The aim of this clinical pilot study is to evaluate the possibility and from a statistical point of view, the predictability of a new mucogingival technique. The purpose of this surgical procedure is to cover the exposed buccal root surface with less than 5 mm clinical probing attachment loss either in corono-apical or mesio-distal aspects. This surgical procedure is particularly indicated in either single or multiple gingival recessions with perfectly preserved mesial and distal interproximal osseous crests, and with a papillary dimension not inferior to the defect which needs to be treated. …

BACKGROUND: Despite being the most predictable root coverage technique, coronally advanced flap with subepithelial connective tissue graft (CAF + SCTG) necessitates graft harvesting from a second surgical site; increasing patient’s post-operative discomfort. Besides, due to the promising regenerative capacity of the periosteum and omitting the need for a second surgical site, periosteal pedicle graft (PPG) is tested as an alternative technique. Eighteen RT1 and RT2 gingival recession defects were randomly assigned to receive either CAF + SCTG or inverted PPG (iPPG). Postoperative discomfort, r…

Background and Objectives: Combined orthodontic-orthognathic treatment is the standard approach for managing severe dentofacial deformities. Nevertheless, its potential impact on periodontal tissues, particularly gingival recession (GR), remains a matter of clinical concern. This study aimed to evaluate the relationship between orthodontic-orthognathic procedures and GR based on available clinical evidence, with particular focus on potential risk modifiers, including orthodontic mechanics, surgical factors, and periodontal phenotype. Materials and Methods: This systematic review was conducted …

Gingival recession is a highly prevalent mucogingival condition that can significantly compromise oral health and esthetics. Patients commonly suffer from dentinal hypersensitivity, root caries, poor esthetics, discomfort during brushing, and in severe cases tooth mobility or tooth loss, ultimately affecting their quality of life. To compare the effectiveness of Platelet Rich Fibrin (PRF) and Amnio-Chorion Membrane (ACM) with Coronally Advanced Flap (CAF) to manage the case of isolated gingival recession defects (GRD). The study is a split-mouth randomised trial that included 13 patients (n = …

This case report describes the use of the modified laterally positioned flap (LPF) technique associated with a subepithelial connective tissue graft (SCTG) for root coverage of homologous mandibular molars with deep gingival recession (GR). A 25-year-old woman with deep GR affecting the mandibular right and left second molars (teeth 31 and 18, respectively) reported bilateral dentinal hypersensitivity. The defect in tooth 31 was 5 mm deep and 4 mm wide. In tooth 18, the defect was 6 mm deep and 5 mm wide. There was 1 mm of keratinized tissue at tooth 31, and no keratinized tissue was present a…

The aim of this randomized controlled trial was to compare the clinical outcomes of modified coronally advanced tunnel technique (MCAT) with subepithelial connective tissue graft (SCTG) with and without recombinant human platelet-derived growth factor-BB (rhPDGF-BB) for mandibular multiple recessions. Twenty-four Miller Class I and III recessions were randomly assigned to a group: test (MCAT+SCTG+rhPDGF-BB) or control (MCAT+SCTG). After 6 months, mean recession-depth reduction was higher in the test group (2.08 ± 0.90 mm) than the control (1.83 ± 0.93 mm). Mean root coverage was achieved, favo

Gingival recession on the lingual aspect of teeth may cause dentinal hypersensitivity problems in patients. Treatment of such recessions is not a regular procedure owing to its anatomical restraints, difficulty in isolation as well as lack of esthetic importance. The present case describes the use of connective tissue graft (CTG) in the treatment of isolated lingual recession on mandibular lateral incisor using minimally invasive tunneling technique. Six-month posttreatment follow-up showed a root coverage of 3.5 mm with enhanced width of keratinized tissue. This report encourages the applicat…

This case report describes alveolar ridge augmentation around a hopeless maxillary canine with severe buccal gingival recession and periodontitis using orthodontic extrusion and a 90-degree buccal root torque in preparation for implant placement. After the orthodontic therapy, the palatal surface of the canine root reached the top of the alveolar bone, parallel to the occlusal plane, with newly formed bone and keratinized mucosa. An implant was successfully placed without combined bone augmentation. This technique may be a useful, minimally invasive approach for implant site development where …

Mucosal fenestrations are rarely encountered in clinical practice, and as such their management is not often reported. Their treatment might be further complicated due to a communication with the oral environment, making them more susceptible to accumulation of debris, plaque, and calculus, thereby reducing the probability of mucosal renewal. The aim of the present case report is to highlight one such rare clinical scenario and its apt and effective management. Surgical management of an uncommon presentation of concomitant gingival recession with an isolated mucosal fenestration in an atypical…

Introduction. Gingival recession is an apical shift of the gingival margin with exposure of the root surface. This migration of the marginal tissue leads to esthetic concerns, dentin hypersensitivity, root caries, and cervical wear. It is, paradoxically, a common finding in patients with a high standard of oral hygiene, as well as in periodontally untreated populations with poor oral hygiene. Changing the topography of the marginal soft tissue in order to facilitate plaque control is a common indication for root coverage procedures and forms a major aspect of periodontal plastic surgeries. The…

BACKGROUND: Controversy exists in the literature between the role of orthodontic treatment and gingival recession. Whilst movement of teeth outside the alveolar bone has been reported as a risk factor for gingival recession, others have found no such association.
FINDINGS: The Angle Society of Europe devoted a study day to explore the evidence surrounding these controversies. The aim of the day was for a panel of experts to evaluate the current evidence base in relation to either the beneficial or detrimental effects of orthodontic treatment on the gingival tissue.
CONCLUSIONS: There remains…

Localised gingival recessions continue to represent an important aesthetic condition requiring treatment in periodontics. Various techniques have been tried to treat exposed root surfaces to improve aesthetics with high percentage of success and minimal discomfort. Root biomodification is done to improve the predictability of these procedures. This clinical report describes periodontal plastic procedure involving subepithelial connective tissue graft with lateral repositioned flap technique and root biomodification with CO(2) laser for the management of gingival recession.
