Repeatability of total Keratometry and standard Keratometry by the IOLMaster 700 and comparison to total corneal astigmatism by Scheimpflug imaging.

OBJECTIVES: We aimed (1) to assess the repeatability of Total Keratometry (TK) and standard keratometry (K) measurements, as provided by the IOLMaster 700 (Carl Zeiss Meditec), and (2) to compare the corneal astigmatism measured by TK to the total corneal astigmatism (TCA) measured by a Scheimpflug camera (Pentacam AXL, Oculus). METHODS: Two groups of patients were prospectively enrolled: Group A included previously unoperated eyes undergoing cataract surgery, and Group B eyes with previous myopic corneal excimer laser surgery. TK and K were measured three times by the same examiner. Repeatab

Double-blind corticosteroid trial in identical twins following photorefractive keratectomy.

The use of topical corticosteroids following excimer laser photorefractive keratectomy (PRK) is widespread, but the role of corticosteroids remains poorly defined. The purpose of this study was to evaluate the use of different corticosteroid postoperative regimens following 193-nanometer excimer PRK in identical twins. Preoperatively, cycloplegic refraction of the left eye of each twin measured -3.00 D sphere. The full correction was treated utilizing the identical technique within the same 30 minute period. The study was conducted in a double-blind fashion and the refraction, corneal clarity,

[Intraocular lens power calculations in eyes with previous corneal refractive surgery].

Results of intraocular lens power calculation were lower than actual required power in eyes with previous corneal refractive surgery because the inaccurate readings of corneal power obtained from standard keratometry. This causes the unacceptable hyperopia after cataract surgery. With increasing of population undergone excimer laser refractive surgery, this problem will be an obvious problem in years or decades. Several methods have been proposed to increase the accuracy of intraocular lens power calculation for cataract surgery in eyes undergone refractive surgery and are reviewed in this art

[Transient band keratopathy associated with hypercalcemia].

PURPOSE: Band keratopathy is a corneal degenerative disease that can produce reduced visual acuity. Different ethiogical mechanisms are described, associated with ocular or systemic diseases, and treatments like EDTA, lamellar keratoplasty and excimer laser photokeratectomy. We report a male with band keratopathy and increased calcic serum that decreased after treatment. CONCLUSIONS: Our patient's clinical course exemplified the importance of considering systemic causes in band keratopathy, and further that the correction of calcium levels can be undertaken before other more aggressive treatm

Photorefractive keratectomy for the treatment of myopia after epikeratoplasty: a case report.

An aphakic eye with a +9.00 diopter (D) refraction developed high myopia of -13.00 D after applying epikeratoplasty using a +14.00 D lenticule. We preformed photorefractive keratectomy by excimer laser. The procedure was uneventful, with no postoperative pain. Epithelialization occurred as usual. Recovery of the cornea followed the usual course with initial overcorrection followed by certain regression. After 7 months follow-up the spherical equivalent was -3.25 D and the lenticule was clear. In myopia after epikeratoplasty photorefractive keratectomy is an effective alternative to lenticule e

[Correction of refractive errors by subtractive corneal surgery: History and perspectives].

The goal of this review is to trace the history of refractive correction, from the earliest optical devices to modern refractive surgical techniques. Following the invention of spectacles in the Middle Ages, the first corneal procedures, such as Sato's and Fyodorov's keratotomies and Barraquer's keratomileusis, highlighted the limitations of early mechanical approaches. In the 1980s, the advent of the excimer laser revolutionized refractive surgery with the introduction of PRK (photorefractive keratectomy) and subsequently LASIK (Laser-Assisted In Situ Keratomileusis). The development of the f

Photodynamic therapy following carbon dioxide laser enhances efficacy in the treatment of extramammary Paget's disease.

Surgical resection is the first-choice therapy for extramammary Paget's disease, but extensive resection is highly invasive and non-surgical treatments are sometimes preferred. Although photodynamic therapy (PDT) has been used for extramammary Paget's disease for some time, recurrence and residual tumour cells are common. In the present study, five patients with extramammary Paget's disease with a total of eight lesions first underwent carbon dioxide (CO2) laser abrasion, followed by 3 h of occlusive application of aminolaevulinic acid (ALA) and then 100 J/cm2 irradiation with a 630-nm excimer

Long-term viability of allogenic donor stroma.

