Concomitant Aortic Root Replacement in Total Arch Replacement With Frozen Elephant Trunk for Type A Aortic Dissection: A Propensity-Weighted Analysis.

OBJECTIVES: This study aims to evaluate the impact of concomitant aortic root replacement (ARR) on early and late outcomes in patients undergoing total arch replacement (TAR) with frozen elephant trunk (FET) for type A aortic dissection (TAAD). METHODS: From January 2010 to December 2019, a total of 1155 patients were enrolled. Among them, 325 patients received concomitant ARR (with ARR group) while 830 patients did not (without ARR group). Baseline differences were adjusted using inverse probability of treatment weighting (IPTW). RESULTS: After IPTW, the with ARR group still had a higher pr

Treatment of a Type II Endoleak from an Atypical Mediastinal Collateral Branch Following Total Arch Replacement with Frozen Elephant Trunk.

We present a rare case of type II endoleak (T2EL) from an atypical mediastinal artery following total arch replacement with a frozen elephant trunk (FET) for chronic aortic dissection. A 63-year-old male with a history of multiple thoracic aortic surgeries, including FET, developed enlargement of an aortic arch aneurysm on follow-up. Computed tomography and diagnostic angiography revealed a T2EL due to a newly developed mediastinal collateral artery arising from the left subclavian artery feeding the sac. Selective transcatheter embolization with N-butyl-2-cyanoacrylate successfully eliminated

A "Dock-Work" Orange: A Dual-Receptor Biochemical Theory on the Deterrence Induced by Citrusy Aroma on Elephant Traffic Central to a Conservation Effort.

Conservation of elephants requires physical, chemical, and biological approaches to ensure the protection of these gargantuan pachyderms. One such approach is using orange plants (as biofencing) for the repellence of elephants, which precludes catastrophic events related to the encroachment of elephants into human habitats. Elephants have sensitive olfactory discrimination of plant volatile compounds for foraging and other behavior using G-protein-coupled receptors (GPCRs). However, 2 such receptors are the A2A and A2B receptors mediating olfaction elicited by a host of ligands, including limo…

Complicated acute type A aortic dissection and severe aortic atherosclerosis predict early mortality after frozen elephant trunk procedure.

OBJECTIVES: To analyse risk factors for early mortality and long-term survival including secondary distal aortic interventions in patients undergoing frozen elephant trunk surgery. METHODS: A retrospective single-centre study was conducted, including all 222 patients who underwent frozen elephant trunk surgery between 2010 and 2022. We used multivariable regression analysis to detect risk factors for early mortality and Kaplan-Meier analysis for long-term survival and secondary interventions. We introduce the term 'complicated acute type A dissection' for those patients in whom the dissection

Hybrid prosthesis in frozen elephant trunk procedures for hereditary thoracic aortic diseases: a 14-year single-aortic center experience.

BACKGROUND: There is a lack of data regarding the use of hybrid stent graft prostheses in patients with hereditary thoracic aortic disease (HTAD) involving the aortic arch and proximal descending aorta. This retrospective analysis aimed to evaluate the short- and mid-term outcomes of hybrid stent-graft prostheses in Frozen Elephant Trunk (FET) procedures for patients with HTAD, with a particular focus on its safety and feasibility. METHODS: A total 280 patients who underwent FET procedures between October 2010 and November 2024 were retrospectively analysed in compliance with the 2024 EACTS/

Single centre Asian experience of first 100 cases of frozen elephant trunk procedures: Humble beginning, evolution and lessons learned.

BackgroundThe frozen elephant trunk (FET) procedure is developed to treat complex aortic arch diseases. We aimed to share our single centre experience of FET operations over time.MethodsA review of first 100 patients who underwent FET procedures for acute, chronic dissection and thoracic aneurysm from January 2016 to January 2024. The patients were divided into two groups: Group 1 (first 50 cases); Group 2 (subsequent 50 cases) and compared the early and late outcomes.ResultsEarly overall mortality was 9% but reduced with time {12% (6/50) in Group 1 vs. 6% (3/50) in Group 2, p = 0.295} with no

Risk factors, prevention, and therapy of intraluminal stent thrombosis in frozen elephant trunk prostheses-what we know so far.

Intraluminal thrombus formation (ILT) is a recently discovered and highly clinically relevant complication after frozen elephant trunk implantation in cardiovascular surgery. In this phenomenon, a thrombus forms within the lumen of the stent graft component of the frozen elephant trunk prosthesis and puts the patient at risk for downstream embolization with visceral or lower limb ischemia. Incidence of ILT reported in the currently available studies ranges from 6% to 17% of patients after frozen elephant trunk implantation. Adverse thromboembolic events include acute occlusion of the celiac an

Impact of concomitant coronary artery bypass grafting on the outcomes of total arch repair with frozen elephant trunk for type A aortic dissection.

OBJECTIVES: Coronary artery bypass grafting (CABG) is often performed alongside type A aortic dissection (TAAD) repair. However, the association between concomitant CABG and the clinical outcomes of TAAD repair remains uncertain. METHODS: This study included 1002 consecutive TAAD patients who underwent total aortic arch replacement (TAR) with frozen elephant trunk from June 2019 to January 2024. Concomitant CABG during TAR and frozen elephant trunk was performed under 3 conditions: planned CABG for coronary ostial involvement, planned CABG for coronary artery disease and rescue CABG. Patients

Branched stented anastomosis frozen elephant trunk repair: Early results from a physician-sponsored investigational device exemption study.

BACKGROUND: Multisegment thoracic aortic disease typically requires total aortic arch replacement, affects a heterogenous population, and carries a high risk even at centers of excellence. Risk has been associated with the duration of operation and complexity of repair. A novel branched stented anastomosis frozen elephant trunk repair (B-SAFER) technique has been developed at our center and is currently being studied as a physician-sponsored investigation device exemption (PS-IDE). OBJECTIVE: This study aimed to assess the early safety of using this investigational technique to treat the prox

Arch vessels' switch in frozen elephant trunk: a new technique to facilitate the second-stage endovascular thoraco-abdominal aorta repair.

In patients with extensive thoraco-abdominal aortic disease, staged hybrid repair involving open total aortic arch replacement and endovascular thoraco-abdominal aorta repair with branched stent graft has emerged as a valuable treatment option. However, total arch replacement with the available branched vascular grafts often results in acute angulation between the reimplanted vessels and the aortic arch hampering antegrade catheterization of the thoraco-abdominal aorta during the second endovascular stage. Here, we present our 'switch technique' for arch vessels' reimplantation to facilitate a
