Clinically significant changes in pain along the Pain Intensity Numerical Rating Scale in patients with chronic low back pain.

Low back pain (LBP) is the most common cause of chronic pain. Numerous clinical scales are available for evaluating pain, but their objective criteria in the management of LBP patients remain unclear. This study aimed to determine an objective cutoff value for a change in the Pain Intensity Numerical Rating Scale (ΔPI-NRS) three months after LBP treatment. Its utility was compared with changes in six commonly used clinical scales in LBP patients: Pain Disability Assessment Scale (PDAS), Pain Self-Efficacy Questionnaire (PSEC), Pain Catastrophizing Scale (PCS), Athens Insomnia Scale (AIS), Euro…

Pain Catastrophizing, rather than Vital Signs, Associated with Pain Intensity in Patients Presenting to the Emergency Department for Pain.

This study examined the relationships of self-reported pain intensity with vital signs, pain catastrophizing, and state anxiety in patients presenting to the emergency department (ED) for acute pain, exacerbations of chronic pain, or acute pain with concurrent chronic (combined) pain, comparing the pattern of relationships among these three pain groups. One hundred fifty-eight patients presenting to the ED for pain were recruited. Vital signs and self-reported pain intensity were obtained at triage, then participants completed self-report measures of pain catastrophizing, state anxiety, and de…

The ACTTION-American Pain Society Pain Taxonomy (AAPT): an evidence-based and multidimensional approach to classifying chronic pain conditions.

UNLABELLED: Current approaches to classification of chronic pain conditions suffer from the absence of a systematically implemented and evidence-based taxonomy. Moreover, existing diagnostic approaches typically fail to incorporate available knowledge regarding the biopsychosocial mechanisms contributing to pain conditions. To address these gaps, the Analgesic, Anesthetic, and Addiction Clinical Trial Translations Innovations Opportunities and Networks (ACTTION) public-private partnership with the U.S. Food and Drug Administration and the American Pain Society (APS) have joined together to dev…

Pain acceptance-based coping in complex regional pain syndrome Type I: daily relations with pain intensity, activity, and mood.

This study aimed to examine the temporal patterning of pain acceptance-based coping, activity, and mood in patients with complex regional pain syndrome Type I (CRPS-I), by using a daily diary method. A total of 30 patients with CRPS-I seeking treatment in a tertiary pain management center located in Seoul, Korea participated in the study. Multilevel random effects analyses indicated that (a) engagement in pain acceptance-based coping was significantly associated with lower same-day pain and negative mood and greater same-day activity and positive mood; (b) pain acceptance-based coping predicte…

Electromyographic recordings of low back pain subjects and non-pain controls in six different positions: effect of pain levels.

Surface electromyographic (EMG) activity recordings of bilateral paraspinal muscle tension were measured twice on 20 non-pain controls and on 46 low back pain subjects (21 individuals with intervertebral disk disorders and 25 subjects with unspecified musculoskeletal backache) during 6 positions: standing, bending from the waist, rising, sitting with back unsupported, sitting with back supported, and prone. Back pain subjects were measured during both low pain and high pain states. Results revealed a non-significant trend for all subjects, regardless of diagnosis, to have higher paraspinal mus…

A Painful Grip: A Case of Gonococcal Arthritis Masquerading as Wrist Pain.

Disseminated gonococcal infection (DGI) is a rare complication of Neisseria gonorrhoeae (N. gonorrhoeae), and it can present variably, often making the diagnosis challenging. While it typically affects the knee, wrist involvement, as seen in this case report, has also been reported. DGI most commonly presents either as an arthritis-dermatitis syndrome or as purulent arthritis resembling septic arthritis. The diagnosis can be challenging, particularly when symptoms are atypical and initial imaging is unrevealing. Blood cultures may be negative in many cases, and hence, providers must cons

Is malocclusion a predictor of pain in patients suffering from TMD pain?

BACKGROUND: Temporomandibular disorders (TMD) affect the masticatory muscles, temporomandibular joints (TMJs) and associated structures. The relationship between occlusion and TMD is a contentious issue in the dental field. OBJECTIVES: Although there is a strong argument against invasive and irreversible therapeutic TMD procedures, the TMD biopsychosocial model is still not accepted by some clinicians. Hence, this study aimed to verify whether malocclusions are related to TMD pain. MATERIAL AND METHODS: The study included 49 adult patients with one or multiple TMD diagnoses and without any o…

Pain in idiopathic scoliosis not associated with known genetic variants for pain.

INTRODUCTION: Back pain is common in idiopathic scoliosis. The aim of this study was to study known genetic variants associated with pain in individuals with idiopathic scoliosis. METHODS: We included 1442 individuals with juvenile or adolescent idiopathic scoliosis from Sweden and Denmark. Single nucleotide variants (SNV) genotyping was performed on 37 SNVs. Pain was assessed using 2 questionnaires. The mean pain domain score on the Scoliosis Research Society 22 revised questionnaire (SRS-22r) ranging between 1 (worst) and 5 (best) was dichotomized into a "back pain group" (score <4) and a "…

[Chronic pain as a disease: An overview of pain diagnoses in ICD-11].

Pain is a symptom that can be associated with dangerous diseases such as cancer. Hence, to avoid delay in diagnosis of an underlying serious condition, it is important to be thorough when assessing pain. However, all pains are not symptoms of an underlying disease. Instead, many chronic pain conditions can be viewed as diseases in their own right. Indeed, sometimes the pain is the disease - such is for instance the case for fibromyalgia, painless fibromyalgia being a contradiction in terms. The new classification of chronic pain conditions according to the eleventh version of International Cla…

Pain severity at emergency department discharge as a predictor for chronification of pain.

INTRODUCTION: Inadequate pain management remains a problem in the emergency department (ED) and might increase the risk of chronic pain. Previous studies suggested that pain intensity is associated with pain chronification in specific patient groups. This study aims to study the association between pain intensity {[verbal] numeric rating scale ([V]NRS) ≥ 7} at discharge from the ED and pain chronification in the general population. OBJECTIVE: To assess whether a high pain score at discharge from the ED increases the risk of chronic pain development. METHODS: Adults who visited the ED with pa…

Psychiatry and Pain Management: at the Intersection of Chronic Pain and Mental Health.

PURPOSE OF REVIEW: Chronic pain impacts millions of people in the USA. At the heart of the problem of chronic pain remains the complex psychosocial aspects associated with living with chronic pain. Given the overlap between chronic pain and mental health, a promising treatment approach is to improve how we integrate psychiatry into pain management. RECENT FINDINGS: Treatment of chronic pain and comorbid mental health issues requires a multidisciplinary approach. Advancements in how pain is understood, especially centralized pain, have helped inform both pharmacological and behavioral interven…

Concurrent validity of pain scales in individuals with myofascial pain and fibromyalgia.

OBJECTIVE: Clinicians and researchers often use the numeric pain rating scale (NPRS) and visual analog scale (VAS) to measure and track pain in individuals with myofascial pain syndrome (MPS) and fibromyalgia (FM). The VAS is often used as a reference standard in chronic pain research. To date, no studies have specifically measured the concurrent validity of the NPRS and VAS in these individuals. The purpose of this investigation was to determine the concurrent validity of the NPRS when compared to the reference standard VAS in patients with MPS and FM. METHODS: This investigation explored th…
