The anatomical basis of complete atrioventricular block in cats with hypertrophic cardiomyopathy.

The cardiac conduction system was examined histologically in 13 feline cases of hypertrophic cardiomyopathy (HCM) with complete atrioventricular (AV) block. Marked degeneration and fibrous replacement of the AV conduction system were consistently observed in the combined regions of the branching portion of the AV bundle and the upper portion of the left bundle branch. These changes were associated with extensive fibrosis of the central fibrous body and endocardial and myocardial fibrosis in the upper border of the ventricular septum. Chondrometaplastic lesions or osseous lesions, or both, pres

Catheter ablation of accessory atrioventricular connection between right atrial appendage to right ventricle: a case report.

A 24-year-old woman had experienced frequent attacks of orthodromic AV reciprocating tachycardia. The polarity of the delta waves suggested a right anterior or anterolateral accessory pathway. After ablation at the tricuspid annulus was unsuccessful, earliest retrograde atrial activation was recorded on the floor of the right atrial appendage, 2 cm above the tricuspid ring. Application of radiofrequency energy at this site abolished accessory pathway conduction. This unusual accessory pathway, located between the floor of the right atrial appendage and the right ventricle, was amenable to radi

Radiofrequency catheter ablation and modulation of atrioventricular conduction in patients with atrial fibrillation.

We attempted radiofrequency ablation of the AV junction with a sequential right- and left-sided approach in 78 patients affected by severely symptomatic, drug refractory atrial fibrillation. Stable third-degree AV block was obtained in 99% of cases and, after 3 months, persisted in 92% of cases. Single session, stepwise, radiofrequency modulation of the AV node was attempted in 13 patients with paroxysmal atrial fibrillation. During sinus rhythm, ablation of the slow and fast AV node pathways was performed in order to increase the nodal refractory period or to slow conduction. Clinically succe

Atrioventricular nodal reentry: evidence supporting an intranodal location.

The exact site of the reentrant circuit in AV nodal reentry remains controversial. While recent ablative techniques have yielded information, the interpretation of which suggests that the atrium is required, other explanations for these interpretations are available. Prior pathophysiological studies with three-dimensional reconstruction of the node suggest that it is a highly anisotropic structure and extends through Koch's Triangle. Data from humans suggesting the atria are not necessary include the presence of AV dissociation during supraventricular tachycardia (SVT), depolarization of atria

Lidocaine-induced second-degree mobitz type II heart block.

Lidocaine-induced atrioventricular (AV) conduction disturbances are an ill-defined phenomenon. Electrophysiological studies in both animal and human subjects have shown that therapeutic doses of lidocaine have no significant effect on AV nodal and His-Purkinje conduction time in the presence of intact AV conduction. Sporadic reports of accelerated AV conduction or complete heart block following lidocaine administration have been published. One case of Mobitz type II heart block has been reported in a patient with a prolonged QTc interval (0.61 sec) who was also receiving prenylamine. Electroph

[Usefulness of electrophysiologic studies. Report of 224 cases].

Two hundred and twenty four electrophysiologic studies were performed in 194 patients during a two-year period. One hundred and six (54.6%) had tachyarrhythmias: 21 had atrial tachycardia, 28 had the Wolff-Parkinson-White syndrome. Of 51 patients with a normal electrocardiogram a concealed left Kent bundle was observed in 21 cases and only one concealed right Kent bundle; functional James fibers were seen in 7 patients and Mahaim fibers in two. In 15 patients A-V nodal reentry was diagnosed and functional ectopy was observed in 5 patients. The sick-sinus-syndrome was seen in 24 patients and at

[Recording and data processing of electrical signals of the specific atrioventricular conduction pathways in man].

Signals of the electrical activity of the specific atrioventricular conduction pathways were recorded with an unipolar lead to obtain an exact time reference. The amplifier used had special characteristics: high gain settings (up to 300,000), very low noise levels, and wide filter range (2 Hz - 1,600 Hz). The low amplitude of the signals under study, of the order of a microvolt, and the wide filter range of the amplifier necessitated placing the patient in a Faraday cage. The signals recorded on magnetic tape were treated by a system of analysis for signal treatment. The method of averaging wa

A histological survey of pre-excitation syndrome and related arrhythmias.

