Assessing the utility of bulbocavernosus reflex for predicting urological outcome in children with spinal dysraphism surgery.

PURPOSE: Surgical untethering for spinal dysraphism carries the risk of neural damage affecting sphincter control. Whilst anal sphincter motor-evoked potentials (MEP) are commonly used, they assess efferent pathways and do not directly reflect bladder innervation. The bulbocavernosus reflex (BCR) evaluates both afferent and efferent components of the sacral reflex arc, potentially serving as a better intraoperative predictor of urinary function. This study aimed to (1) assess the correlation between baseline BCR and preoperative urinary continence and (2) determine whether intraoperative BCR l

A prognostic model for long-term outcome in moderate-severe traumatic brain injury using multimodal neuromonitoring: a retrospective cohort analysis.

BACKGROUND: Moderate-to-severe traumatic brain injury (msTBI) remains a leading cause of long-term disability and mortality. While multimodal neuromonitoring (MMM) provides real-time insights into cerebral pathophysiology, its integration into prognostic models for long-term outcomes requires further validation. OBJECTIVE: To develop and evaluate a prognostic model for 6-month neurological outcomes in msTBI patients using acute-phase multimodal monitoring parameters, clinical variables, and comorbidities. METHODS: We conducted a retrospective cohort study of 143 adult patients with msTBI [Gl

ML-STIM: Machine Learning for SubThalamic nucleus Intraoperative Mapping.

Objective.Deep Brain Stimulation (DBS) of the SubThalamic Nucleus (STN) is effective in alleviating motor symptoms in medication-refractory patients with Parkinson's Disease (PD). Intraoperative identification of the STN relies on MicroElectrode Recordings (MERs), typically analyzed by trained operators. However, this approach is time-consuming and subject to variability. For this reason, this study proposes ML-STIM (Machine Learning for SubThalamic nucleus Intraoperative Mapping), a ML pipeline designed to automate STN classification from MERs, ensuring high accuracy and real-time performance

Association between intraoperative electroencephalograph complexity index and postoperative delirium in elderly patients undergoing orthopedic surgery: a prospective cohort study.

PURPOSE: The primary method for predicting POD (postoperative confusion) relies on the analysis of clinical features. Brain activity complexity is a promising factor associated with the state of consciousness. The aim of this study was to investigate the role of EEG (electroencephalography) complexity changes in predicting POD in elderly patients undergoing orthopedic surgery. METHODS: From January 2024 to August 2024, 289 elderly patients undergoing orthopedic surgery were recruited at the Second Affiliated Hospital of Nanjing Medical University. Intraoperative EEG data from patients were co

Experience and safety of intraoperative Neuropixels: a case series of 56 patients.

OBJECTIVE: The Neuropixels probe, a high-density silicon microelectrode array, has been a transformative tool for extracellular recording of large numbers of single neurons across animal models. Traditional tungsten microelectrodes available for clinical neurophysiology typically only record 1-2 neurons at a given time. Human intraoperative Neuropixels recording increases access to single-neuron spiking by an order of magnitude and is poised for rapid adoption. The authors' objective was to determine the safety and yield of intraoperative Neuropixels for large-scale simultaneous neuronal recor

Brainstem Auditory Evoked Potentials to Guide Minimally Invasive Decompression in Chiari 1.5: Technical Case Report.

Chiari malformation types 1 and 1.5 can be treated with posterior fossa decompression, though surgical techniques vary considerably, with more aggressive approaches often considered for type 1.5. Given this variability, an objective intraoperative marker of adequate decompression would support more tailored surgery. While brainstem auditory evoked potentials (BAEPs) have been explored in pediatric populations, their utility in adults remains unstudied. We present a 26-year-old female with Chiari 1.5 and symptoms including migraines, visual disturbances, balance issues, and right-hand clumsines

Urinary and sexual dysfunction after rectal cancer surgery: A surgical challenge.