We report the clinical history and histopathology of the longest known postoperative course of a myopic cornea lenticule implanted via epikeratoplasty that also had been subjected to excimer laser and mitomycin. Despite the mechanical and photochemical processes the allogenic cornea stroma had sustained, it is notable that it retained structural integrity and clarity. This report affirms the potential long-term clarity and resilience of allogenic stromal cornea and supports its use as a potential stabilizing option in cornea allogenic intrastromal ring segments for keratoconus and variations t

Microkera to mechronicles : Management of a free flap.

BACKGROUND: In LASIK (laser in situ keratomileusis), a hinged corneal flap is made, which enables the flap to be lifted and the excimer laser to be applied to the stromal bed. If the hinge of the corneal flap detaches from the cornea, the flap is called a free cap. A free cap is a rare intra-operative complication of LASIK most commonly associated with the use of a microkeratome on corneas with flat keratometry, which predisposes to a small flap diameter. Free caps are preventable and treatable. Rarely does the complication lead to a severe or permanent decrease in visual acuity. PURPOSE: As

Overview of laser refractive surgery.

Since approval of the use of the excimer laser in 1995 to reshape the cornea, significant developments in the correction of refractive errors such as myopia, hyperopia, and astigmatism have been achieved. Combined with other advanced ophthalmological instruments (e.g. anterior segment imaging systems, the femtosecond laser, wavefront-guided customized ablation) and the knowledge accumulated concerning the basic science of refractive errors (e.g. biomechanics and wound healing of the cornea, higher-order aberrations), laser refractive surgery has promisingly outshone other conventional techniqu

Laser refractive surgery.

BACKGROUND: Refractive errors; myopia, hypermetropia and astigmatism, have traditionally been corrected by glasses and more recently contact lenses. For nearly 100 years surgical techniques to treat such disorders have also been developed. The latest of these are based upon the use of the excimer laser to ablate the corneal stroma either directly under the corneal epithelium (surface PRK) or under a flap of superficial stroma (LASIK). OBJECTIVE: To look at the development of laser refractive surgery and to put this in the context of appropriate and inappropriate indications for usage. DISCUS

An in vitro evaluation of electromagnetic interference between implantable cardiac devices and ophthalmic laser systems.

AIMS: Ophthalmic laser treatments are discouraged in patients with implantable pulse generators (IPGs, pacemakers) and implantable cardioverter defibrillators (ICD) due to potential effects of the electromagnetic interference (EMI) emitted by ophthalmic laser systems. We assessed the effects of EMI generated by ophthalmic laser systems and laser discharge on IPG and ICD function. METHODS AND RESULTS: Two implantable devices, one Victory dual-chamber IPG (St Jude Medical, Minneapolis, MN, USA) and one Atlas II + dual-chamber ICD (St Jude Medical), were consecutively placed in a simulated thora

The stenotic stent: mechanisms and revascularization options.

As demonstrated by the two recent randomized studies of elective, single stent placement versus balloon angioplasty of de novo lesions in the coronary arteries, angiographic restenosis occurs significantly less after stent implantation. However, reported stent restenosis rate varies from 14% to more than 60%, depending on patient characteristics, stent design, number of stents implanted, vessel treated, location of the lesion, and acute luminal gain. The lowest rate of stenosis occurs in de novo lesions. The highest rate of stent restenosis is encountered in multiple stents and in ostial saphe

Femtosecond-laser-assisted Small Incision Lenticule Extraction to Treat Refractive Error: A Narrative Review.

With the approval of the Federal Drug Agency of the USA in 2018, femtosecond laser-assisted small incision lenticule extraction (SMILE) became an officially available option in several countries including Saudi Arabia to treat myopia and astigmatism. Intense marketing by industries has placed both clients and corneal surgeons into a dilemma of the plus and minuses of this option. Although several reviews compare SMILE to other conventional methods, especially laser in situ keratomileusis, this narrative review perhaps for the first time offers a detailed summary of this procedure and compares
