Optogenetic determination of the myocardial requirements for extrasystoles by cell type-specific targeting of ChannelRhodopsin-2.

Extrasystoles lead to several consequences, ranging from uneventful palpitations to lethal ventricular arrhythmias, in the presence of pathologies, such as myocardial ischemia. The role of working versus conducting cardiomyocytes, as well as the tissue requirements (minimal cell number) for the generation of extrasystoles, and the properties leading ectopies to become arrhythmia triggers (topology), in the normal and diseased heart, have not been determined directly in vivo. Here, we used optogenetics in transgenic mice expressing ChannelRhodopsin-2 selectively in either cardiomyocytes or the

Reduction of fibrosis-related arrhythmias by chronic renin-angiotensin-aldosterone system inhibitors in an aged mouse model.

Myocardial fibrosis increases arrhythmia vulnerability of the diseased heart. The renin-angiotensin-aldosterone system (RAAS) governs myocardial collagen synthesis. We hypothesized that reducing cardiac fibrosis by chronic RAAS inhibition would result in reduced arrhythmia vulnerability of the senescent mouse heart. Wild-type mice (52 wk old) were treated for 36 wk: 1) untreated control (C); 2) eplerenone (E); 3) losartan (L); and 4) cotreatment with eplerenone and losartan (EL). Ventricular epicardial activation mapping was performed on Langendorff-perfused hearts. Arrhythmia inducibility was

Transcoronary mapping of ventricular asynchrony due to left bundle branch block in a porcine model.

BACKGROUND: Optimal positioning of the left ventricular (LV) lead at the latest activated part of the left ventricle is one of the major challenges in implantation of cardiac resynchronization therapy (CRT) devices with respect to ascertaining an optimal resynchronization effect resulting in a high responder rate. In the present study, we evaluated the feasibility of transcoronary measurement of LV electrical activation by a coated guidewire in a porcine model. METHODS AND RESULTS: Transcoronary measurement of ventricular activation was performed in 16 pigs under general anesthesia. Left bund

AI-Driven segmentation and morphogeometric profiling of epicardial adipose tissue in type 2 diabetes.

BACKGROUND: Epicardial adipose tissue (EAT) is associated with cardiometabolic risk in type 2 diabetes (T2D), but its spatial distribution and structural alterations remain understudied. We aim to develop a shape-aware, AI-based method for automated segmentation and morphogeometric analysis of EAT in T2D. METHODS: A total of 90 participants (45 with T2D and 45 age-, sex-matched controls) underwent cardiac 3D Dixon MRI, enrolled between 2014 and 2018 as part of the sub-study of the Swedish SCAPIS cohort. We developed EAT-Seg, a multi-modal deep learning model incorporating signed distance maps

Perimitral ventricular tachycardia associated with remote inferior myocardial infarction.

INTRODUCTION: Circuits underlying ventricular tachycardias (VTs) accompanying remote inferior myocardial infarction (IMI) are regarded to be located in the scar. Rotation around the mitral annulus (MA) had also been postulated. We tested whether entrainment mapping could confirm whether MA rotation in VTs post-IMI represented a "driving circuit." METHODS: Three patients with IMI (male, left ventricular ejection fraction range 13%-40%) with hemodynamically tolerated VT (cycle length 365-690 ms) were studied with activation and entrainment mapping of the MA. RESULTS: Patients showed QRS morpho

Epicardial adipose tissue-based defragmentation approach to persistent atrial fibrillation: its impact on complex fractionated electrograms and ablation outcome.

BACKGROUND: Increased epicardial adipose tissue (EAT) volume is associated with atrial fibrillation (AF). However, the efficacy of EAT-based left atrial (LA) ablation for persistent AF (PsAF) is unclear. OBJECTIVE: The purpose of this study was to assess whether EAT-based LA ablation is effective for PsAF. METHODS: In 60 PsAF patients (group I), 3-dimensional reconstructed computed tomography images depicting EAT were merged with NavX-based dominant-frequency (DF) and complex fractionated electrogram (CFE) maps obtained during AF. Pulmonary vein antrum isolation (PVAI) was followed by map-gu

Initial experience of sequential surgical epicardial-catheter endocardial ablation for persistent and long-standing persistent atrial fibrillation with long-term follow-up.

