Repeated Exposures to Air Travel-Relevant Hypobaria Induce Anxiety-Like Behavior and Alter Functional Connectivity and White Matter Integrity in a Ferret Model of Traumatic Brain Injury.

Under-vehicle blast (UVB) caused by landmine detonation induces a distinct traumatic brain injury (TBI) and can be accompanied by head impact. Injured soldiers often undergo multiple flights after injury to access medical care and return to duty. Previous work has shown that low air pressure during air travel (hypobaria [HB]) exacerbates neurological injury, but the effects of one or more HB exposures on chronic brain injury are unknown. We hypothesized that multiple HB exposures after TBI would result in worse outcomes than 0-1 exposures. Sedated male ferrets underwent UVB and controlled cort

Brain oxidative damage in murine models of neonatal hypoxia/ischemia and reoxygenation.

The brain is one of the main organs affected by hypoxia and reoxygenation in the neonatal period and one of the most vulnerable to oxidative stress. Hypoxia/ischemia and reoxygenation leads to impairment of neurogenesis, disruption of cortical migration, mitochondrial damage and neuroinflammation. The extent of the injury depends on the clinical manifestation in the affected regions. Preterm newborns are highly vulnerable, and they exhibit severe clinical manifestations such as intraventricular hemorrhage (IVH), retinopathy of prematurity (ROP) and diffuse white matter injury (DWMI) among othe

Alterations in Microstructure and Local Fiber Orientation of White Matter Are Associated with Outcome after Mild Traumatic Brain Injury.

Mild traumatic brain injury (mTBI) can have long-lasting consequences. We investigated white matter (WM) alterations at 6-12 months following mTBI using diffusion tensor imaging (DTI) and assessed if the alterations associate with outcome. Eighty-five patients with mTBI underwent diffusion-weighted magnetic resonance imaging (MRI) on average 8 months post-injury and patients' outcome was assessed at the time of imaging using the Glasgow Outcome Scale-Extended (GOS-E). Additionally, 30 age-matched patients with extracranial orthopedic injuries were used as control subjects. Voxel-wise analysis

Pathogenesis, neuroimaging and management in children with cerebral palsy born preterm.

With advances in obstetric and perinatal management, the incidence of intraventricular hemorrhage in premature infants has declined, while periventricular leukomalacia remains a significant concern. It is now known that brain injury in children born preterm also involves neuronal-axonal disease in supratentorial and infratentorial structures. The developing brain is especially vulnerable to white matter (WM) injury from 23 to 34 weeks gestation when blood vessels serving the periventricular WM are immature. Oligodendrocyte progenitors, which are beginning to form myelin during this time, are s

Radiation response of the central nervous system.

This report reviews the anatomical, pathophysiological, and clinical aspects of radiation injury to the central nervous system (CNS). Despite the lack of pathognomonic characteristics for CNS radiation lesions, demyelination and malacia are consistently the dominant morphological features of radiation myelopathy. In addition, cerebral atrophy is commonly observed in patients with neurological deficits related to chemotherapy and radiation, and neurocognitive deficits are associated with diffuse white matter changes. Clinical and experimental dose-response information have been evaluated and su

Brain dysfunction underlying prolonged post-concussive syndrome: A systematic review.

BACKGROUND: One out of 4 patients who sustains a mild traumatic brain injury (mTBI) experiences persistent complaints, despite the absence of structural brain damage on conventional neuroimaging. Susceptibility to develop post concussive symptoms (PCS) is thought to originate from occult brain dysfunction. However, the influence of such neural changes on the development of persistent PCS is poorly characterized. METHODS: In this article, we aim to integrate findings from longitudinal studies that investigated across the spectrum of neuroimaging modalities the changes within the first twelve m

Differential tractography as a track-based biomarker for neuronal injury.

Diffusion MRI tractography has been used to map the axonal structure of the human brain, but its ability to detect neuronal injury is yet to be explored. Here we report differential tractography, a new type of tractography that utilizes repeat MRI scans and a novel tracking strategy to map the exact segment of fiber pathways with a neuronal injury. We examined differential tractography on multiple sclerosis, Huntington's disease, amyotrophic lateral sclerosis, and epileptic patients. The results showed that the affected pathways shown by differential tractography matched well with the unique c

The evolution of T1-weighted lesion inpainting tools in patients with brain injury: A scoping review.

Focal brain lesions from Acquired Brain Injuries (ABIs) present as regions of abnormal signal intensity on T1-weighted Magnetic Resonance Imaging (MRI) scans. These can disrupt automated neuroimaging processing algorithms traditionally developed on and for healthy brains. Lesion filling (or inpainting) can replace lesioned image voxels with signal intensities approximating healthy tissue. This creates a 'lesion free' brain to use as input to the image processing algorithms thus aiming to reduce the presence of lesion induced errors. This scoping review provides a detailed overview of the avail

Neurovascular Interactions in the Neurologically Compromised Neonatal Brain.

