[Knee tendinopathies and sports practice].

Knee tendinosis are frequent in sports practice. They are often in relation with sports gesture. Chronic patellar tendinosis are found so more often at the specialist's of jump (athletics and collective sports). The tendinopathies of the tensor of the fascia lata are more often diagnosed at the runner's, while the tendinopathies of the biceps or the harmstrings concern more frequently cyclists. The clinical examination allows to precise the sport concerned, to make the diagnosis, and to look for abnormalities of the morphotype or technical errors. Imaging must be envisaged in a second time (et…

[Early diagnosis of knee osteoarthritis].

Early diagnosis of knee osteoarthritis is evoked in two situations: fortuitous discovery on a normal X-ray or in front of symptoms. In the latest condition the diagnosis is usually easy. This diagnosis could be more difficult when the X-rays are normal or when the pain has a pseudo inflammatory rhythm. In those conditions, MRI is very useful, eliminating other differential diagnosis. Thus, it is rare to prescribe an arthrography coupled with TDM to detect the cartilage lesions. The early diagnosis of OA implies preventive therapies, mainly non-pharmacological ones. The discussion of surgery is…

Stiffness in total knee arthroplasty.

Stiffness is a relatively uncommon complication after total knee arthroplasty. It has been defined as a painful limitation in the range of movement (ROM). Its pathogenesis is still unclear even if some risk factors have been identified. Patient-related conditions may be difficult to treat. Preoperative ROM is the most important risk factor, but an association with diabetes, reflex sympathetic dystrophy, and general pathologies such as juvenile rheumatoid arthritis and ankylosing spondylitis has been demonstrated. Moreover, previous surgery may be an additional cause of an ROM limitation. Posto…

[Treatment of floating knee injury].

OBJECTIVE: To explore the clinical outcome of floating knee injury treated by open reduction and internal fixation. METHODS: The course of treatment of floating knee in 78 cases by open reduction and internal fixation were reviewed. There were 59 males and 19 females, aged from 17 to 58 years old, with an average age of 37.5 years. Intramedullary nail, pressure plate hollow screw, multi-function single side external fixation holder and other internal fixature were used in the operation. Early exercises were followed postoperatively. RESULTS: All patients were followed-up for 8 to 35 months (…

Thompson's quadricepsplasty for stiff knee.

BACKGROUND: Stiffness of the knee after trauma and/or surgery for femoral fractures is one of the most common complications and is difficult to treat. Stiffness in extension is more common and can be reduced by vigorous physiotherapy. If it does not improve then quadricepsplasty is indicated. The present study was undertaken to evaluate the results of Thompsons quadricepsplasty. MATERIALS AND METHODS: Twenty-two male patients (age range 20-45 years) with posttraumatic knee stiffness following distal femoral fractures underwent Thompson's quadricepsplasty where knee flexion range was less than…

Pseudogout after total knee arthroplasty.

We report a case of a patient who presented with signs and symptoms of acute septic arthritis of the knee 9 years after total knee arthroplasty. Thick white purulent fluid was aspirated from the knee. Microscopy of the fluid demonstrated calcium pyrophosphate dihydrate crystals, and no organisms were cultured. The patient made a rapid recovery after the single aspiration, rest, and nonsteroidal antiinflammatory drugs alone. Eight years after this episode, the patient remains asymptomatic. To the best of our knowledge, no case of calcium pyrophosphate dihydrate arthropathy after total knee arth…

[Surgical treatment of knee osteoarthritis].

Surgery for knee osteoarthritis is indicated if the clinical subjective symptoms in conjunction with radiological findings can no longer be adequately treated by conservative measures. Fundamentally, the treatment procedures are differentiated between joint preserving interventions (arthroscopy, knee revision, if necessary with meniscal debridement, partial synovectomy, osteophyte removal, cartilage surgery with microfracturing, autologous chrondrocyte transplantation, mosaic plasty or spacer implantation), corrective osteotomies near the knee joint and joint replacing procedures (knee endopro…

Wilbrand's knee: does it exist?

OBJECTIVE: In 1904, Hermann Wilbrand reported that nasal ON fibers form a loop into the contralateral ON, subsequently referred to as Wilbrand's knee. He had further theorized that a lesion affecting Wilbrand's knee would develop a distinct visual field defect characterized by an ipsilateral central scotoma with a contralateral superotemporal visual field defect. We present clinical evidence that disputes the existence of Wilbrand's knee. METHODS: A retrospective analysis of 3 patients whose ONs were divided at the ON-chiasm junction is presented. Two patients had an ONS meningioma and the ot…

Stress fractures around the knee.

Stress fractures of the lower extremities are common, especially in the younger athletic population. The current literature consists mainly a variety of case reports but is devoid of any sizeable series of knee stress fracture investigations. Diagnosing a stress fracture around the knee can be a challenge. The proximity of the stress fracture to the knee joint may lead the clinician to investigate intra-articular or other periarticular pathology. The differential diagnosis can be large, including bursitis, tendonitis, mechanical causes, insufficiency fracture, and tumor. A high index of suspic…

Patellectomy after total knee arthroplasty.

UNLABELLED: We retrospectively reviewed nine patients who had condylar TKA and subsequently had a patellectomy between 1969 and 2003. One patient was lost to followup, but the remaining eight patients were followed up for an average of 49 months after patellectomy. All patellectomies were done after comminuted patellar fractures at an average of 21 months (range, 4-88 months) after the initial arthroplasty. Knee scores after the initial arthroplasty were 83 points (range, 69-97 points) and 49 points (range, 10-100 points) for pain and function, respectively. Knee scores after patellectomy were…

Bandaging technique after knee replacement.