This manuscript focused on the surgical challenge of urinary and sexual dysfunction after rectal cancer surgery based on the interesting results demonstrated by the observational study of Chen et al, which was published in the World Journal of Gastrointestinal Surgery. Urinary dysfunction occurs in one-third of patients treated for rectal cancer. Surgical nerve damage is the main cause of urinary dysfunction. Radiotherapy seems to exacerbate sexual dysfunction. The role of Denonvilliers' fascia preservation vs resection when performing total mesorectal excision (TME), the impact of robotic and

Pausing propofol during neurosurgery to record intraoperative electrocorticography is feasible;10 years of clinical experience.

OBJECTIVE: Intraoperative electrocorticography (ioECoG) during neurosurgery is influenced by anesthetics. In our center we stop the propofol to enable interpretation of ioECoG. We reported our clinical experience and evaluated awareness and hemodynamic changes during the propofol-free periods (PFP). METHODS: We retrospectively included surgeries with paused propofol administration to record ioECoG (period: 2008-2019). Clinical reports were screened for symptoms of awareness. We compared mean arterial blood pressure (MAP; mmHg) and heart rate (HR;bpm) during PFP to baseline (ten minutes preced

Risk predictive score and cord morphology classification for intraoperative neuromonitoring alerts in kyphosis surgery.

BACKGROUND: Intraoperative neuromonitoring (IONM) alert is one of the worrying events of kyphosis corrective surgery, which can result in a postoperative neurological deficit. To our knowledge, there is no risk prediction score to predict such events in patients undergoing kyphosis surgery. PURPOSE: To develop a new preoperative MRI-based cord morphology classification (CMC) and risk prediction score for predicting IONM alerts in patients with kyphotic deformity. STUDY DESIGN: Retrospective analysis of prospectively collected data. PATIENT SAMPLE: About 114 patients undergoing surgical corr

Neuroapraxia of Trigeminal Nerve Controlled by Neuromonitoring During Microvascular Decompression in Multiple Sclerosis Patients Affected by Drug-Resistant Trigeminal Neuralgia Recurrent After Previous Operations.

BACKGROUND AND IMPORTANCE: Trigeminal neuralgia (TN) can complicate the clinical course of multiple sclerosis (MS) and can be very difficult to treat. Usually, these patients experience multiple recurrences after surgical procedures with a poor overall outcome. To the best of our knowledge, we report the first 3 cases of drug-resistant MS-related TN recurrent after previous operations in which intraoperative neuromonitoring controlled neuroapraxia of trigeminal nerve was performed. We describe the surgical technique, report the clinical outcomes of patients, and review the pertinent literature

Intraoperative lateral rectus electromyographic recordings optimized by deep intraorbital needle electrodes.

OBJECTIVE: We demonstrate the advantages and safety of long, intraorbitally-placed needle electrodes, compared to standard-length subdermal electrodes, when recording lateral rectus electromyography (EMG) during intracranial surgeries. METHODS: Insulated 25 mm and uninsulated 13 mm needle electrodes, aimed at the lateral rectus muscle, were placed in parallel during 10 intracranial surgeries, examining spontaneous and stimulation-induced EMG activities. Postoperative complications in these patients were reviewed, alongside additional patients who underwent long electrode placement in the late

Transcranial Motor Evoked Potential Monitoring in a Patient With a Deep Brain Stimulator: A Case Report.

A major complication of surgical scoliosis correction is permanent injury of the spinal cord. Intraoperative neuromonitoring continually evaluates spinal cord function through monitoring sensory and corticospinal motor tracts. There is no literature or manufacturer recommendation on whether transcranial motor evoked potential (tcMEP) monitoring can be performed safely in the presence of a deep brain stimulator (DBS) system. A 17-year-old adolescent boy with severe neuromuscular scoliosis presented for a posterior spinal fusion. The patient suffered from generalized dystonia treated with a DBS

Contemporary analysis of phrenic nerve injuries following cryoballoon-based pulmonary vein isolation: A single-centre experience with the systematic use of compound motor action potential monitoring.

