MR arthrographic depiction of tears of the rotator cuff: benefit of abduction and external rotation of the arm.

PURPOSE: To show the effectiveness of positioning the arm in abduction and external rotation (ABER) during magnetic resonance (MR) arthrography for depiction of tears of the rotator cuff. MATERIALS AND METHODS: Five throwing athletes with shoulder pain were prospectively imaged with standard MR imaging, MR arthrography, and ABER MR arthrography. The status of the rotator cuff was recorded; arthroscopic correlation was obtained. RESULTS: One partial tear of the distal infraspinatus tendon was detected with ABER MR arthrography only. One full-thickness tear was detected with both MR arthrograp

Current concepts and recent advances in the athlete's shoulder.

The demands placed on the shoulder girdle during athletics can often exceed its physiologic limits and result in significant injury. The goal of orthopedists, therapists, trainers, and ancillary personnel who care for athletes involved in sports which require extreme demands of the shoulder is to enhance athletic performance, extend their longevity, and prevent injury. With the new concepts available from basic science and clinical research, a clear understanding of normal architecture and biomechanical function of the shoulder girdle is better appreciated. A greater understanding of sporting

Good outcome after repair of trauma-related anterosuperior rotator cuff tears-a prospective cohort study.

BACKGROUND: Anterosuperior rotator cuff tears with a displaced long head of the biceps tendon are most often trauma-related, and patients with these conditions often present with severe pain and shoulder dysfunction. Repair of the subscapularis and supraspinatus and a biceps tenodesis or a tenotomy are the recommended treatments based on retrospective studies. The aim of this study was to prospectively evaluate clinical and structural treatment outcome in a cohort of trauma-related anterosuperior injuries in patients with previously healthy shoulders. MATERIALS AND METHODS: Patients seeking c

Glucagon-like peptide-1 receptor agonists and rotator cuff disease: a scoping review.

BACKGROUND: Obesity and type 2 diabetes mellitus (T2DM) are associated with impaired rotator cuff tendon healing and inferior clinical outcomes following surgical repair. Glucagon-like peptide-1 receptor agonists (GLP-1RA) are increasingly used to manage obesity and T2DM. However, their influence on rotator cuff disease biology and outcomes following cuff repair remains unknown. METHODS: A scoping review was conducted in accordance with PRISMA-ScR guidelines. MEDLINE, Embase, PubMed, and the Cochrane Library were searched from inception to 28 January 2026 for studies evaluating associations b

Editorial Commentary: Rotator Cuff Repair Augmentation: Arguably the Only Meaningful Improvement in Rotator Cuff Repair Outcomes in Last 40 Years, While Being Cost-Effective.

Rotator cuff repair (RCR) remains one of the most commonly performed orthopaedic procedures and represents a dynamic scientific evolution. This progression toward optimizing patient outcomes has certainly been nonlinear, and it requires that we ask of ourselves: have advances in RCR delivered on this promise? Advances in surgical approach, repair configuration, and the introduction of marrow stimulation have produced largely equivalent outcomes, leaving retear rates persistently high, especially for large and massive tears. To that end, structural augmentation with allograft or xenograft has e

MRI and histological features of supraspinatus tendon degeneration in rotator cuff tears: An underexplored perspective.

BackgroundThe supraspinatus muscle and tendon are an integral part of shoulder joint function and stability. The supraspinatus tendon is one of the four tendons forming the rotator cuff in the shoulder. Supraspinatus tendon tear is a frequent lesion that occurs due to various causes like lifting heavy objects, falls onto the shoulder, a dislocation of the shoulder, swimming, trauma, sports injury, occupational activity and degenerative change. Smoking, hypercholesterolemia, and hereditary predisposition are all established risk factors for developing RC tears, which affect the age dependent te

Surgical anatomy, biomechanics and clinical outcomes of latissimus dorsi transfer for irreparable rotator cuff tears: a review.

BACKGROUND: Latissimus dorsi transfer is a well-established joint-preserving option for irreparable rotator cuff tears in young, active patients without glenohumeral arthritis. Originally described by Gerber in 1988 for posterosuperior tears, indications now also include anterosuperior reconstruction, either as isolated transfer to the lesser tuberosity or in combination with the teres major. No recent review has integrated surgical anatomy, biomechanics, fixation strategies, clinical outcomes and patient selection in a single resource. MAIN TEXT: We reviewed the anatomy, biomechanics, surgic

Potential Misinformation in the Official Disability Guidelines About the Diagnosis and Treatment of Rotator Cuff Tendinopathy.

OBJECTIVE: We selected statements in the Official Disability Guidelines that had the potential to reinforce misconceptions regarding symptoms from rotator cuff tendinopathy. These statements were revised and presented with the original statement to specialists. METHODS: Twelve statements regarding rotator cuff tendinopathy were identified as deviating from principles based on ethics, values, and the evidence regarding both pathophysiology and human illness behavior. One hundred fifteen upper extremity surgeons reviewed both original and revised versions of the statements and indicated their p

[Scapula dyskinesis in medium size full-thickness supraspinatus muscle tears].