BACKGROUND: Firm bandaging of the knee following knee replacement may prevent bleeding into the joint by a tamponade effect. We studied the pressure required to achieve tamponade, and then clinically compared the use of a compression bandage with the use of a standard crêpe bandage, with or without a drain. METHOD: Transducers were used to measure the pressure achieved on the surface of the knee under different bandages, and within the knee following release of the tourniquet. We prospectively compared 3 series of 50 patients each: (1) with compression bandaging from toes to mid-thigh, (2) wi…

Management of osteoarthritic knee pain.

Osteoarthritis (OA) is one of the most common forms of arthritis seen in primary care practice. The pain associated with this condition is the chief complaint of most patients, prompting them to seek medical attention. Pain can originate from the synovial membrane, joint capsule, periarticular muscles and ligaments, and periosteum and subchondral bone, among other sources. Osteoarthritis is traditionally thought of as a noninflammatory type of arthritis; however, inflammatory mechanisms can be present. Therefore, the management of osteoarthritic pain involves nonpharmacologic modes of therapy

[Arthroscopic knee arthrodesis: 4 cases].

PURPOSE OF THE STUDY: Arthroscopic arthrodesis of the knee joint is an alternative to classical surgery, which retains a few exceptional indications. We report the first four cases of our experience, describing the technical modalities and indications. MATERIAL AND METHODS: The four patients had undergone multiple operations for severe trauma. All four had persistent severe pain with a very limited walking distance. Before the procedure, the IKS score ranged from 11 to 44 and the mean function score was 20 to 45 points. Arthroscopic arthrodesis was proposed after several consultations and too…

Soft tissue balancing: the knee.

During flexion of the normal knee, the tibia stays within a plane that is aligned anterior-to-posterior and passes near the center of the hip, knee, and ankle. To align the knee during total knee arthroplasty, the distal femoral cuts are aligned in 5 degrees to 7 degrees valgus to the long axis of the femur, and the tibial surface is cut perpendicular to the long axis of the tibia. To align the knee in the flexed position, the femoral surfaces are resected perpendicular to the anteroposterior axis of the femur. After alignment, sizing, and implant positioning are done, only tight ligaments are…

Anaesthesia for outpatient knee surgery.

Surgical procedures of the knee are increasingly common in outpatient centres. Advances in arthroscopy and other technologies allow more complex knee procedures to be performed on an outpatient basis. This chapter focuses on the application and advantages of peripheral nerve block regional anaesthesia in the anaesthetic management of knee surgery patients. Specific nerve blocks and local anaesthetics suitable for various knee procedures are discussed. The use of perineural catheters in the outpatient management of major knee surgery patients is also considered.

[Knee injuries in general practice].

Careful technique of examination of the knee joint allows to establish the diagnosis of the most frequent knee joint injuries already through simple clinical and radiological evaluation: Recurrent dislocation of the patella with the Apprehension sign; meniscal injuries with the test of McMurray, pain on the joint line and pain on passive hyperextension; the anterior cruciate ligament tear with Lachman's sign and the Pivot Shift test and posterior cruciate injury with the Sag sign. Further imaging techniques allow to establish a definite diagnosis, which has replaced in many cases the classical…

Arthrofibrosis after total knee arthroplasty.

Six total knee arthroplasties in five patients were revised because of persistent limited motion after the primary arthroplasty. All of the revised implants were of an appropriate size and not malpositioned. No cause of stiffness was identified other than soft tissue contracture. Four of the components were posterior cruciate retaining and two were posterior cruciate substituting. Heterotopic bone formation was observed in two knees before the revision surgery and five knees after the revision surgery. Arc of motion was increased from 36 degrees (range, 20 degrees-70 degrees) before revision s…

[Complex knee trauma--cartilage injuries].

Traumatic cartilage defects of the knee joint occur in an isolated way due to contusions or due to shear forces as a combined injury with ligamentous damage. They must be documented according to localisation, surface involved and depth. In the adult, only the fragment extending into subchondral bone can be refixed. Purely cartilagineous lesions heal spontaneously under partial repair with fibrocartilage; this can equally be surgically stimulated through surgical drillholes or impaction fractures with awls and pins. Since fibrocartilage is known to be mechanically insufficient and is often not …

MRI of posterior knee masses.

A wide variety of masses may be visualized in the posterior knee. Some are encountered incidentally on MRI obtained for seemingly unrelated symptoms; others are palpable and present for MRI characterization. We describe the appearance, significance and pathophysiology of some of the most common lesions found in the popliteal fossa as well as several of the less common entities we have encountered. Thorough understanding of the appearance and location of the more specific masses should augment precise diagnosis in a majority of cases. In turn, this will eliminate unnecessary additional investig…

Knee dislocations with vascular injuries.

Any knee that is dislocated or that has both anterior and posterior cruciates and one collateral ligament torn should be carefully evaluated for neurologic and vascular injury. Reduction and immobilization should be performed. If the pulses are anything but normal, arrangements should be made to proceed to the operating room immediately. A one-shot arteriogram can be obtained in the operating room, but proceeding directly with popliteal artery exploration is reasonable if the pulses have not returned to normal. If the pulses have returned to normal, an arteriogram should be obtained. After the…
