Tendon connects muscle to bone and functions to transmit muscular forces across joints to stabilize or move those joints. Tendons in the foot and ankle are subject to enormous loads and consequently make up a substantial portion of the body's tendon injuries. Understanding the mechanisms of these injuries requires an understanding of the relative rates of muscle, tendon, osteotendinous junction, and myotendinous junction adaptation. This article provides the practitioner with an overview of tendon anatomy, physiology, healing, and repair and correlates didactic and clinical aspects so that pra…

Severe motorcycle spoke injuries of the heel lead to Achilles tendon defects, calcaneal tubercle exposure or loss, and overlying soft tissue defects, which are challenging to treat. Given the special physiological and developmental characteristics of children, severe spoke injuries of the heel in children are especially troublesome. We report details of 31 cases of severe motorcycle spoke injuries of the heel in children. The reconstruction timing depended on the time since injury, systematic conditions, and concurrent injuries. Eighteen cases were reconstructed at the time of emergency surgery

Healing of rotator cuff (RC) injuries with current suture or augmented scaffold techniques fails to regenerate the enthesis and instead forms a weaker fibrovascular scar that is prone to subsequent failure. Regeneration of the enthesis is the key to improving clinical outcomes for RC injuries. We hypothesized that the utilization of our tissue-engineered tendon to repair either an acute or a chronic full-thickness supraspinatus tear would regenerate a functional enthesis and return the biomechanics of the tendon back to that found in native tissue. Engineered tendons were fabricated from bone

Fluoroquinolone toxicity on cartilages and tendons has been well known since 1983. Tendon inflammation or rupture has been described. Achilles tendon rupture is the most frequent complication but many other sites of tendon injuries have been reported. This article presents a case of rupture of extensor tendons of the hand in an elderly woman treated by fluoroquinolones. As far as we know, this site of tendon lesion has never been previously described. Histological examination of tendon injuries was possible after surgical treatment. Histological structures were similar to the classical descrip…

Finger fractures are common injuries with a wide spectrum of presentation. Although a vast majority of these injuries may be treated non-operatively with gentle reduction, appropriate splinting, and careful follow-up, health care providers must recognize injury patterns that require more specialized care. Injuries involving unstable fracture patterns, intra-articular extension, or tendon function tend to have suboptimal outcomes with non-operative treatment. Other injuries including terminal extensor tendon injuries (mallet finger), stable non-articular fractures, and distal phalanx tuft fract…

Tendon injuries, particularly massive tendon defects, pose significant clinical challenges due to the limited regenerative capacity of tendons and suboptimal outcomes of current therapies. This study presents a bioinspired scaffold that integrates genetically engineered extracellular vesicles (EVs) with collagen-targeting capabilities into a decellularized bovine tendon sheet (DBTS) for tendon regenerative repair. Rat tendon-derived stem cells (TDSCs) were genetically modified to overexpress biglycan (Bgn) and fibromodulin (Fmod), producing bioactive EVs that drive tenogenic differentiation vi…

Closed tendon injuries of the hand and wrist are very common in the athletic population. Most of these injuries, especially those involving the extensor tendons, can be treated successfully with nonoperative management if seen acutely. Acute closed flexor tendon injuries, however, usually require operative intervention for the best result. Although evaluation and diagnosis of flexor tendon injuries are relatively straightforward, diagnosis is still commonly delayed secondary to both delayed presentation and missed diagnoses. If diagnosis is delayed, operative intervention becomes less optimal …

Triceps rupture is the least common among all tendon injuries. The usual mechanism of injury is a fall on an outstretched hand, although direct contact injuries have also been reported to cause this injury. The diagnosis of acute triceps tendon rupture may be missed, which can result in prolonged disability and delayed operative management. We presented three cases of acute triceps tendon rupture each at different site showing the spectrum of injury to the muscle and mechanism of injury and management were also discussed.