The scanty available data in histology of accessory AV pathways, namely James', Mahaim's and Kent's fibres, together with Paladino's bundle, defects in the fibrous annuli and "Ring tissue", were considered for their bearing on old and new pathophysiological questions about ventricular pre-excitation and complicating tachy-arrhythmias. The histological findings in nine hearts allowed for a realistic and synthetic schematization of all the main types of anomalous AV bypasses, which have been emphasized in discussing the subject. The inadequacy of basic knowledge concerning the anatomoclinical si

His-Purkinje conduction system pacing and atrioventricular node ablation in treatment of persistent atrial fibrillation refractory to multiple ablation procedures: A case report.

In patients with symptomatic atrial fibrillation refractory to optimal medical therapy, atrioventricular node ablation followed by permanent pacemaker implantation is an effective treatment option. A 66-year-old woman with symptomatic persistent atrial fibrillation refractory to multiple ablation procedures was referred to our institution. After optimal drug therapy, the patient still had obvious symptoms. Sequential His-Purkinje conduction system pacing and atrioventricular node ablation were performed. Left bundle branch pacing was used as a backup pacing method if thresholds of His bundle p

Comparative Left Bundle Branch Block Correction With Physiological Pacing-An Intrapatient Electrocardiographic Study.

INTRODUCTION: Several studies have compared His bundle pacing (HBP) and left bundle branch area pacing (LBBAP) regarding paced QRS duration and clinical outcomes. This study compared acute intraprocedural depolarization and repolarization electrocardiographic changes between corrective HBP and LBBAP in the same patient with baseline left bundle branch block and reduced left ventricular ejection fraction. MATERIAL AND METHODS: Thirty-six patients with successful corrective LBBAP for cardiac resynchronization therapy (CRT), in whom corrective HBP was also demonstrated, were retrospectively revi

Septal Intramyocardial Purkinje Network: A Potential New Mechanism Explaining Left Bundle Branch Area Pacing Physiology.

BACKGROUND: Despite the growing use of left bundle branch area pacing (LBBAP) to deliver conduction system pacing, the mechanism underlying the narrow QRS interval conferred by this pacing modality remains unclear. OBJECTIVES: This study aimed to evaluate the mechanism that provides a most plausible explanation of LBBAP physiology. METHODS: A cohort of 13 patients who had surface electrocardiographic (ECG) or intracardiac recording features not explainable by either selective or nonselective LBBAP were evaluated. Unique ECG patterns and intracardiac recordings over the right interventricular

[Left bundle branch (area) pacing: lead positioning and implant criteria-step for step].

Left bundle branch area pacing is currently the most common form of physiological pacing prior to His bundle pacing. It is intended to prevent or correct the development of pacemaker-induced cardiomyopathy and is being used more and more frequently. In order to be able to perform this successfully, knowledge regarding the specific anatomy and radiological anatomy as well as the ECG criteria for left bundle branch pacing is required in addition to knowledge of the tools. In this article, the technical requirements and steps for successful implantation are summarized and pitfalls are highlighted

Durability of output-dependent QRS transition and left bundle branch capture in left bundle branch area pacing.

BACKGROUND: Although output-dependent QRS transition is a specific indicator that confirms left bundle branch (LBB) capture during left bundle branch area pacing (LBBAP), its durability remains unclear. OBJECTIVE: The purpose of this study was to evaluate the presence of output-dependent QRS transition and capture thresholds of the LBB and left ventricular septal myocardium immediately and up to 1 year after the LBBAP procedure. METHODS: We enrolled 129 patients with successful LBBAP who were available for 1-year follow-up postoperatively. Threshold testing was performed immediately after LB

Variability of the VA interval at tachycardia induction: a simple method to differentiate orthodromic reciprocating tachycardia from atypical atrioventricular nodal reentrant tachycardia.

BACKGROUND: The differential diagnosis between orthodromic atrioventricular reentry tachycardia (AVRT) and atypical AV nodal reentrant tachycardia (aAVNRT) is sometimes challenging. We hypothesize that aAVNRTs have more variability in the retrograde conduction time at tachycardia onset than AVRTs. METHODS: We aimed to assess the variability in retrograde conduction time at tachycardia onset in AVRT and aAVNRT and to propose a new diagnostic tool to differentiate these two arrhythmia mechanisms. We measured the VA interval of the first beats after tachycardia induction until it stabilized. The
