Hemobilia secondary to gallbladder polyposis is rare in children but has been reported in a few children with metachromatic leukodystrophy. We present a case with preoperative multidetector computed tomography (MDCT) diagnosis of massive hemobilia caused by gallbladder polyposis in a patient with metachromatic leukodystrophy. Our report highlights the importance of both awareness of the association of gallbladder polyposis with other syndromes such as metachromatic leukodystrophy as well as the possibility of this entity presenting with life-threatening bleeding.

Compression from median arcuate ligament was observed during multidetector 64-row computed tomography in a Caucasian 30-year-old female. The patient was referred for examination to exclude anatomical pathologies causing hypertension. The examination demonstrated that left renal artery, which had its origin in the chest (at the level of upper one-third of Th12), was compressed as it passed by median arcuate ligament of the diaphragm. In addition, aortic compression and kinked shape was also revealed.

Interrupted aortic arch is a rare congenital malformation of the aortic arch defined as a loss of luminal continuity between the ascending and descending portions of the aorta. In a simple interrupted aortic arch, only a ventricular septal defect and patent ductus arteriosus are observed. We present a rare complex form of type A interrupted aorta with an aortopulmonary window, an atrial septal defect, a ventricular septal defect, and a patent ductus arteriosus on multidetector computed tomography (MDCT) imaging.

Congenital abnormalities of coronary arteries are an uncommon cause of sudden cardiac death and are difficult to detect at coronary angiography. We describe two patients with acute coronary syndrome and non-occlusive coronary artery disease in which a 64-multidetector computed tomography (MDCT) coronary angiography showed the presence of a malignant coronary anomaly. Sixty-four-MDCT with the possibility of 3D reconstructions allows for easier diagnosis of coronary anomalies and provides essential details necessary for operative intervention.

Intercostal artery pseudoaneurysm is extremely rare, with only a few case reports in the literature. Presented in this case report is a patient who developed an intercostal artery pseudoaneurysm after thoracentesis for recurrent pleural effusion. Diagnosis traditionally required arteriography; however, as shown in this case, the resolution of current multidetector computed tomography scanners and the routine acquisition of isotropic data sets with 3-dimensional visualization enable identification and detailed characterization of small vascular lesions such as these.

Multidetector computed tomography (CT) allows for an accurate, noninvasive means of evaluating tracheal anatomy and pathology. The integration of information obtained from axial images, multiplanar reformats and 3-D rendering enables precise anatomical localization of tracheal pathology, provides detailed information on surrounding structures, aids in surgical planning, and allows for assessment of posttreatment response. Although bronchoscopy remains the gold standard in the diagnosis of tracheal pathology, the proper utilization of radiologic imaging can allow for improved patient care.

A rare case of bilateral coronary artery dissection with rupture of aortic valve commissure following type A aortic dissection is described. 64-slice multidetector computed tomography (MDCT) was able to demonstrate both this findings along with involvement of other neck vessels. TEE demonstrated the severity and mechanisms of aortic valve damage and assisted the surgeon in valve repair. MDCT has played an invaluable role in the diagnosis of the abnormal details of such life-threatening vascular complications.

A single coronary artery is a rare and potentially serious anomaly. We present the case of a 54-year-old woman with chest pain predominantly at rest. A single coronary artery was demonstrated by 64-row multidetector computed tomography (CT) imaging, which is useful for the diagnosis and classification of coronary anomaly. In our case, no coronary artery stenosis was demonstrated on CT angiographic images, and totally occlusive coronary spasm was confirmed by intracoronary infusion of acetylcholine.

Echinococcal cysts located inside the pulmonary artery are extremely rare. The cause is usually rupture of intracardiac cysts or, more rarely, dissemination from a hepatic focus. The case of a 39-year-old patient with multiple hydatid cysts in the right and left pulmonary arteries and left ventricle wall is reported. The patient had undergone surgery for a hepatic hydatid cyst 10 years ago. Multidetector computed tomography angiography and magnetic resonance imaging were performed for the diagnostic evaluation.

A 79-year-old woman presented with a systolic murmur and dyspnea on exertion. Transthoracic echocardiography and multidetector-row computed tomography revealed a giant aneurysm in an abnormal vessel known as Vieussens' arterial ring (VAR). A pulmonary artery VAR fistula was also observed. Cardiac catheterization revealed a high pulmonary-to-systemic flow ratio (Qp/Qs = 2.1). We ligated the VAR, resected the aneurysm, and closed the fistula using a bovine pericardial patch. The patient's postoperative clinical course was uneventful.

Contrast-enhanced multidetector computed tomography (MDCT) has become a critical tool in the evaluation of the trauma patient. MDCT can quickly and accurately assess trauma patients for renal, ureteral, and bladder injuries. Moreover, CT guides clinical management triaging patients to those requiring discharge, observation, angioembolization, and surgery. Recognition of urinary tract trauma on initial scan acquisition should prompt delayed excretory phase imaging to identify urine leaks. Urethral and testicular trauma are imaged with retrograde urethrography and sonography, respectively.

Abdominal aortic aneurysm (AAA) rupture can occur in different ways, such as closed rupture into the retroperitoneum, open rupture into the peritoneal cavity, rupture into surrounding hollow structures, and chronic contained or sealed rupture. Here, we report an unusual case of spontaneous rupture of AAA into a renal cyst that presented with hematuria, abdominal pain and shock, and which was diagnosed with multidetector computed tomography. We also review the literature on unusual patterns of AAA rupture.

We describe a rare case of cyanosis following the Fontan operation with right-to-left shunting at the venous level, that is, an azygos venous to pulmonary venous connection. Few cases with partial anomalous pulmonary venous connection to azygos vein have been reported; however, there have been no reports describing the connection from azygos vein to pulmonary vein, which results in desaturation after Fontan operation. Multidetector-row computed tomography (MDCT) was a useful tool to depict this vascular malformation.

Polyarteritis nodosa is a rare disease, which is characterized by aneurysm formation and occlusion in the arteries of multiple systems. Due to its extensive involvement, whole-body evaluation is necessary for diagnosis and treatment monitoring. We report a case of polyarteritis nodosa using multidetector-row computed tomography (MDCT) as a "one-stop shop" modality for whole-body arterial evaluation. With precise protocol design, MDCT can be used as a reliable noninvasive modality providing comprehensive whole-body arterial evaluation.

In the study of inflammatory kidney diseases, the use of integrated imaging is considered the best diagnostic approach. However, given the high cost of radiological examinations, the radiologist is responsible for the choice of the best method to solve the question asked by the clinician.