BACKGROUND: Phrenic nerve injury (PNI) remains one of the most frequent complications during cryoballoon-based pulmonary vein isolation (CB-PVI). Since its introduction in 2013, the use of compound motor action potential (CMAP) for the prevention of PNI during CB-PVI is increasing; however, systematic outcome data are sparse. METHODS: The CMAP technique was applied in conjunction with abdominal palpation during pacing manoeuvres (10 mV, 2 ms) from the superior vena cava for 388 consecutive patients undergoing CB-PVI between January 2015 and May 2017 at our tertiary arrhythmia centre. Cryoabla

Continuous Laryngeal Adductor Reflex Versus Intermittent Nerve Monitoring in Neck Endocrine Surgery.

OBJECTIVE: Intraoperative neuromonitoring (IONM) techniques aim to identify and potentially prevent nerve injury during surgeries. Prior studies into the efficacy of recurrent laryngeal nerve (RLN) IONM convey mixed results, with some claiming equivalence between IONM and no monitoring at all. The goal of the current study was to compare continuous RLN monitoring using the laryngeal adductor reflex (LAR) to intermittent RLN monitoring (intermittent IONM) to determine whether continuous monitoring reduces the incidence of intraoperative RLN injury during neck endocrine surgeries. METHODS: In t

Chiari 1 malformation and raised intracranial pressure.

BACKGROUND: The pathophysiology of Chiari 1 malformation (CM1) is inextricably related to intracranial pressure (ICP). The characteristic cerebellar tonsil herniation at the foramen magnum may either cause raised ICP by disturbing CSF flow (as observed in idiopathic CM1) or may itself be the effect of raised ICP (as observed in acquired CM1). Distinguishing between these two phenomena, therefore, is of paramount importance in successfully alleviating the symptoms of the condition and preventing serious complications. OBJECTIVES: In this article, we discuss the pathophysiology of raised ICP in

Intraoperative Assessment of Cortical Perfusion After Intracranial-To-Intracranial and Extracranial-To-Intracranial Bypass for Complex Cerebral Aneurysms Using Flow 800.

BACKGROUND: Revascularization strategies for complex cerebral aneurysms are largely based on subjective interpretation of flow demands, or indirect measures of perfusion in at-risk territories. Indocyanine Green -based flow analyses ((ICG-BFA); Flow 800, Carl Zeiss, Oberkochen, Germany) provide a real-time, semiquantitative measure of intraoperative cortical perfusion during cerebral bypass surgery for complex aneurysms. OBJECTIVE: To determine the utility of intraoperative ICG-BFA for assessing cortical perfusion in at-risk territories during cerebral bypass for complex aneurysms requiring v

CTA-guided outflow-targeted embolization of direct carotid-cavernous fistula.

This video presents a case of new-onset visual blurring, diplopia, and conjunctival injection after head injury. CTA of the brain revealed a direct carotid-cavernous fistula (dCCF) of the right side. Careful evaluation of CTA source images revealed that the fistula point was at the ventromedial aspect of the right cavernous internal carotid artery (ICA), about 3.6 × 3.6 mm2 in size, with 3 main outflow channels (2 intracranial and 1 extracranial) (CTA-guided concept). DSA of the brain also confirmed the diagnosis but was unable to locate the fistula point in a large-sized dCCF. Through a trans

Motor Evoked Potential Recordings from the Urethral Sphincter Muscles (USMEPs) during Spine Surgeries.

Bowel and bladder function are at risk during tumor resection of the conus, cauda equina, and nerve roots. This study demonstrates the ability to acquire transcranial electrical motor evoked potentials (TCeMEPs) from the urethral sphincter muscles (USMEPs) by utilizing a urethral catheter with an embedded electrode. A retrospective analysis of intraoperative neurophysiological monitoring (IONM) data from nine intradural tumors, four tethered cord releases, and two spinal stenosis procedures was performed (n = 15). The cohort included seven females and eight males (median age: 38.91 years). A c…