Neurological brain injuries such as hypoxic ischaemic encephalopathy (HIE) and associated conditions such as seizures have been associated with poor developmental outcome in neonates. Our limited knowledge of the neurological and cerebrovascular processes underlying seizures limits their diagnosis and timely treatment. Diffuse optical tomography (DOT) provides haemodynamic information in the form of changes in concentration of de/oxygenated haemoglobin, which can improve our understanding of seizures and the relationship between neural and vascular processes. Using simultaneous EEG-DOT, we obs

Acute tissue tear hemorrhages of the brain: computed tomography and clinicopathological correlations.

Tissue tear hemorrhages (TTHs) are often seen on high-resolution computed tomographic scans after closed head injury. Generally, TTHs have been thought to be visible manifestations of more severe forms of diffuse axonal injury and thus portend a poor prognosis. Computed tomographic scans from 600 patients with head injuries were reviewed; 48 (8%) were found to have TTHs. The clinical spectrum of TTHs was characterized. No direct relationship could be established between either the presence or the number of TTHs and the severity and/or outcome from the head injury in this group, except that pat

Heart Failure Self-care Associated With Brain Injury in Executive Control Regions.

BACKGROUND: Inadequate self-care is linked to poor health outcomes in heart failure (HF). Self-care depends on decision-making abilities, but links between self-care and brain injury to executive decision-making regulatory areas (prefrontal cortices) are unclear. OBJECTIVE: We investigated the relationships between HF self-care and status of prefrontal cortices. METHODS: Magnetic resonance imaging-based diffusion tensor imaging was performed in 21 patients with HF (age, 53.8 ± 7.9 years; 15 men; left ventricular ejection fraction, 25.1% ± 6.1%), and self-care and executive function were meas

Impaired Glymphatic Function in Diffuse Axonal Injury: Evaluation Using the Diffusion Tensor Imaging Analysis Along the Perivascular Space (DTI-ALPS) Method.

PURPOSE: Glymphatic system impairment has been suggested in previous animal and human studies regarding traumatic brain injury (TBI). Diffuse axonal injury (DAI) is an important pathological feature of TBI and is frequently diagnosed in patients with moderate to severe TBI. This study evaluated the glymphatic system function in patients with DAI using diffusion tensor imaging analysis along the perivascular space (DTI-ALPS), a non-invasive technique. METHODS: A total of 162 patients with TBI, including 84 with DAI and 78 without DAI, underwent MRI with DTI within 6 months of the date of injur

Diffusion Kurtosis Imaging of Leptin Intervention in Early Hypoxic-Ischemic Brain Edema.

The role of leptin in neuroprotection has recently been recognized. However, there are few reports on the use of imaging methods to dynamically evaluate the neuroprotection role of leptin. Diffusion kurtosis imaging (DKI), which is a method used to measure non-Gaussian water diffusion, can reflect the real water diffusion in brain tissues. In this study, a newborn piglet model was used to dynamically evaluate the leptin intervention in early hypoxic-ischemic brain edema via DKI. Thirty-two Yorkshire newborn piglets were divided into three groups: the hypoxic-ischemic encephalopathy (HIE) group

Numerical investigation of the rider's head injury in typical single-electric self-balancing scooter accident scenarios.

As the use of electric self-balancing scooters (ESSs) increases, so does the number of related traffic accidents. Because of the special control method, mechanical structure and driving posture, ESSs are prone to various single-vehicle accidents, such as collisions with fixed obstacles and falls due to mechanical failures. In various ESS accident scenarios, the rider's head injury is the most frequent injury type. In this study, several typical single-ESS accident scenarios are reconstructed via computational methods, and the risk of riders' head/brain injury is assessed in depth using various

Hemorrhagic-ischemic cerebral injury in the preterm infant: current concepts.

PV-IVH and adjacent white matter injury remains a significant problem in the premature infant. The potential mechanisms contributing to injury are complex and involve factors related to blood flow and its regulation, as well as cellular mediators including cytokines, free radical formation, and excitotoxin release. Although a reduction in the occurrence of severe IVH can be achieved with indomethacin, it does translate into long-term neurodevelopmental benefit. This reinforces the concept of a more diffuse injury to brain in sick premature infants than is apparent from the appearance of curren

Evolution of brain injury after transient middle cerebral artery occlusion in neonatal rats.

BACKGROUND AND PURPOSE: Stroke in preterm and term babies is common and results in significant morbidity. The vulnerability and pathophysiological mechanisms of neonatal cerebral ischemia-reperfusion may differ from those in the mature cerebral nervous system because of the immaturity of many receptor systems and differences in metabolism in neonatal brain. This study details the neuropathological sequelae of reperfusion-induced brain injury after transient middle cerebral artery (MCA) occlusion in the postnatal day 7 (P7) rat. METHODS: P7 rats were subjected to 3 hours of MCA occlusion follo
