Clavicle fracture management: A comparison of a tertiary hospital and rural telehealth sites.

The clavicle is one of the most commonly fractured bones in the adult population. Management has traditionally been conservative, however more recent research has yielded higher non-union rates associated with non-operative management. This study aims to analyse the proportion of operative and non-operative treatment methods for clavicle fracture in a tertiary hospital facility and rural health sites accessed via telehealth, comparing patient outcome between these two methods of fracture management. Following ethical approval, a retrospective review was conducted of patients presenting with cl

Managing health changes for people with a learning disability in a residential care home setting.

BACKGROUND: People with learning disability (LD) have complex comorbidities that develop at an earlier age than the general population and with which they are now living longer. Identification, assessment and management of these conditions is important but challenging. AIM: To develop resources with care staff to enable them to recognise and manage changes and decline in the health of a person with a LD. METHODS: Two resources (PIP-LD and CIRC) were developed through undertaking a literature review; networking with experts; and collaborating with staff in the care homes for people with a LD.

Adverse effects of androgen deprivation therapy: defining the problem and promoting health among men with prostate cancer.

Androgen deprivation therapy (ADT) plays a central role in the management of men with locally advanced, recurrent, and metastatic prostate cancer. Because most men diagnosed with prostate cancer will die of something other than their cancer, treatment-related adverse effects are highly relevant to their long-term health. Benefits of ADT in each clinical setting must be weighed against ADT-related adverse effects. ADT is detrimental to several metabolic end points and to bone health. ADT has been prospectively shown to cause decreased lean muscle mass, increased fat mass, weight gain, increased

Current approaches to tuberculosis in the United States.

Tuberculosis is a major threat to global health, infecting a third of the world's population. In the United States, however, control of tuberculosis has been increasingly successful. Only 3.2% of the US population is estimated to have latent tuberculosis and there are only 11,000 cases annually of active disease. More than half the cases in this country occur in individuals born outside the United States. Human immunodeficiency virus coinfection is not a major factor in the United States, since only approximately 10% of cases are coinfected. Drug resistance is also uncommon in this country. Be

A Contemporary Review of Spontaneous Coronary Artery Dissection: CLINICAL CARE, CARDIAC REHABILITATION, AND FUTURE DIRECTIONS.

PURPOSE: Spontaneous coronary artery dissection (SCAD) has emerged as an important cause of acute coronary syndrome predominantly affecting women. Although the knowledge of SCAD has expanded over the past 2 decades, limited evidence exists to guide post-event management and prevention of future adverse events. This review aims to synthesize the current understanding of SCAD and outline considerations for exercise prescription and cardiac rehabilitation (CR) in this unique population. REVIEW METHODS: A comprehensive literature review was conducted, incorporating original research articles, sys

Symptom Burden Clustering in Chronic Kidney Disease Stage 5.

Patients with chronic kidney disease (CKD) Stage 5, experience multiple symptoms that negatively affect the health-related quality of life (HRQoL). This study examined the cluster of symptoms and their association with disease severity and comorbidities. The study sample included 123 patients with CKD Stage 5; 60 patients were in the dialysis group and 63 patients in the Conservative Kidney Management group. Symptom data were collected using the Spanish modified version of Palliative Care Outcome Scale-Symptoms (POS-S) Renal, a validated questionnaire to assess symptoms in this population. Mor

Cryptosporidium and Giardia in surface water: a case study from Michigan, USA to inform management of rural water systems.

Cryptosporidium and Giardia pose a threat to human health in rural environments where water supplies are commonly untreated and susceptible to contamination from agricultural animal waste/manure, animal wastewater, septic tank effluents and septage. Our goals for this paper are to: (1) explore the prevalence of these protozoan parasites, where they are found, in what quantities, and which genotypes are present; (2) examine relationships between disease and land use comparing human health risks between rural and urban environments; and (3) synthesize available information to gain a better under

Improving mental health in autistic young adults: a qualitative study exploring help-seeking barriers in UK primary care.

BACKGROUND: Autistic people are at increased risk of developing mental health problems. To reduce the negative impact of living with autism in a non-autistic world, efforts to improve take-up and access to care, and support in early years, which will typically start with a GP appointment, must be grounded in the accounts of autistic young adults. AIM: To explore how autistic young adults understand and manage mental health problems; and to consider help seeking as a focus. DESIGN AND SETTING: A cross-sectional, qualitative study. Autistic participants were purposively selected to represent a

User Needs Assessment of ASTHMAXcel Voice: A Novel Mobile Health App for Asthma Management.

Existing mobile health apps for asthma predominantly focus on patient education related to self-management. Through a federally-funded project in the U.S., we developed ASTHMAXcel Voice, a novel app that incorporates voice biomarkers and social determinants of health to enhance asthma self-management, shared decision-making, and facilitation of referrals for low-income populations through personalized digital data and collaborations with multiple health services and stakeholders. As part of the user-centered design effort, we conducted a series of user needs assessment focus groups with patien

Cardiovascular Disease in Australians Experiencing Homelessness.

