The tear (lacrimal) meniscus height in human eyes: a useful clinical measure or an unusable variable sign?

PURPOSE: To review notes and reports in the literature on assessments of human tear meniscus height (TMH) with the view to defining what the current state of knowledge is, and allow comparisons with data obtained on elderly individuals. METHODS: TMH data was obtained from 97 elderly individuals without significant eye disease. Photo-slit-lamp views, perpendicular to the lower marginal tear strip, were used to obtain close-up images by videography over approximately 30s. Repeated measures at the same location separated by a few seconds (time-dependent TMH measures, tTMH), or repeated measures …

Induction of osteoarthritis in the rat by surgical tear of the meniscus: Inhibition of joint damage by a matrix metalloproteinase inhibitor.

OBJECTIVE: Characterize a model of osteoarthritis (OA) induced by a surgically transecting the medial collateral ligament and meniscus. Evaluate the effectiveness of a matrix metalloproteinase (MMP) inhibitor in this model. METHODS: The medial collateral ligament of the right knee of rats was transected and a single full thickness cut was made through meniscus. Rats were sacrificed at various times after the surgery to assess the severity of gross cartilage damage using an image analyser and microscopically by histology. The effect of an MMP inhibitor in this model was assessed by administeri…

The arrow versus horizontal suture in arthroscopic meniscus repair. A prospective randomized study with arthroscopic evaluation.

In a prospectively randomized study including 68 patients, the results of inside-out horizontal meniscus suturing were compared to meniscus repair using the meniscus arrow. 96% of the patients underwent re-arthroscopy after 3-4 months. Only lesions in the red/red or red/white areas were included. Patients were treated with a hinged brace for 9 weeks. 30 patients had an isolated bucket-handle lesion. In 19 cases the repair was done in conjunction with an ACL reconstruction and in 19 cases the repair was performed in an ACL-insufficient knee. The two groups were comparable. Operating time in the…

The long term outcome of open total and partial meniscectomy related to the quantity and site of the meniscus removed.

Two hundred patients were randomly selected for either a partial or a total meniscectomy for a meniscal tear during open operation. They were followed for a median of 7.8 years after operation. After partial meniscectomy, posterior horn tears had the worst outcome, but this was only apparent when more than two-thirds of the meniscus had been removed. The amount of meniscal tissue excised was inversely related to the function of the knee, except with bucket-handle tears treated by partial meniscectomy. Patients with bucket-handle, anterior and posterior horn tears had similar functional results…

Function, post-traumatic sequelae and current concepts of management of knee meniscus injuries: a review article.

Long thought to be expendable or insignificant for function, the menisci may well be very important structures in the knee joint. Menisci are of critical importance from the standpoint of stability, shock absorption, load transmission, and lubrication, and also play a role in preventing synovial impingement, and in defining the endpoints of flexion and extension. In the wake of meniscectomy, certain predictable time-related events afflict the joint, i.e., instability and degenerative arthrosis. These eventualities are less pronounced following partial resection, and lend credence and support t

MRI evaluation of hypermobile lateral meniscus beyond distance measurements: the role of integrated popliteomeniscal fascicle assessment.

BACKGROUND: Hypermobile lateral meniscus (HLM) causes lateral knee pain and mechanical symptoms, but preoperative identification remains challenging because HLM is a dynamic abnormality that is not always captured on static magnetic resonance imaging (MRI). Previous MRI-based studies have mainly focused on linear measurements or qualitative popliteomeniscal fascicle (PMF) abnormalities. This study aimed to evaluate whether spatial MRI assessment beyond conventional linear distance measurements could help characterize arthroscopically confirmed HLM. We also aimed to determine whether MRI-derive…

Arthroscopic Lateral Meniscus Posterior Root Repair Using All-Suture Anchor and Modified Transtibial Pull-Out Technique.

Lateral meniscus posterior root tears lead to disruption of hoop tension, elevation of contact pressure, increased knee instability, and accelerated cartilage degeneration of articular cartilage. Anatomical repair is required to re-establish normal knee biomechanics and to slow the progression of osteoarthritis. A modified transtibial pull-out technique using an all-suture anchor is described. The tail suture is applied for root fixation, creating a suture ball on the meniscal surface, while a 2 mm tibial tunnel is created at the anatomic footprint to achieve refixation. This technique provide…

PEG-Based Hydrogels for Meniscus Replacement: Advancing Scaffold Fabrication Flexibility through a Customized Injection Molding Setup.