Tennis injuries are common in both the upper and lower extremities. The most common, and often most difficult, upper extremity injuries are shoulder tendinitis and tennis elbow (lateral and medial). Key considerations in the treatment of tendinitis include an understanding of the injury process and the resultant character and quantity of the pathologic spectrums. Tendon degeneration rather than tendon repair is the primary pathologic entity secondary to intrinsic muscle-tendon overload. For best treatment results, the protocols of treatment, both surgical and nonsurgical, must be individualize

Hamstring injuries can be classified with regard to the site of involvement. Traumatic disorders at the proximal bone-tendon origin are best defined as avulsion injuries, such as ischial tuberosity fractures and hamstring tendon tears. Musculotendinous lesions include muscle strains and muscle contusions. Most hamstring injuries occur after in-direct trauma from excessive stretching or forceful contraction, leading to avulsion injuries or muscle strains and tears. Insufficient warm-up, lack of flexibility, inadequate muscle strength and endurance, or abnormal contraction and running may predis

The current classification of extensor tendon zones has remained unchanged for several decades. In this article, a panel of international experts give their consensus and diverse views about the classification and suggest treatment guidelines for acute extensor tendon injuries for each zone. Collectively they recommend adaptation of a classification which is simpler and resembles that for flexor tendon classification. They outline specific areas of extensor tendon zones, i.e. the distal parts of the fingers, which need particular attention and specific treatment approaches. The treatment appro…

Lameness in athletic horses is often caused by forelimb tendon injuries, especially in the interosseus tendon (TI) and superficial digital flexor tendon (SDF), but also in the accessory ligament (AL) of the deep digital flexor tendon (DDF). In an attempt to explain the aetiology of these injuries, the present study investigated the loading of the tendons during landing after a jump. In jumping horses, the highest forces can be expected in the trailing limb during landing. Therefore, landing kinematics and ground reaction forces of the trailing forelimb were measured from 6 horses jumping singl…

Rotator cuff supraspinatus tendon injuries are clinically challenging due to the high rates of failure after surgical repair. One key limitation to functional healing is the failure to regenerate the enthesis transition between tendon and bone, which heals by disorganized scar formation. Using two models of supraspinatus tendon injury in mouse (partial tear and full detachment/repair), the purpose of the study was to determine functional gait outcomes and identify the origin of the cells that mediate healing. Consistent with previous reports, enthesis injuries did not regenerate; partial tear …

Osseous injuries may be articular, extra-articular, or physeal, and may be related to direct trauma, avulsion forces, or chronic microtrauma. Muscle and tendon injuries about the knee may occur alone or in association with more significant osseous and ligament injuries. Radiographs or computed radiography images remain an important screening technique for patients who have suspected knee injuries. After initial radiographs, MR imaging has become the most common modality for detection and complete evaluation of osseous and soft-tissue injuries about the knee when those radiographs and other tec…

BACKGROUND: Tendon adhesions after hand tendon repair are one of the most difficult complications of hand surgery and can cause severe disability. This study aimed to assess the risk factors associated with tendon adhesions after hand tendon repair to provide a theoretical foundation for the early prevention of tendon adhesions in patients with tendon injuries. Moreover, this study intends to increase doctors' awareness of the issue and serves as a reference for developing new prevention and treatment strategies.
METHODS: We retrospectively analyzed 1,031 hand trauma cases that underwent repa…

Patellar tendon ruptures are functionally devastating injuries that result in failure of the knee extensor mechanism and can lead to a loss of ambulation. Chronic patellar tendon injuries are defined as tears greater than 2 weeks old and are typically more complex to manage than acute tears. Recently, the use of double-row suture anchor configurations has been explored as a technique to provide improved strength in addition to tendon-to-bone compression at the anatomic footprint. The purpose of this article is to describe a surgical technique involving chronic patellar tendon rupture repair us…

Attritional and traumatic injuries to the tendons around the foot and ankle are not uncommon. Treatment of overuse-type injuries (tendinitis) remains straightforward. However, surgical treatment of peroneal subluxation, Achilles tendon ruptures, and posterior tibial tendon insufficiency remains somewhat controversial. Generally speaking, soft-tissue reconstruction of the superior peroneal retinaculum is superior to bony procedures for peroneal dislocation. Open repair of a torn Achilles tendon is more predictable than closed treatment. Good clinical judgment is needed in determining the best t
