Orthognathic Surgery in Completely Edentulous Patients: A Systematic Review of Planning and Outcomes.

The management of dentofacial deformities in completely edentulous patients remains one of the most demanding challenges in maxillofacial surgery due to the absence of occlusal references and severe bone resorption. This systematic review, conducted in accordance with PRISMA guidelines, critically analyzed available evidence on the planning and execution of orthognathic surgery in this population. A comprehensive search of 8 databases identified 8 eligible studies encompassing 34 fully edentulous patients. Le Fort I osteotomy was the predominant procedure (91%), frequently combined with bone g

Piezoelectric-assisted osteotome-mediated sinus floor elevation: an innovative approach.

Sinus floor elevation can be done by lateral maxillary approach or crestal approach by osteotome techniques as a function of residual bone height. The osteotome technique has been developed for minor sinus floor elevation. This is a method with relatively high predictability but is technically demanding. During the past decade, different osteotome-mediated sinus floor elevation techniques have been developed to augment the sinus for predictable implant placement. All these methods aim to access a better, predictable sinus augmentation with low morbidity. Some of these techniques require specia

Reconstruction of the severely atrophic edentulous maxilla using Le Fort I osteotomy with simultaneous bone graft and implant placement.

PURPOSE: This study evaluated the outcome of patients who underwent simultaneous Le Fort I osteotomy, an interpositional bone graft, and implant placement for reconstruction of the severely atrophic edentulous maxilla. PATIENTS AND METHODS: Twenty patients operated by the same surgeon were included in the study. Patients were followed annually with clinical and radiographic examinations for an average of 33 months. RESULTS: One hundred thirty-nine implants were initially inserted in the bone grafts at the time of Le Fort I osteotomy. Twenty-five implants (18%) failed to osseointegrate. Seven

Long-term follow-up of implants placed after Le Fort I osteotomy with interpositional autogenous bone grafts: a retrospective study.

BACKGROUND: Oral implantology has become a standard procedure for replacing missing teeth. However, patients with severe maxillary bone resorption often require complex surgical interventions, such as Le Fort I osteotomy with interpositional bone grafting, prior to implant placement. This study aimed to evaluate the long-term outcomes of oral implants placed following this surgical technique. PATIENTS AND METHODS: The present study was two-fold. First, a retrospective review was performed using medical records and conventional panoramic radiographs from 26 patients who had undergone Le Fort I

A Pilot Study Assessing the Incidence of Chronic Sinusitis Following Le Fort I Osteotomy in Maxillofacial Surgery.

Orthognathic surgery utilizing a Le Fort I osteotomy is performed regularly by oral surgeons to correct midface and dental occlusal abnormalities, yet little has been written discussing the impact these operations may have on sinonasal function. The objective of this study was to assess the incidence of objective sinonasal inflammation and subjective sinonasal symptoms following the use of Le Fort I osteotomies for maxillary advancement surgery.Thirty-eight subjects who previously underwent Le Fort I osteotomies for purposes of elective orthognathic surgery were enrolled retrospectively to ass

The effect of an early Le Fort III surgery on permanent molar eruption.

The purpose of this retrospective study was to evaluate the extent to which an early Le Fort III osteotomy affects the position and eruption of the permanent maxillary first and second molars. To test the null hypothesis that there are no changes in eruption patterns, 31 patients diagnosed with craniosynostoses (13 with Crouzon's syndrome, nine with Apert's syndrome, eight with Pfeiffer's syndrome, and one with Carpenter's syndrome) with a mean age at the time of surgery of 5.3 +/- 1.3 years were studied. All patients underwent a Le Fort III osteotomy performed by a single surgeon to correct t

Double sleeve guided placement of quadruple zygomatic implants for rehabilitation of a patient with ectodermal dysplasia: A clinical report.

The surgical and prosthodontic treatment for a 22-year-old man with ectodermal dysplasia is described and illustrated. He had never managed to wear complete dentures, and implant-retained or implant-supported prostheses were indicated. However, the placement of conventional maxillary endosseous implants was contraindicated. A novel surgical template with double sleeves was used to guide osteotomies for 4 zygomatic implants used with an unloaded, one-stage approach. After confirming osseointegration, prosthetic rehabilitation began with an interim implant-supported fixed prosthesis to evaluate

The Surgical Management of Skeletal Disproportion with Lingual Orthodontics and Three-dimensional Planning.

This case report describes the successful treatment of a 26-year-old Caucasian male with skeletal and dental Class III malocclusion associated with mild maxillary and mandibular crowding. The patient had anteroposterior and transverse discrepancies with a reverse overjet and bilateral posterior crossbites. The nonextraction treatment plan included aligning and leveling of the teeth in both arches, Le Fort I and bilateral sagittal split osteotomies, and postsurgical correction of the malocclusion. Orthodontic treatment was initiated with custom lingual appliances followed by orthognathic surger

Maxillary deformities.

The management of congenital maxillary anomalies can be managed with ease by the well trained facial plastic surgeon. An understanding of dental anatomy, occlusion, and the rudiments of cephalometric analysis are important prerequisites for this type of work. A background in the management of facial trauma prepares the surgeon for the surgery involved. A sense of aesthetics is required to adequately anticipate what the final cosmetic and functional outcome should be in these cases. The Le Fort III osteotomy is particularly difficult, especially if there is associated hypertelorism that require

Modified Rotational Anterior Segmental Osteotomy for Prevention of Common Complication (Aged Appearance).

