a partially united fracture transecting the left im

Close approximations to the sound of a cochlear implant.

Cochlear implant (CI) systems differ in terms of electrode design and signal processing. It is likely that patients fit with different implant systems will experience different percepts when presented speech via their implant. The sound quality of speech can be evaluated by asking single-sided-deaf (SSD) listeners fit with a cochlear implant (CI) to modify clean signals presented to their typically hearing ear to match the sound quality of signals presented to their CI ear. In this paper, we describe very close matches to CI sound quality, i.e., similarity ratings of 9.5 to 10 on a 10-point sc

Video-assisted sacral nerve stimulation.

BACKGROUND: Sacral nerve simulation (SNS) is an accepted therapy for patients with urinary or bowel dysfunction. However, infection rates are as high as 20% and can result in removal of the expensive device. We present a new video-assisted technique minimizing the risk of infection. METHODS: Between April and July 2005, six consecutive women of median age 68 years (range, 60-74), with faecal incontinence (4 patients) and idiopathic constipation (2 patients) underwent video-assisted electrode implantation for SNS. The motor response of the pelvic floor during percutaneous nerve evaluation and

Acute and long-term ventricular stimulation thresholds with a new, iridium oxide-coated electrode.

Efforts have been made to design electrodes that significantly reduce not only the acute and chronic stimulation thresholds, but also attenuate the early peaking phenomenon and polarization. At two voltage levels (2.7 V and 5.4 V, respectively), we evaluated the right ventricular stimulation thresholds obtained with a new, iridium oxide-coated electrode in ten patients who received a VVI pacemaker. Measurements were made at implant and at multiple intervals for 1 year. Pulse width stimulation thresholds at implant were as follow: 0.04 +/- 0.008 msec at 2.7 V, 0.03 +/- 0.004 msec at 5.4 V; valu

Electroactive nanostructured polypyrrole coatings with on-demand drug release for immunomodulation of neural electrode interfaces.

Neural electrodes are central to a wide range of neuroprosthetic and neuromodulation technologies. However, persistent macrophage-driven inflammation often leads to fibrotic encapsulation and elevated electrical impedance, severely limiting stimulation efficiency and device longevity. Here, we reported an electroactive, electrically controlled drug-releasing neural electrode coating based on nanostructured polypyrrole (NPPy), enabling on-demand and sustained immunomodulation at neural interfaces. The nanostructured coating exhibited a high drug-loading capacity and released dexamethasone in a

Grafting and electrical stimulation of the auditory nerve in cats.

The effect of implanting wire electrodes into the cochlea is limited by the deficit of nerve fibers in most diseases of the cochlea. Therefore, in a series of experiments it was tried, for the first time, to direct new outspreading nerve fibres into the afferent acoustic nerve by grafting a motor nerve to it. A second series of experiments should demonstrate the possibility of direct electrical stimulation of the eighs nerve by implanting wire electrodes via the transoccipital approach. The electrically evoked potentials, recorded at the auditory cortex, were compaired to potentials evoked by

Effect of the C-terminal tetrapeptide amide of gastrin (CCK-4) and pancreatic polypeptide on gastrointestinal electrical activity in the conscious miniature pig.

The effect of IV infusion of CCK-4, 33.2 and 332 pM/kg/min, and pancreatic polypeptide (PP), 4.8 and 48 pM/kg/min, on gastrointestinal electrical activity was studied in conscious miniature pigs with electrodes implanted in the wall of the antrum pylori and small intestine. In the antrum pylori infusion of the higher dose of both peptides provoked an increase in frequency of the basic electrical rhythm together with a decrease in frequency of spike bursts. In the studied dose range CCK-4 and PP were without influence on small intestinal electrical activity.

Therapeutic stereotactic procedures on the thalamus for motor movement disorders.

The value of functional neurosurgery in the treatment of motor movement disorders is emphasized. The two methods of stereotactic procedures, namely a destructive one with small lesions centered on specific targets, and a non-destructive one with chronically inserted electrodes connected with an also implanted programmable neuropacemaker are described in detail. The results in Parkinsonian tremor, essential tremor, tremor of multiple sclerosis, post-traumatic tremor and in other involuntary movement disorders are reported and demonstrate that stereotactic neurosurgical treatment of these condit

[The role of adrenergic receptors in the realization of hypothalamic effects on the motor function of the digestive tract in hypophysectomized dogs].

Chronic experiments on hypophysectomized dogs with gastric and small intestinal fistulas as well as with hypothalamus-implanted electrodes have shown that blockade of alfa-adrenoreceptors by phentolamine increases the number of the excitatory motility responses while stimulating hypothalamic structures during rest of the gastrointestinal tract and decreases inhibitory effects of the stomach while stimulating hypothalamic structures during work of the digestive tract. The blockade of beta-adrenoreceptors by obsidan induces weakening of excitatory responses of the digestive tract and intensifica

[The effect of pyridoxine on the cerebral hemodynamics in vestibular disorders].

By means of hydrogen clearance on conscious rabbits with implanted platinum electrodes it was established that pyridoxine (1 and 10 mg/kg) used against the background of sea sickness decreased the dilatational reaction of the cerebral vessels occurring during the stimulation of the vestibular apparatus, reduced the blood supply to the cerebral hemispheres with insignificant changes of oxygen tension in the cortical structures, relieved acidosis and hypoxemia developing during sickness in the orthostatic position, increased oxygenation of the arterial blood without influencing significantly pat

New possibilities of improving the function of the hand of patients with spastic hemiplegia.

The report presents a therapeutic proposal aiming at the improvement of the functions of the paretic hand in spastic hemiplegics. To achieve this aim the author suggests a combination of phenolization of the medial and ulnar nerves and the stimulation training of the wrist and fingers extensors. An implanted stimulator is used; the stimulator electrodes are fixed to the radial nerve. The strengthening of the extensors during the period of increased muscular tension of the flexors makes it possible to improve the functions of the paretic upper extremity.

Sustained Seizure Freedom After Stereo-Electroencephalography Without Definitive Intervention: Evidence for Network Modulation by Invasive EEG.

Stereo-electroencephalography (sEEG) is a diagnostic technique used to localize seizure-onset networks in drug-resistant epilepsy, typically guiding resection, ablation, or neuromodulation. Sustained seizure freedom after diagnostic implantation alone is rarely described. We describe two patients who achieved long-term remission after sEEG monitoring without definitive surgical intervention. A 39-year-old man with posttraumatic right hemispheric epilepsy underwent implantation of 11 depth electrodes, during which no habitual seizures were captured despite medication withdrawal and provocation.

In vitro investigation of electrospun PVDF-TrFE fiber mats to reduce connective tissue growth after cochlear implantation.

INTRODUCTION: Cochlear implant electrodes are frequently ensheathed by connective tissue as a consequence of surgical trauma and foreign body response, which can impair electrical stimulation. In this study, fiber mats fabricated by electrospinning with three different spinning times (15, 30 and 45 min) were evaluated for their suitability as surface modifications for cochlear implant electrodes to reduce connective tissue-induced insulation after implantation. METHODS: Their biocompatibility was assessed through incubation with an extract solution generated from the fiber mat using two diffe
