[Left main coronary artery disease].

There are many causes of left main coronary artery disease, the first of which is atherosclerosis. Other rarer causes may be observed, such as acute and chronic occlusions, spasm and primary and secondary dissection. The prevalence of stenosis of the left main coronary artery at coronary angiography is about 5%. The risk factors are the same as for coronary artery disease. The symptoms are angina, especially unstable angina. The diagnosis is suspected on the finding of an extremely positive exercise stress test, confirmed by coronary angiography. The results of the prospective large scale Vete

Virtual Reality-Guided Simulation for Percutaneous Coronary Intervention in a Patient with Anatomical Anomalies: A Case Report.

BACKGROUND Virtual reality (VR)-guided GC simulation for patients with anatomical anomalies using cardiac computed tomography (CT) has been recently reported. Rotational atherectomy (RA) for the left circumflex (LCX) ostium is challenging due to the tortuous anatomy, acute angulation, and variable vessel size compared to other lesions. The appropriate positioning and coaxiality of the guide catheter (GC) are key factors for safely performing RA. It would be beneficial if it could be simulated prior to percutaneous coronary intervention (PCI). CASE REPORT We treated a 55-year-old man with angin…

Unusual interventional treatment of a complex calcified coronary artery lesion in a child with Kawasaki disease: a case report.

BACKGROUND: Kawasaki disease (KD) is a medium vessel vasculitis occurring in children, as yet of undetermined aetiology. KD can lead to seve … ial ischaemia, a complex angioplasty guided by intracoronary imaging was performed using rotational atherectomy, a cutting balloon, and a high pressure balloon. The control coronary angiography performed 1 year later revealed two false aneurysm on the RCA at the angioplasty site, which were successfully treated with a covered stent. DISCUSSION: This case rep …

Successful retrieval of an entrapped Rotablator burr using 5 Fr guiding catheter.

BACKGROUND: Although burr entrapment is a rare complication of the Rotablator, it is extremely difficult to retrieve a fixedly entrapped burr without surgical procedure. CASE REPORT: An 84-year-old male with effort angina had heavily calcified coronary trees as well as severe stenosis in the mid LCx, and moderate stenosis in the proximal LCx, and in the LMT. We planned to perform rotational atherectomy in the LCx lesions. Using 7 Fr Q-curve guiding catheter and Rotawire floppy, we began to ablate using 1.5-mm burr at 200,000 rpm. Because the burr could not pass the proximal stenosis, we excha…

Coronary artery perforations in the contemporary interventional era.

BACKGROUND: Coronary perforations represent a serious complication of percutaneous coronary intervention (PCI). METHODS: We performed a ret … ilic wires, of coronary stenting in 11 (16.2%), of angioplasty alone in 6 (8.8%), and of rotational atherectomy in 8 (11.8%). The perforation was sealed with an angioplasty balloon alone in 16 patients (23.5%), and with stents in 14 patients (20.6%) (covered stents: 11.8% and noncovered stents: 8.8%). Emergency CABG was performed in 2 patients (2.9%). Five pa …

Drug-eluting stents: preventing restenosis.

The introduction of percutaneous transluminal coronary angioplasty has revolutionized the field of cardiology by providing patients with cor … n the stent: in-stent restenosis (ISR). Mechanical approaches, including directional and rotational atherectomy and systemic pharmacotherapy, have failed to demonstrate a reduction in ISR in randomized clinical trials. Intravascular brachytherapy is currently the only approved therapy for ISR, although this treatment has numerous unresolved questions and is n …

Localization of tissue transglutaminase in human carotid and coronary artery atherosclerosis: implications for plaque stability and progression.

Although atherosclerosis progresses in an indolent state for decades, the rupture of plaques creates acute ischemic syndromes that may culmi … ted co-localization with TG antigen. Transglutaminase activity was found in 6 of 14 coronary artery atherectomy samples. Cross-linking of TG substrates such as fibrinogen, fibronectin, vitronectin, collagen type I, and protease inhibitors stabilized the plaque. Furthermore, the activation of transforming growth factor-beta-1 by TG might be an additional mecha …

Identification of fibrillins as a major component of coronary atherosclerotic plaques.

Fibrillins are two distinct 350 kD RGD-containing glycoproteins found in microfibrils in the extracellular matrix. Fibrillin-containing microfibrils are required for elastic fiber formation and serve as an anchor for protein binding via β1 and/or αvβ3 receptors. Fibrillin-1 is causally linked to Marfan's syndrome, which is in part characterized by disruption of the aortic media. Fibrillin-2 is causally linked to congenital contractural arachnodactyly, which is rarely characterized by cardiovascular symptoms. We hypothesized that fibrillins might be present in coronary atherosclerotic plaques a

The Clinical Outcome of Drug-Coated Balloon Following Jetstream and Drug-Coated Balloon for Femoropopliteal Nodular Calcification Lesions: The Results of the Multicenter ORION Study.

BACKGROUND: Nodular calcification (NC) lesions in the femoropopliteal artery (FPA) are associated with poor outcomes after drug-coated balloon (DCB) angioplasty due to inadequate expansion and limited drug uptake. Jetstream atherectomy may enhance vessel preparation and improve DCB efficacy; however, the clinical outcome for FPA NC lesions remains unclear. AIMS: To compare the efficacy of DCB following Jetstream versus DCB alone for FPA NC lesions in a real-world setting. METHODS: In this retrospective, multicenter study, we enrolled 2213 consecutive patients (2294 de novo FPA lesions) who u…

Bail-out therapy in ST-segment elevation myocardial infarction due to calcified lesion causing stent underexpansion: Intravascular lithotripsy is in the lead.

A 52-year-old male was referred for an acute anterior ST-segment elevation myocardial infarction (STEMI). Coronary angiography revealed an acute left anterior descending artery occlusion. The patient was treated with a drug-eluting stent (DES). Despite long and repeated high-pressure inflations (>20 atm) of non-compliant balloons, OPN NCⓇ high-pressure balloon (SIS Medical AG; Frauenfeld, Switzerland), rotational atherectomy, and cutting balloon, there was a severe hourglass stent underexpansion caused by coronary calcification. Thus, intravascular lithotripsy (IVL) (Shockwave Medical, Fremont…

Prevalence and Clinical Significance of Layered Plaque in Patients With Stable Angina Pectoris - Evaluation With Histopathology and Optical Coherence Tomography.

BACKGROUND: Healed plaques are identified as a layered pattern with optical coherence tomography (OCT) imaging, but the exact relationship between healed plaques and the development of significant coronary stenosis in stable angina pectoris (SAP) is not fully understood. METHODS AND RESULTS: A retrospective clinincal study investigated the OCT characteristics of culprit lesions of SAP patients (n=205), and a prospective study examined the histopathological characteristics of layered plaque in directional coronary atherectomy (DCA) samples (42 samples from 18 SAP patients).