INTRODUCTION: Scapular motion abnormality in rotator cuff tears is a well-known symptom, but its significance is not clear. Some authors consider it as a cause of rotator cuff tear, others as a consequence of the disease. OBJECTIVE: The aim of our study was to assess the changes in scapular motion in medium size full-thickness rotator cuff tear of degenerative origin compared to a healthy control group. MATERIAL AND METHOD: 10 healthy (control group) and 9 subjects with a medium size (1-3 cm), complaining rotator cuff tear (study group) were included in our study, in whom we analyzed the mov

Editorial Commentary: Social Determinants of Health Influence the Success of Shoulder Rotator Cuff Repair Outcomes: When You Do Everything Right, Yet Things Still Go Wrong.

Our ability to perform a technically sound surgery is not sufficient to ensure patients have an excellent clinical outcome. Social determinants of health disparities (SDHDs) profoundly impact health equality. Health disparities that exist in the United States are risk factors for inferior patient-reported outcomes and result in greater complication rates following rotator cuff repair surgery. The presence of SDHDs was associated with an increased risk of revision, stiffness, emergency department visits, medical complications, and costs. Economic and educational SDHDs were associated with the g

Does the presence of arthroscopically detected stage 1-2 glenohumeral osteoarthritis have any clinical impact on the outcome of arthroscopic rotator cuff repairs?

BACKGROUND: : Rotator cuff rupture (RCR) and glenohumeral osteoarthritis (GHO) are two common disorders of the shoulder joint. However, there are very few reports that examine the relationship between them. This study aimed to present at least two years' clinical results of arthroscopic rotator cuff repair of full-thickness and massive tears accompanied by arthroscopically detected early-stage osteoarthritis. METHODS: From August 2016 to December 2017, three hundred and twenty patients with total or massive rotator cuff tears were evaluated retrospectively. Thirty-five patients who were deter

Current Research on the Influence of Statin Treatment on Rotator Cuff Healing.

Rotator cuff tears are a condition characterized by damage to the muscles and tendons that connect the scapula and humerus, which are responsible for shoulder rotation and arm lifting. Metabolic factors such as diabetes, thyroid disease, high cholesterol, vitamin D deficiency, obesity, and smoking have been associated with an increased risk of rotator cuff tears. Interestingly, patients with hyperlipidemia, a condition characterized by high levels of cholesterol and other fats in the blood, have been found to have a higher incidence of rotator cuff tears and breakdown of tendon matrix. As a re

Platelet lysate and tendon healing: comparative analysis of autologous frozen-thawed PRP and ketorolac tromethamine in the treatment of patients with rotator cuff tendinopathy.

Platelet-rich blood derivatives are being nowadays increasingly used in the treatment of tendon-related pathologies as a rich source of growth factors. We sought to ascertain if local application of platelet lysate (PL) to augment rotator cuff repair ameliorates patient outcomes compared to ketorolac tromethamine treated group. A total of forty patients, with clinical diagnosis of Rotator Cuff Tendinopathy were randomized to receive sub acromial injections of PL every week for a total of 3 injections and two injection of ketorolac tromethamine once every two weeks. Editorial Commentary: Predictors of Best Outcomes After Latissimus Dorsi Transfer for Irreparable Rotator Cuff Tear.

The management of patients with massive posterosuperior rotator cuff tears without glenohumeral arthritis remains a challenge to arthroscopic surgeons. A wide variety of treatment options have been described, including latissimus dorsi tendon transfer (LDTT) and lower trapezius tendon transfer. These tendon transfers have been utilized to rebalance the glenohumeral force couple for patients with massive or irreparable posterosuperior rotator cuff tears. Proponents of the latissimus dorsi tendon transfer have touted several theoretical advantages, including improvement of the shoulder fulcrum,

A multicenter randomized controlled trial comparing gamification with remote monitoring against standard rehabilitation for patients after arthroscopic shoulder surgery.

BACKGROUND: Gamification has become increasingly popular in rehabilitation and is viewed as a tool to improve patient activation, motivation, and engagement. The aim of this study was to compare the efficacy of validated exergames played through a system using "depth sensor" and bespoke software against standard physiotherapy in patients treated with arthroscopic shoulder surgery. This included the following common conditions: subacromial impingement syndrome, calcific tendinopathy, and rotator cuff tear. METHODS: Following arthroscopic shoulder surgery, patients were randomized into 1 of 2 g

The role of loading in murine models of rotator cuff disease.

Rotator cuff disease pathogenesis is associated with intrinsic (e.g., age, joint laxity, muscle weakness) and extrinsic (e.g., mechanical load, fatigue) factors that lead to chronic degeneration of the cuff tissues. However, etiological studies are difficult to perform in patients due to the long duration of disease onset and progression. Therefore, the purpose of this study was to determine the effects of altered joint loading on the rotator cuff. Mice were subjected to one of three load-dependent rotator cuff tendinopathy models: underuse loading, achieved by injecting botulinum toxin-A into
