Petrositis is still a very real complication of otitis media. The most striking diagnostic feature is facial and/or retrobulbar pain, and it is the most consistent symptom. External rectus paralysis was present in only one of the four cases presented in this series. Treatment is surgical. In order to spare the cochlea and give adequate exposure to the apical cells, the middle fossa approach to the petrous apex is presented, to be used alone or in conjunction with mastoidectomy.

CONCLUSIONS: External ear canal exostosis is more prevalent in northern coastal groups than in the highlands, suggesting that ocean activities facilitate the appearance of exostosis. However, southern coastal groups exposed to colder ocean water have a lesser incidence of exostosis, possibly due to less duration of exposure. There was a high incidence of otitis media in all groups of native population in Chile. One coastal group had a higher incidence, presumably due to racial factors.

Patient-centric care has garnered the attention of the radiology community. The authors describe a patient-centric approach to iodinated contrast administration designed to optimize the diagnostic yield of contrast-enhanced CT while minimizing patient iodine load and exposure to ionizing radiation, thereby enhancing patient safety while providing reasonable diagnostic efficacy. Patient-centric CT hardware settings and contrast media administration are important considerations for clinical CT quality and safety.

Prevention of contrast-induced nephropathy is founded on minimizing the pathophysiologic consequences of contrast media (CM) interacting with a vulnerable kidney. In this article, the rationale for administering fluid (oral or intravenous) is discussed, and the clinical trials exploring different protocols are reviewed. A benefit from administration of fluids before CM exposure, which corrects volume depletion and increases urine output, can be expected. Forced diuresis without adequate volume replacement is deleterious.
