Bone graft implantation for correction of the anophthalmic orbit.

Prosthetic rehabilitation of the anophthalmic orbit may result in a syndrome characterized by retrodisplacement and tilt of the prosthesis, deepening of the upper eyelid sulcus, retraction of the upper eyelid, and stretching of the lower eyelid. Patients requiring enucleation of the orbit following trauma can also have accentuation of the enophthalmos, with hypophthalmos and displacement of the malar eminence. These cosmetic defect are difficult to manage, and a number of corrective surgical techniques have been described. In this study we review the pathophysiology of the anophthalmic orbit a…

Aspergilloma in the frontal sinus expanding into the orbit.

A case of primary frontal sinus aspergilloma in a 79 year old non-immunocompromised woman, who presented with a right sided pyocele expanding into the orbit, is presented. The low susceptibility of the frontal sinus is probably related to the brachycephalic shape of the human skull which locates this sinus far anterosuperiorly to the nasal cavity. In human frontal sinus aspergillosis nasal symptoms are absent; the clinical manifestation of this rare disease is initiated by complications, especially orbital or intracranial invasion. Sinus opacity may raise early suspicion. Treatment consists of…

Multidisciplinary approaches to complicated vascular lesions of the orbit.

BACKGROUND: Hemodynamically active vascular lesions of the orbit are often clinically challenging to treat, both because of the complexity of the vascular anatomy and the delicacy of the cardinal structures that are involved and vulnerable to damage. Attempts to achieve complete embolization of such vascular lesions may result in damage to vital visual structures. Conversely, incomplete closure of the feeders or vascular shunts will usually result in recanalization and, therefore, recurrence of such lesions. METHODS: Surgical excision after selective optimal embolizations may convert dynamic …

Aesthetic considerations in the reconstruction of the anophthalmic orbit.

Twenty-four patients with severe anophthalmic orbit syndrome were treated with a combination of techniques utilizing autogenous tissue. Autogenous corticocancellous bone grafts were used in the orbit as a volume filler. The temporalis muscle and a fasciae flap with a pericranial flap were used as a curtain to cover the bone grafts and to give a softening effect. The prosthesis used was a regular eye shell. With this method it can be smaller, thinner, and lighter to produce less deformation of the lower lid, particularly when used for a long period of time. In children who have this deformity c…

[Computed tomography of the orbit. I. Trauma-induced changes].

The CT findings in 120 patients following trauma to the orbits, and in 60 patients with iatrogenic abnormalities, have been analysed. CT has been found to be extremely useful in the diagnosis of trauma and postoperative changes by its ability to demonstrate fractures, foreign bodies, haematomas, abscesses, soft tissue displacement or perforation of orbital structures, as well as showing prosthetic material.

Endodermal sinus tumor (yolk sac tumor) of the orbit.

The endodermal sinus tumor is a malignant germ cell neoplasm that usually arises in gonads, but on rare occasion occurs in extragonadal locations. Five tumors of the orbit, which by light microscopy displayed features characteristic of gonadal endodermal sinus tumor, were tested for the presence of alpha fetoprotein using paraffin-embedded tissue and an immunoperoxidase technique. Each tumor contained intracytoplasmic alpha fetoprotein. Review of the clinical histories revealed that orbital endodermal sinus tumors differ from other extragonadal endodermal sinus tumors in that they occur at a y…

Fibrous dysplasia of the orbit with optic nerve decompression.

We describe an unusual case of monostotic fibrous dysplasia involving the right sphenoid bone with gradual loss of vision occurring in a sexually precocious young woman. Vision remained stable after a four-year follow-up surgical decompression of the right optic canal.

Lymphoma and other lymphoid lesions of the orbit (II).

The incidence, survival rate and causes of death of patients with orbital lymphomas and other lymphoid lesions of the orbit in Denmark, during the period 1943-62 inclusive, were studied by means of a clinico-pathological analysis. In the period examined, the incidence of histologically verified primary orbital tumours was 0.06%. The incidence of primary orbital lymphomas and lymphoid lesions was at least 0.012%. These can be histologically divided into 3 categories: benign, indeterminate (intermediary) and malignant. The present study confirms the relevance of this classification, as there is …
