Opening-wedge osteotomy, bone graft, and external fixation for correction of radius malunion.

A technique of radius opening-wedge osteotomy, bone graft, and external fixation for the treatment of symptomatic radius malunion is presented. It provides direct rigid fixation to the osteotomy components, thus maintaining the correction while allowing early wrist exercises. This technique has been effective for 7 patients in correcting deformities that averaged--20 degrees palmar tilt with radial shortening of 3.4 mm to a postoperative average palmar tilt of 5.3 degrees and -0.4 mm radial shortening. It is an alternative technique for the hand surgeon treating radius malunion and can be easi

Superior results of tibial rodding versus external fixation in grade 3B fractures.

From 1987 to 1993, 41 Grade 3B open tibial shaft fractures were treated with the unreamed tibial nail or an external fixator. The method of treatment was left to the choice of the operating surgeon. Three below knee amputations were performed; three patients died; and three were lost to followup. Thirty-two patients were observed until union or for at least 1 year. There were no significant statistical differences between the two groups with respect to fracture type, fracture location, age, gender, or accompanying injuries. The unreamed tibial nail group showed significantly better results reg

External fixation of the distal end of the radius: the same technique for different patient populations?

A series of 56 consecutive patients with 59 fractures of the distal end of the radius, treated with a unilateral Hoffmann external fixator between 1980 and 1988, is presented. Forty-seven of these patients underwent clinical re-examination at home. We could distinguish two different groups of patients: an older female group with an average age of 59.2 years and a younger male group with an average age of 40.1 years. The fractures in the women were caused by lower energy trauma than those in the men (P less than 0.05). The men had significantly more complex fractures (Frykman type VII or VIII)

Treatment of open tibial-shaft fractures. External fixation and secondary intramedullary nailing.

Between 1979 and 1986, sixteen of 369 open fractures of the tibial shaft were treated by external fixation followed by intramedullary nailing. These fractures comprised one Type-I, two Type-II, and thirteen Type-III injuries. This method of treatment was the original treatment plan in nine patients (56 per cent), for delayed union while the external fixator was still in place in four patients (25 per cent), for loss of reduction in a plaster cast in two patients (13 per cent), and for osteomyelitis and a segmental defect in one patient (6 per cent). The average duration of external fixation wa

Acute management of major trauma involving the foot and ankle with Hoffmann external fixation.

Massive trauma involving the foot was treated by external fixation in 33 cases and followed for 1 to 3 years. Amputations were performed in seven (21%) because of uncontrollable deep sepsis in two and massive injury in five. The other 26 (79%) had functionally acceptable feet at follow-up, but all had some stiffness and cosmetic deformity. Complications attributed to the Hoffman external fixation were limited to three pin tract infections. We recommend Hoffmann external fixation when it is necessary to: 1) stabilize major open fracture dislocations; 2) maintain length where bone is lost or ext

Leg lengthening with a new type of external fixator (a preliminary report on 24 cases).

A new type of external fixator bas been designed for absolute fixation and prevention of lateral migration of fragments of bone. The apparatus has been used for leg lengthening. The apparatus and the technique are described and results of the first 24 cases are presented. Complications were minimal. The technique proved to be simple to apply, and painless to the patient with his ambulation from the start. And less expense are involved because of a very short period of hospitalization.

External fixation of fractures of the distal radius: a randomized comparison of the Hoffman compact II non-bridging fixator and the Dynawrist fixator in 75 patients followed for 1 year.

BACKGROUND AND PURPOSE: External fixators allowing movement during fracture healing are commonly used for treatment of unstable distal radius fractures. The dynamic Dynawrist fixator with the distal pins in metacarpal bone may avoid fixation problems in comminuted fractures and may reduce the risk of nerve injury. We compared anatomical and functional outcome for the well-established Hoffmann compact II non-bridging fixator and for the Dynawrist fixator. PATIENTS AND METHODS: 75 patients with unstable distal radius fractures were randomized to treatment with either the Hoffman compact II fixa…

Comparative evaluation of pinning and cast fixation vs. external fixation after lateral closing-wedge osteotomy for cubitus varus in children.

BACKGROUND: Cubitus varus has been regarded as a poor functional and cosmetic consequence of supracondylar humerus fracture in children. The aim of this study was to assess the clinical and radiologic outcomes of cubitus varus treatments based on fixation methods: Kirschner (K)-wire and cast fixation or external fixation. METHODS: Forty consecutive patients with cubitus varus secondary to supracondylar fractures were retrospectively enrolled between October 2015 and December 2018. Following lateral closing-wedge osteotomy, those undergoing K-wire and cast fixation were included in group A (n

Surgical management of delayed irreducible Gartland III supracondylar fractures in children: open reduction and internal fixation versus external fixation.

BACKGROUND: Percutaneous pinning has been accepted as the "gold standard" for displaced supracondylar humeral fracture (SHF) in children. However, to secure an anatomic reduction and to reduce the rotated fragment, open reduction is often necessary when there is inadequate stabilization or a satisfactory reduction has failed to be achieved. The study aimed to compare the efficacy of the open reduction and bioabsorbable poly-d,l-lactic acid (PDLLA) pin fixation method with the closed reduction and lateral external fixation method in irreducible delayed Gartland type III SHF in children.