BACKGROUND: Patients with long-standing persistent (LSP) atrial fibrillation (AF) who have previously undergone catheter ablation represent a challenging patient population. Repeat catheter ablation in these patients is arduous and associated with a high failure rate, whereas surgical ablation can be complicated by multiple flutters. We sought to determine if minimally-invasive surgical ablation, followed by catheter ablation of all inducible flutters, would improve success rates over repeat catheter ablation alone. METHODS: Fifteen patients (Sequential) with persistent or LSP AF who failed a

Sympatho-excitatory response to pulmonary chemosensitive spinal afferent activation in anesthetized, vagotomized rats.

The sensory innervation of the lung is well known to be innervated by nerve fibers of both vagal and sympathetic origin. Although the vagal afferent innervation of the lung has been well characterized, less is known about physiological effects mediated by spinal sympathetic afferent fibers. We hypothesized that activation of sympathetic spinal afferent nerve fibers of the lung would result in an excitatory pressor reflex, similar to that previously characterized in the heart. In this study, we evaluated changes in renal sympathetic nerve activity (RSNA) and hemodynamics in response to activati

Differences in organization between acute and chronic atrial fibrillation in dogs.

OBJECTIVES: The purpose of this study was to determine differences in acute and chronic atrial fibrillation (AF) "organization" in canine models. BACKGROUND: Electrophysiologic changes occur during atrial remodeling, but little is known about how remodeling affects AF organization. We hypothesized that atrial remodeling induced by long-term rapid atrial rates heterogeneously decreases AF organization. METHODS: In seven dogs, acute AF was induced by atrial burst pacing, and in eight dogs chronic AF was created by six weeks of continuous rapid atrial pacing. Atrial fibrillation was epicardiall

High-Voltage Focal Pulsed Field Ablation to Treat Scar-Related Ventricular Tachycardia: The First-in-Human VCAS Trial.

BACKGROUND: Unlike the tremendous progress made in atrial fibrillation ablation, the greatest unmet clinical need is for innovative ablation treatments for scar-related ventricular tachycardia (VT), particularly given the thick, often fibrotic tissue characteristic of the scarred substrate. A focal pulsed field ablation (PFA) catheter with a novel waveform was designed: high voltage to provide tissue penetration and low energy (using short-duration pulses) to avoid tissue overheating. We present the outcomes of VCAS (Ventricular Catheter Ablation Study), a first-in-human study of this PFA cath

Phasic electrical remodeling of ventricular myocardium affects arrhythmogenesis in rats with type 1 diabetes mellitus.

BACKGROUND: Diabetes mellitus (DM) causes myocardial electrical remodeling and promotes ventricular tachycardia and/or fibrillation (VT/VF). However, experimental studies have been frequently unsuccessful in developing a DM model with the expected high level of arrhythmic outcomes. The present study aims at evaluating cardiac electrophysiological properties in the rats with different Type 1 DM (T1DM) durations and identifying an electrophysiological phenotype associated with the high incidence of VT/VF. METHODS: The experiments were performed in 109 male Wistar rats (6-10 weeks old), subdivid

Modified Unipolar Return Pulsed Field Ablation in Ventricular Myocardium.

BACKGROUND: Various pulsed field ablation (PFA) parameters have been proposed to improve lesion depth. This study evaluated a modified unipolar return PFA system to create deep lesions in healthy and infarcted ventricular myocardia. METHODS: Numerical modeling was used to compare a modified unipolar return PFA system configuration with a conventional unipolar return (skin patch). We then performed ablation in 14 swine (5 with chronic myocardial infarction and 9 healthy). PFA lesions were created in the left ventricle using a focal catheter (4-mm tip) with a return electrode positioned in the

Low-Dose Cardiac Radiation Improves Electrical Function and Reduces Ventricular Arrhythmogenesis in Mice With Nonischemic Cardiomyopathy.

BACKGROUND: Clinical cardiac radiation therapy (RT-25 Gy) decreases ventricular tachycardia (VT). Nonischemic cardiomyopathy (NICM) murine studies showed improved left ventricular (LV) ejection fraction (LVEF) with low-dose RT (LDRT-5 Gy), attributed to a decrease in macrophages. However, whether LDRT reduces VT remains unknown. OBJECTIVES: The goal of this study was to investigate the effects of LDRT on VT in NICM mice. METHODS: NICM was modeled by using long-chain acyl-CoA synthetase-1 (ACSL1TG) mice. Post LDRT or sham treatment, ACSL1TG mice underwent echocardiography, epicardial mapping …