Australia has one of the highest rates of homelessness in the world, at 498 per 100,000 people, and Australians experiencing homelessness (AEH) are a particularly vulnerable cohort, with a greater prevalence of cardiovascular disease, and poorer health outcomes, when compared to the general population. This narrative review explores how a combination of inadequately managed traditional and non-traditional cardiovascular risk factors, along with several personal, practical and relationship challenges with the health system, have created unique barriers in the diagnosis and management of cardiov

Global disability-adjusted life years burden of lower respiratory infections caused by Streptococcus pneumoniae, Staphylococcus aureus, and Klebsiella pneumoniae, 1990-2021: A systematic analysis of the global burden of disease study 2021.

BACKGROUND: Streptococcus pneumoniae, Staphylococcus aureus, and Klebsiella pneumoniae are the principal pathogens responsible for fatalities due to lower respiratory infections (LRIs), which pose a significant global health burden. During 2020 and 2021, the COVID-19 pandemic substantially altered the pathogenic landscape of LRIs. However, the specific impacts on S. pneumoniae, S. aureus, and K. pneumoniae have yet to be clearly determined. This study aims to analyze the changes in disability-adjusted life years (DALYs) associated with LRIs caused by these pathogens from 1990-2021, with a part

Control of sexually transmitted infections and prevention of HIV transmission: mending a fractured paradigm.

Control of sexually transmitted infections (STIs) is feasible, leads to improved sexual and reproductive health and contributes to preventing HIV transmission. The most advanced HIV epidemics have developed under conditions of poor STI control, particularly where ulcerative STIs were prevalent. Several countries that have successfully controlled STIs have documented stabilization or reversal of their HIV epidemics. STI control is a public health outcome measured by reduced incidence and prevalence. The means to achieve this include: (i) targeting and outreach to populations at greatest risk; (

Navigating Adulthood with Maple Syrup Urine Disease: Patient and Caregiver Perspectives on Healthcare Transition and Independent Living.

BACKGROUND: Medical advances in diagnosis and management have increased the life expectancy of many pediatric-onset rare diseases, including inherited metabolic disorders (IMDs). Historically fatal childhood conditions are now manageable chronic illnesses, with over 90% of individuals with IMD reaching adulthood. As more individuals with IMDs reach adulthood (age 18 years), there is a new imperative for health care transition planning that promotes developmentally-appropriate skills for medical self-management and independent living. Knowledge gaps on the long-term disease-related outcomes lim

Modified Brain Injury Guidelines for preinjury anticoagulation in traumatic brain injury: An opportunity to reduce health care resource utilization.

INTRODUCTION: The Brain Injury Guidelines (BIG) stratify patients by traumatic brain injury (TBI) severity to provide management recommendations to reduce health care resource burden but mandates that patients on anticoagulation (AC) are allocated to the most severe tertile (BIG 3). We sought to analyze TBI patients on AC therapy using a modified BIG model to determine if this population can offer further opportunity for safe reductions in health care resource utilization. METHODS: Patients 55 years or older on AC with traumatic intracranial hemorrhage (ICH) from two centers were retrospectiv

Using synthetic controls to estimate the population-level effects of Ontario's recently implemented overdose prevention sites and consumption and treatment services.

BACKGROUND: Between 2017 and 2020, Ontario implemented overdose prevention sites (OPS) and consumption and treatment services (CTS) in nine of its 34 public health units (PHU). We tested for the effect of booth-hours (spaces within OPS/CTSs for supervised consumption) on opioid-related health service use and mortality rates at the provincial- (aggregate) and PHU-level. METHODS: We used monthly rates of all opioid-related emergency department (ED) visits, hospitalizations, and deaths between January 2015 and March 2021 as our three outcomes. For each PHU that implemented OPS/CTSs, we created a

ISP25: health crisis management training for public health residents in France, a pilot and feasibility study.

INTRODUCTION: Public health residents play a critical role in crisis management but operate under administrative and population-level constraints that differ fundamentally from traditional clinical "bedside" Crisis Resource Management (CRM). There is a need to adapt existing training models to this specific institutional context. OBJECTIVE: This study aims to (i) describe a simulation-based training model specifically adapted to the French administrative context for public health residents; (ii) assess the feasibility and acceptability of this pilot program; and (iii) provide a translation an

Mental health apps and U.S. military veterans: Perceived importance and utilization of the National Center for Posttraumatic Stress Disorder app portfolio.

U.S. veterans have historically experienced more mental health concerns as compared to the general population, yet face a variety of barriers to accessing care. Evidence-based and accessible resources, such as mobile apps, are needed to respond to the unique needs of a diverse veteran population. The U.S. Department of Veterans Affairs (VA's) National Center for Posttraumatic Stress Disorder has created a one-of-a-kind portfolio of mental health apps to target the needs of veterans and support the self-management of common concerns related to posttraumatic stress disorder.
