MicroRNAs in platelet function and cardiovascular disease.

Cardiovascular disease--a leading cause of morbidity and mortality among adults-is strongly influenced by platelet function through acute thrombotic and atherogenic mechanisms. Pathways that regulate platelet activity and lead to coronary occlusion are central to the pathogenesis of acute coronary syndromes. Platelet activation contributes to other thrombotic disorders and cardiovascular diseases, including stroke. Anucleate platelets are now understood to contain transcripts that might relate to other physiological or pathophysiological conditions, be released into the circulation, participat

Anomalous Origin of the Left Coronary Artery From the Right Pulmonary Artery of an Infant.

A female presented 2 weeks after birth with an unbalanced atrioventricular canal, double outlet right ventricle, mild pulmonary stenosis, and patent ductus arteriosus that eventually caused pulmonary over circulation. After pulmonary artery banding, she experienced myocardial ischemia, suggesting interference with coronary blood flow by the band that had been placed on the main pulmonary trunk. The band was removed and revised to bilateral branch pulmonary artery banding, and cardiac function improved. An anomalous left coronary artery from the underside of the right pulmonary artery was ident

[Ischemic heart disease with the anterior branches of the right coronal artery].

The coronary arteries shunting was performed in four elderly men with severe coronary heart disease, complicated by stagnant heart insufficiency and right ventriculus insufficiency (RVI). One patient could not be disconnected from the artificial blood circulation (ABC) apparatus till the venous shunting (VS) to atherosclerotically strictured right ventriculus anterior branch of right coronary artery was not accomplished. Other three patients, to whom VS to this branch was already conducted, were disconnected from the ABC without complications. In three patients echocardiography trusted RV diso

[Aberrant origin of the left coronary artery from the pulmonary artery as cause of ventricular fibrillation in a young woman].

An anomalous left coronary artery originating from the pulmonary artery is a rare, but frequently lethal congenital disorder. A wide range of symptoms due to heart failure and myocardial ischaemia may appear soon after birth. We describe a young woman who was admitted to the hospital after resuscitation for ventricular fibrillation. An anomalous origin of the left coronary artery from the pulmonary trunk was diagnosed. An echocardiographic evaluation in this adult patient illustrated the haemodynamic disturbances in the coronary circulation. The thallium scintigram showed reversible perfusion …

[Angiographic signs and pathophysiology of abnormal origin of the left coronary artery from the pulmonary trunk].

The results of intracardiac examination of 37 patients with anomalous origin of the left coronary artery from the pulmonary trunk were studied. On the basis of anatomo-angiographic++ comparisons the authors point out the characteristic features of coronary circulation and affection of the left ventricular myocardium which determine the clinical manifestations of its dysfunction. The functional condition of the left ventricle was appraised depending on the character of development of the intercoronary collaterals.

Coronary microvascular dysfunction in stable ischaemic heart disease (non-obstructive coronary artery disease and obstructive coronary artery disease).

Diffuse and focal epicardial coronary disease and coronary microvascular abnormalities may exist side-by-side. Identifying the contributions of each of these three players in the coronary circulation is a difficult task. Yet identifying coronary microvascular dysfunction (CMD) as an additional player in patients with coronary artery disease (CAD) may provide explanations of why symptoms may persist frequently following and why global coronary flow reserve may be more prognostically important than fractional flow reserve measured in a single vessel before percutaneous coronary intervention. Thi…

Intracoronary physiological indices for coronary microvascular dysfunction: from concept to clinical application.

The coronary microcirculation has long been considered a "black box" due to the difficulty of its direct assessment. Abnormalities in this compartment, termed coronary microvascular dysfunction (CMD), are now recognized as important contributors to myocardial ischemia and are increasingly implicated across a wide range of cardiovascular diseases, underscoring the need for precise diagnosis and targeted treatment. Over the past two decades, invasive physiological assessments (IPA) performed during coronary catheterization have been developed to evaluate coronary microcirculation and provide a u

The Application of Coronary Contrast Emptying Time in Diagnosing Coronary Slow Flow Phenomenon: A Serial Case Report.

The Coronary Slow Flow Phenomenon doesn't achieve as much attention as its counterpart Coronary Arterial Disease because it is considered a rather benign entity. But now it is proven that coronary slow flow phenomenon can also manifest as an acute coronary syndrome, myocardial ischemia, malignant arrhythmia, and even sudden cardiac death. This entity is usually diagnosed from coronary angiography study when a delayed coronary contrast filling time is found without the presence of significant epicardial narrowing of the related arteries. But, in our center's years of experience, we frequently fo

Coronary Microvascular Dysfunction: A Practical Approach to Diagnosis and Management.

