Therapy-resistant symptoms in Parkinson's disease.

In recent years, the management of Parkinson's disease (PD) has come a long way, leading to an increase in therapeutic options that now include oral and transdermal drug delivery, infusion as well as surgical treatments. Nonetheless, in the evolution of this complex neurodegenerative disorder, several symptoms remain refractory to dopaminergic therapy. It is our aim to review the literature to date and to bring them into focus, as well as emphasizing on pathophysiological mechanisms, profile of risk factors in their development, and therapeutic options. We will focus on freezing of gait, campt

Camptocormia as presenting sign in myofibrillar myopathy.

Camptocormia, due to paraspinal muscle weakness, is seen in several types of myopathy. Myofibrillar myopathies (MFM) are histopathologically characterized by desmin-positive protein aggregates and myofibrillar disintegration. Camptocormia can be seen in the late stages of the known MFM diseases. We present a case of MFM with progressive camptocormia since the age of 64, isolated for 6years, followed later by upper and lower limb weakness. Camptocormia has never been described as the presenting clinical sign of MFM. MFM joins the growing number of myopathies potentially presenting with camptoco

Spinal deformity and Parkinson disease: a treatment algorithm.

OBJECT: The authors review the literature on the treatment of spinal deformity in patients with Parkinson disease (PD) and formulate a treatment algorithm. METHODS: The authors provide representative cases of patients with PD and spinal deformity who underwent deep brain stimulation (DBS) or spinal surgery. RESULTS: In patients with PD and spinal deformity who undergo spinal surgery there is a high rate of acute and delayed complications. Patients who undergo DBS, while having significantly fewer complications, often do not regain sagittal balance. CONCLUSIONS: Cases involving PD and campto

Concomitant correction of spinal deformity and spasticity management with posterior instrumentation and baclofen pump implantation.

Patients with spastic quadriplegia secondary to cerebral palsy manifest with severe spasticity and often with neuromuscular scoliosis. In order to optimize treatment and minimize morbidity in selected cases, the authors present a surgical option concurrently addressing these problems. This study reviews one case and describes the surgical technique in detail where intrathecal baclofen pump implantation and posterior spinal fusion are concomitantly performed to manage spasticity and scoliosis, respectively.

One-stage posterior-only correction in severe and rigid scoliosis exceeding 90°: radiographic and surgical outcomes.

PURPOSE: Scoliosis, if left untreated, can lead to severe curvatures exceeding 90°, causing devastating functional impairments. Various surgical methods exist, but the optimal approach is still debated. One-stage posterior spinal fusion (PSF)-only surgery is gaining favor for its comparable outcomes and reduced morbidity. This study, focusing on radiographic and surgical outcomes, evaluates the efficacy and safety of one-stage PSF for scoliosis curves over 90° in pediatric patients. METHODS: Thirty-five pediatric patients with scoliosis (Cobb angle ≥ 90°) who underwent one-stage posterior spi

Fundamental principle of adult spinal deformity.

The management of adult spinal deformity (ASD) requires a personalized, multidisciplinary approach. Effective treatment hinges on thorough assessment using advanced imaging to understand the severity and impact of the spinal curvature. This paper underscores the importance of tailoring treatment plans to individual patient factors such as age, health, and psychological well-being, weighing both surgical and non-surgical options.Non-surgical treatments like pain management and physical therapy are preferred initially. If surgery is necessary, candidate selection and the choice of surgical techn

[Scoliosis in infancy].

1. The paper deals with 101 patients (52 male, 49 female) who have been treated in the first and second years of life because of resolving scoliosis and could be re-examined later at the age of 15 between 17 years. 2. The clinical and roentgenological findings of seven patients only had been normalized, where as in all other cases low lateral deviations of the spinal column (77 times), frequently at a modified type of curvature (44 times) or structural modifications (77 times, 17 spinal columns without lateral deviations), or not rarely, even both of them, had been found. 3. During development

Dual vs. single surgeon in spinal deformity correction: a meta-analysis.

BACKGROUND CONTEXT: Spinal deformity correction is technically demanding and often associated with extended operative time, substantial blood loss, and elevated complication rates. Dual-attending surgeon (DS) models have emerged as a strategy to enhance operative efficiency and patient safety, though published outcomes remain inconsistent. PURPOSE: To evaluate the impact of a DS approach compared with a traditional single-attending surgeon (SS) strategy on perioperative and postoperative outcomes in spinal deformity surgery. STUDY DESIGN/SETTING: Meta-analysis of comparative studies assessin

Different Risk Factors of Rod Fracture between L5-S1 and ≥ L4-5 Levels in Adult Spinal Deformity Surgery.

BACKGROUND: Given the different biomechanical properties and surgical techniques between L5-S1 and at ≥ L4-5 levels, different factors might contribute to rod fracture (RF) development between the L5-S1 and ≥ L4-5 levels. Recognizing segment-specific risk factors can be helpful when planning surgery. Therefore, this study aims to analyze the risk factors for RF separately for L5-S1 and ≥ L4-5 levels after adult spinal deformity surgery. METHODS: This study analyzed consecutive patients who underwent ≥ 5-level fusion including the sacrum or pelvis and were followed up for ≥ 2 years. RF was ass

Adult spinal deformity with low pelvic incidence: a challenging condition demanding a tailored strategy.

BACKGROUND CONTEXT: Existing surgical alignment goals derived from populations with a high pelvic incidence (PI) are not applicable for patients with adult spinal deformity (ASD) and a low PI, who account for a high proportion of Asian populations. The surgical treatment for patients with a low PI is challenging because of their limited pelvic compensation capacity and because there are no criteria to guide corrective spinal deformity surgery in this population. PURPOSE: To develop and validate a tailored sagittal correction strategy for patients with ASD and a low PI. STUDY DESIGN/SETTING:

Camptocormia in a young man with anti-GAD-seropositive stiff-person syndrome.

Stiff-person syndrome (SPS) usually manifests as an autoimmune neuromuscular disorder characterised by pronounced and advancing rigidity, primarily affecting the trunk and proximal muscles. There are various clinical subtypes like classic SPS (truncal stiffness, generalised rigidity and muscle spasms), partial SPS (stiff-limb syndrome) and uncommon forms including progressive encephalomyelitis with rigidity and myoclonus. Camptocormia, defined as forward flexion of the spine in the upright position that disappears in the supine position, without fixed deformity, has been described only in two
