Herpes zoster (HZ) is caused by the reactivation of varicella zoster virus. The incidence of herpes zoster and associated problems increases with age. With a life-long prevalence of 30%, every second 85-year-old person experiences HZ once in his lifetime. Three therapeutic columns are based on antiviral, topical and analgetic therapies. Pregnancy is known to aggravate pre-existing chronic painful conditions. We reported a case of secondary infraorbital neuralgia due to a foreign body involving the nasal ala. As it was a very rare care, we reviewed reported literatures. This case highlights that infraorbital neuralgia could be caused by any tiny problems such as foreign body involving the area innervated by infraorbital nerve.

This 31-year-old woman presented with typical right trigeminal neuralgia caused by a trigeminocerebellar artery, manifesting as pain uncontrollable with medical treatment. It is unclear whether hypertension (HTN) is a predisposing factor for the development of trigeminal neuralgia (TN). The purpose of this study was to determine the prevalence of HTN in TN patients and controls at the USC Orofacial Pain and Oral Medicine Center. A retrospective chart review was conducted from a database of over 3,000 patient records from 2003 to 2007. We identified patients diagnosed with TN with or without HTN. Surgery should be considered only after anticonvulsant medications have failed or if medical treatment is not well-tolerated, including in cases of asthenia or drowsiness. In most reference centers, consensus is that MVD is the first option when patients are in good health. Pure microvascular decompression (MVD) can cure (that is, no pain, no medication) primary trigeminal neuralgia (TN) caused by vascular compression in 75% of patients (90% when compression is pronounced), according to a Kaplan-Meier survival study at 15 years. MRI with high resolution evidences neurovascular conflicts with good reliability.