PURPOSE OF REVIEW: The purpose of this review is to provide an overview of diagnostic and treatment considerations in patients with coronary microvascular dysfunction (CMD) in the absence of obstructive coronary artery disease (CAD). RECENT FINDINGS: The prevalence of obstructive CAD in unselected patient populations referred for evaluation of angina is less than 10%. A significant proportion of patients with angina and no obstructive CAD have CMD, a condition associated with impaired cardiovascular prognosis. Non-invasive and invasive evaluation of coronary microvascular function is feasible

Pathophysiology of Coronary Microvascular Dysfunction.

Ischemic heart disease (IHD) is commonly recognized as the consequence of coronary atherosclerosis and obstructive coronary artery disease (CAD). However, a significant number of patients may present angina or myocardial infarction even in the absence of any significant coronary artery stenosis and impairment of the coronary microcirculation has been increasingly implicated as a relevant cause of IHD. The term "coronary microvascular dysfunction" (CMD) encompasses several pathogenic mechanisms resulting in functional and/or structural changes in the coronary microcirculation and determining an

Reduced myocardial blood flow reserve in kidney transplant candidates may hamper risk stratification.

BACKGROUND: Vasodilator nuclear stress testing is frequently ordered for risk stratification prior to kidney transplantation. Since 82Rb-positron emission tomography-computed tomography can measure myocardial blood flow (MBF), the response to vasodilator stress can be verified rendering the results of the scan more reliable. METHODS: We reviewed the MBF response to dipyridamole infusion in 328 patients with end-stage kidney disease (ESKD) prior to transplant (188 hemodialysis-HD, 120 peritoneal dialysis-PD, and 20 pre-dialysis patients-CKD5) and in 100 controls with normal kidney function. Coronary microembolization and microvascular dysfunction.

Plaque erosion, fissuring or rupture occurs spontaneously or during coronary interventions. At some residual blood flow, the atherothrombotic debris is washed into the coronary microcirculation, causing physical obstruction, vasoconstriction, inflammation and ultimately microinfarction. Coronary microembolization also contributes to microvascular obstruction in reperfused acute myocardial infarction. Patients with microvascular obstruction after reperfused myocardial infarction have worse prognosis. Cardioprotective strategies to avoid acute coronary microembolization and rescue myocardium fro

Coronary Physiology Beyond Coronary Flow Reserve in Microvascular Angina: JACC State-of-the-Art Review.

Angina with no angiographic stenosis, commonly called "microvascular angina," encompasses a wide continuum of coronary pathophysiology in conflicting published reports. Comprehensive quantitative myocardial perfusion offers new insights beyond overly simplistic coronary flow reserve. Integrating regional absolute stress flow, relative stress flow, coronary flow reserve, and qualitative subendocardial perfusion gradient on tomograms of relative images, provides correct diagnosis, quantitative physiological classification, and potential treatment. Angina without angiographic stenosis is associat

[Non-ST-elevation myocardial infarction with normal coronary arteries, clinical features and coronary artery flow].

BACKGROUND: No assessment has been made up today concerning clinical features, coronary artery flow and mid-term prognosis between acute non-ST-elevation myocardial infarction (NSTEMI) patients without epicardial coronary disease and those with epicardial coronary artery stenosis > 50% of at least one vessel. METHODS: We evaluated consecutive NSTEMI patients who had undergone coronary angiography within the first 48 hours of infarction. We examined their age, sex, smoking habits, the incidence of diabetes, dyslipidemia, hypertension, and left ventricular ejection fraction. The coronary blood

[Intravenous obsidan-induced changes of coronary venous blood flow in patients with coronary heart disease in the presence of transient myocardial ischemia].

Intravenous obsidan was evaluated for its effects on the blood flow in the coronary sinus and the major cardiac vein in 23 coronary heart disease patients with hyperkinetic central hemodynamics. The use of the drug was found to be effective when the patients had had high baseline coronary venous blood flow. At the peak of cardiac pacing, the patients with low coronary sinus blood flow on obsidan showed decreased blood flow in the ischemic myocardial portions, developed signs of left ventricular failure and diminished anginal threshold. Those with moderate baseline blood flow on obsidan display