Meniscal injuries are one of the most frequent orthopedic injuries and often lead to joint degeneration and osteoarthritis if left untreated. Current meniscus prostheses are limited by inadequate mechanical properties and poor integration with native tissue, leading to high failure rates and limited long-term success. The field of meniscus replacement aims to develop scaffolds that mimic the native meniscus's complex structure and mechanical properties while promoting tissue regeneration and integration (when targeting a tissue-engineered meniscus). However, traditional fabrication methods, su…

Posterior Horn Meniscus Centroid Position Is Altered Soon After Noncontact ACL Injury in Males and Females.

Anterior cruciate ligament (ACL) injury and subsequent changes in the magnitude and distribution of contact stress about the articular surfaces of the knee are associated with post-traumatic osteoarthritis. Soon after ACL injury, changes in tibial articular cartilage thickness occur that can be explained, in part, by abnormal positioning of the tibia relative to the femur. However, little is known about the effects of ACL injury on the positions of the menisci. The purpose of this case-control study was to determine the effect of ACL injury on posterior horn meniscus centroid (PHMC) position r…

A stacking based deep learning framework integrating random search neural architecture search for meniscus tear diagnosis.

BACKGROUND: Accurate and rapid diagnosis of meniscal tears is crucial for effective management of sports-related injuries and degenerative knee disorders. Magnetic resonance imaging (MRI) is widely used for meniscus evaluation; however, manual interpretation is time-consuming and subject to inter-observer variability. Automated and reliable classification systems may therefore support clinical decision-making. PURPOSE: This study aims to develop a robust and interpretable deep learning framework for the automatic four-class classification of meniscal conditions using MRI images. METHODS: A t…

Arthroscopic Circumferential Lateral Meniscus Reconstruction Using Pull-Through Technique and Anterior Half Peroneal Longus Tendon Autograft.

Total meniscectomy for irreparable meniscal tears results in kinematics of the lower limb and early osteoarthritis of the knee. Some strategies for reconstruction of the meniscus include meniscus allograft transplantation and meniscal scaffold replacement. Although meniscus allograft transplantation is widely used, it has limitations such as the graft sources, high costs, and high risk of disease transmission. The research on meniscal scaffold replacement is still in the animal experimental stage, and there is a lack of large-scale clinical trial data. In this Technical Note, the lateral menis…

Popliteomeniscal fascicles tear with lateral meniscus instability: arthroscopic all-inside technique with two-year follow-up.

BACKGROUND: The popliteomeniscal fascicles (PMFs) connect the lateral meniscus to the popliteal hiatus and provide stability to the lateral meniscus. A high percentage of knees with acute and chronic anterior cruciate ligament (ACL), posterolateral corner injuries and/or hypermobile lateral meniscus (HLM) have concurrent damage to the PMFs. The present study evaluated the outcome of a two-year follow-up of an all-inside arthroscopic procedure to manage tears of the PMFs associated with lateral meniscus instability. METHODS: A total of 11 patients with a tear of the PMFs diagnosed clinically a…

Biomechanical effects of LCWHTO vs. MOWHTO on medial meniscus posterior root repair: a finite element analysis.

Whether isolated high tibial osteotomy (HTO) or HTO combined with medial meniscus posterior root (MMPR) repair is the optimal surgical treatment of MMPR tears remains controversial. This study compared the biomechanical effects of isolated HTO versus MMPR repair combined with medial open wedge HTO (MOWHTO) or lateral closed wedge HTO (LCWHTO) using finite element analysis (FEA). In this study, the MRI and CT data of the knee joint were used to create an FEA model. MOWHTO and LCWHTO simulations adjusted the mechanical axis to the Fujisawa point, with MMPR conditions simulated as intact, tear, o…

Seven tesla knee MRI T2*-mapping detects intrasubstance meniscus degeneration in patients with posterior root tears.

BACKGROUND: Medial meniscus root tears often lead to knee osteoarthritis. The extent of meniscal tissue changes beyond the localized root tear is unknown. PURPOSE: To evaluate if 7 Tesla 3D T2*-mapping can detect intrasubstance meniscal degeneration in patients with arthroscopically verified medial meniscus posterior root tears (MMPRTs), and assess if tissue changes extend beyond the immediate site of the posterior root tear detected on surface examination by arthroscopy. METHODS: In this prospective study we acquired 7 T knee MRIs from patients with MMPRTs and asymptomatic controls. Using a…

Bucket-Handle Meniscus Tear Management With Meniscectomy Versus Repair Correlates With Patient, Socioeconomic, and Hospital Factors.

INTRODUCTION: Bucket-handle meniscus tears are common knee injuries that are often treated surgically with meniscectomy or meniscal repair. Although clinical factors may influence the choice of one treatment approach over the other, the influence of patient, socioeconomic, and hospital factors remains poorly characterized. This study aimed to estimate the relative nationwide utilization of these two procedures and delineate a variety of factors that are associated with the selection of one treatment approach over the other. METHODS: Meniscal repair and meniscectomy procedures conducted for is…
