After limb amputation, neuromas may be asymptomatic when not compressed, but can cause unexplained discomfort when a prosthesis is worn. The sonographic presentation of multiple postamputation neuromas has rarely been reported. A 40-year-old female with a left, below-elbow amputation suffered from late-onset stump pain and prosthesis intolerance. Physical examination revealed a painful nodule, whereas sonographic findings disclosed three hypoechoic masses derived from the median, ulnar, and radial nerves. Marked pain reduction was reported 2 weeks after sonography-guided steroid injection. Inv

Two cases are presented to illustrate the potential for knee salvage in popliteal artery occlusion from plateau fracture. In both cases, the time interval from injury to exploration precluded revascularization of the lower leg. Because of the necrosis of the deep leg muscles and the nonvital nature of the foot, amputation was required. Careful evaluation of the myocutaneous circulation of the gastrocnemius skin island provides a means of knee salvage. Extensive debridement, salvage of the tibial fragment as a bone graft, and coverage with the vital gastrocnemius and its skin provide a function

Epimorphic regeneration in mice is stimulated at a non-regenerative digit amputation by sequential treatment with FGF2 and BMP2 (FGF2→BMP2). FGF2 stimulates digit amputation wound cells to form a blastema and BMP2 induces blastema differentiation to regenerate the amputated distal phalangeal element, albeit imperfectly. The formation of a phalangeal growth plate suggests that the induced regenerate recapitulates embryonic development and cell lineage studies show that wound cells that enter the blastema cells are positionally re-specified during regeneration. FGF2→BMP2 treatment also stimulate

Complex regional pain syndrome (CRPS) is a debilitating poorly understood condition with unpredictable course and heterogeneous therapeutic responses. Early, coordinated multidisciplinary management, including psychological and psychiatric support, is essential. In severe, therapy-resistant CRPS (long-standing CRPS), particularly intractable infection or severe recurrent wounds, amputation may be considered as a last-resort intervention after thorough multidisciplinary assessment and failure of conservative or less invasive strategies. Amputation rarely results in pain resolution, as CRPS may

This article portrays the authors' experience with a complex lower limb bone and soft tissue defect, following chronic osteomyelitis and pathological fracture, which was managed by the multidisciplinary orthoplastic team. The decision for functional amputation versus limb salvage was deemed necessary, enhanced by the principles of "spare parts" in reconstructive microsurgery. This case describes the successful use of the osteocutaneous distal tibia turn-up fillet flap that allowed "lowering the level of the amputation" from a through knee to a below-knee amputation (BKA) to preserve the knee j

Despite modern conservative and surgical procedures, the number of amputations of the lower limb remains at a consistently high level. With the demographic changes and the consequent prevalence of atherosclerosis and diabetes mellitus, there is a steady increase of the risk factors that can lead to an amputation. The cause, which ultimately leads to the loss of the affected limb is therefore a symptom of the underlying disease. Primarily, the purpose of any medical treatment is the prevention of any amputation. If the preservation of the limb is not achievable, the surgical procedure follows.

BACKGROUND: Prosthetic replacement after amputation or loss of function of the upper extremity has gained therapeutic value over the last years. The control of upper arm prostheses has been refined by the use of selective nerve transfers, and the indication for prosthetic replacement has been expanded.
OBJECTIVES: Overview regarding surgical, therapeutic and prosthetic options in upper extremity amputations or their loss of function.
METHODS: Selective literature research including the authors' own experience in everyday clinical practice, as well as a review of medical records.
RESULTS: Se

The soft tissues of residual limb amputees are subject to large volume fluctuations over the course of a day. Volume fluctuations in residual limbs can lead to local pressure marks, causing discomfort, pain and rejection of prostheses. Existing methods for measuring interface stress encounter several limitations. A major problem is that the measurement instrumentation is applied in the sensitive interface between the prosthesis and residual limb. This paper presents the principle investigation of a non-intrusive technique to evaluate the fit of orthopaedic prosthesis sockets in transfemoral am

BACKGROUND/AIM OF THE STUDY: Nerve capping is a method of neuroma treatment or prevention that consists of the transplantation of a proximal nerve stump into an autograft or other material cap, after surgical removal of the neuroma or transection of the nerve. The aim was to reduce neuroma formation and symptoms by preventing neuronal adhesions and scar tissue. In this narrative literature review, we summarize the studies that have investigated the effectiveness of nerve capping for neuroma management to provide clarity and update the clinician's knowledge on the topic.
METHODS: A systematic

BACKGROUND: Thermal discomfort because of elevated residual limb skin temperatures and/or perspiration within the prosthetic socket is frequently reported among people with amputation.
OBJECTIVE: To evaluate the impact of the thermal conductivity characteristics of prosthetic liners on transtibial prosthesis heat dissipation, following postactivity rest.
STUDY DESIGN: Time-dependent heat transfer study in solids using finite element analysis.
METHODS: A three-dimensional model of the transtibial residual limb was developed by simplifying the geometry to tapered, layered cylinders. Four pros

BACKGROUND: At present, the quantifiable pressure distribution at the interface between the socket and stump is seldom applied in the design and fabrication of the socket.
OBJECTIVES: This study aimed to optimize the socket based on the interface pressure of residual limb-socket, thereby avoiding excessive local load on the residual limb, reducing the load on the pressure-sensitive (PS) regions and making the limb more evenly loaded.
METHODS: The residual limb was divided into the main load-bearing regions, the pressure-tolerant regions, and the PS regions according to the carrying capacity

BACKGROUND: Individuals with lower limb loss often wear a gel liner and enclosed socket for connecting to a terminal prosthetic device. Historically, a significant limitation to traditional liners and sockets is that they are thermal insulators, thereby trapping heat and moisture within, which can lead to numerous deleterious issues, including loss of suspension and residual limb skin problems, and, in turn, reductions in mobility, function, and overall quality of life. To mitigate these issues, new approaches are therefore needed to enhance the residual limb climate (e.g. breathability and ai

CASE: A 56-year-old man with a remote history of a left below-the-knee amputation (BKA) presented to the emergency department with an ipsilateral intertrochanteric fracture (IT). This case produced an operative challenge because commonly used intraoperative traction methods could not be applied to a patient with a BKA.
CONCLUSION: We describe a rarely utilized surgical technique of applying traction to an amputated extremity through an inverted boot on a fracture table during open reduction and internal fixation of an IT.

In pig production, piglets are tail docked at birth in order to prevent tail biting later in life. In order to examine the effects of tail docking and docking length on the formation of neuromas, we used 65 pigs and the following four treatments: intact tails (n=18); leaving 75% (n=17); leaving 50% (n=19); or leaving 25% (n=11) of the tail length on the pigs. The piglets were docked between day 2 and 4 after birth using a gas-heated apparatus, and were kept under conventional conditions until slaughter at 22 weeks of age, where tails were removed and examined macroscopically and histologically