Although anterior segmental osteotomy (ASO) is powerful procedures in treating the patient who has bimaxillary dentoalveolar protrusion, there is significant change in all soft and hard tissue parameters. Soft tissue changes in response to ASO can have a deleterious effect of an aged appearance. To prevent this aged appearance, the authors tried to rotate the maxillary anterior segment rather than simply posteriorly translating the segments. The application of a clockwise pivot rotation method during ASO can prevent overcorrection by controlling the degree of setback of the piriform aperture,

Simultaneous Paranasal Augmentation and Le Fort I Osteotomy Using a Patient-Specific Implant Designed to Share the Fixation Space: Two-Year Clinical Outcomes.

BACKGROUND: Residual paranasal concavity may persist after Le Fort I osteotomy despite adequate maxillary repositioning, limiting the aesthetic impact of orthognathic surgery. Simultaneous paranasal implant placement is rarely reported due to spatial conflict with rigid internal fixation systems and concerns regarding infraorbital nerve safety. METHODS: Two patients underwent Le Fort I osteotomy combined with simultaneous placement of patient-specific paranasal implants designed through 3-dimensional virtual surgical planning (VSP) to intentionally share the anatomical space occupied by Le Fo

Stability of unilateral sagittal split ramus osteotomy for correction of facial asymmetry: long-term case series and literature review.

Bilateral sagittal split ramus osteotomy is considered a standard technique in mandibular orthognathic surgeries to reduce unexpected bilateral stress in the temporomandibular joints. Unilateral sagittal split ramus osteotomy (USSO) was recently introduced to correct facial asymmetry caused by asymmetric mandibular prognathism and has shown favorable outcomes. If unilateral surgery could guarantee long-term postoperative stability as well as favorable results, operation time and the incidence of postoperative complications could be reduced compared to those in bilateral surgery. This report hi

Facial sensibility in adolescents with and without clefts 1 year after undergoing Le Fort I osteotomy.

Static two-point discrimination, pressure, and vibratory threshold values were measured at standard coordinates in the area corresponding to the infraorbital nerve distribution in 59 adolescents 1 year after undergoing LeFort I osteotomy. The patients were divided into three groups: those born with unilateral cleft lip and palate (n = 30; mean age, 18 years; SD = 2); those with bilateral cleft lip and palate (n = 12; mean age, 19 years; SD = 3); and those without a cleft (n = 17; mean age, 19 years; SD = 3). One year after the osteotomy, none of the patients complained of altered sensibility i

Speech and velopharyngeal function following maxillary advancement in patients with cleft lip and palate.

The influence of maxillary advancement by osteotomy on speech was examined in 10 patients with cleft palates (6 males, 4 females). Ages at the time of surgery ranged from 16 to 26 years (mean, 19.5 yr). LeFort I and II osteotomies were performed in 9 patients and 1 patient, respectively. Preoperatively and postoperatively, hypernasality, nasal emission on pressure consonants, and articulation disturbances were evaluated perceptually, and velopharyngeal function was evaluated by lateral cephalographic and nasopharyngoscopic studies. Hypernasality, which had been judged preoperatively to be abse

[Total atrophy of the mandibular alveolar crest. A new therapeutic proposal (the use of a tissue expander) and a clinical case report].

Atrophy of the alveolar crest, meant as a syndrome due to a certain variety of pathogenetic episodes developing in a more or less marked framework of resorption of the alveolar to the maxillary processes, represents an issue of debated and uncertain solution, especially in a view of the extreme degrees of the phenomenon. Various kinds of surgical approach have been suggested over time: among them mention should be made of the onlay graft technique, the osteotomy intervention visor type, the osseous interposition sandwich type, and other methods, which were often the result of a pontered mediat

Orthodontic premaxillary setback versus premaxillary osteotomy with gingivoperiosteoplasty for Bilateral cleft lip and palate patients: 4-year observation outcomes.

UNLABELLED: Patients with bilateral cleft lip and palate (BCLP) generally require several stages of treatment, and adequate maxillary development without orthognathic surgery is not easy to achieve. The purpose of this study was to determine short-term outcomes of orthodontic premaxillary setback (OPS) and premaxillary osteotomy (PO) for complete BCLP treatment. PATIENTS AND METHODS: Twenty-six patients with BCLP were consecutively treated for 4 years by a single surgeon. All patients were categorized by three indications for the first operation: OPS1, cheiloplasty + gingivoperiosteoplasty (G…

Slide preservative rhinoplasty stabilized with intra oral titanium plates in deviated nose - a technical note.

The dorsal preservation technique is an excellent way to perform rhinoplasty when anatomically appropriate. Certain post-traumatic deviated noses with a harmonious dorsum and no prominent hump can be managed through slide preservative rhinoplasty. This technic enables nasal straightening with minimal dissection of nasal and septal tissues. An intraoral approach is employed to perform osteotomies and stabilize the result using maxillary bone plates for osteosynthesis. Slide preservative rhinoplasty with intraoral approach and osteosynthesis represents an innovative, expedient, and efficient way
