Classical Rett syndrome in sisters: variability of clinical expression.

Familial cases of Rett syndrome (RS) are rare. No significant differences have been reported in the clinical courses of concordant monozygotic twins with RS. We present the variability of clinical expression in two Japanese sisters with classic RS. The younger sister, currently 6 years and 6 months old, never stood or walked alone, showed severe spasticity, growth retardation, and microcephaly and developed sleep-wake rhythm disturbance from age 4 years and seizures from age 5 years. The elder, currently 7 years and 9 months old, walked alone and had mild spasticity, no growth retardation, nor

Trichotillomania and non-epileptic seizures as sleep-related dissociative phenomena.

The occurrence of non-epileptic seizures (NES) and trichotillomania during sleep is rare. We describe the case of an adult woman with a personal history of childhood maltreatment and psychiatric morbidity (major depression, trichotillomania, and conversion disorder), who was referred to the sleep unit because of nocturnal hair-pulling and psychomotor agitation during sleep. An all-night PSG recording with audiovisual monitoring documented seven episodes of trichotillomania and one NES, all of which arose from unequivocal wakefulness. Improvement of nocturnal behaviors was observed after long-t

Sex-related differences within sleep-wake dynamics, cataplexy, and EEG fast-delta power in a narcolepsy mouse model.

Narcolepsy type 1 (NT1) is a sleep-wake disorder caused by selective loss of hypocretin (HCRT, also called orexin) neurons. Although the prevalence of NT1 is equal in men and women, sex differences in NT1 symptomatology have been reported in humans and other species. Yet, most preclinical studies fail to include females, resulting in gender bias within translational drug development. We used hcrt-tTA;TetO DTA mice (NT1 mice) that lose their HCRT neurons upon dietary doxycycline removal to examine in detail the effect of sex on NT1 symptoms and sleep-wake characteristics. We recorded 24-h elect

Sleep disturbances in clinic patients with functional bowel disorders.

OBJECTIVE: Sleep deprivation can lower visceral perception thresholds and nonregenerative sleep has been implicated as an etiological factor in chronic hyperalgesia syndromes. The aims of our study were to quantify the self-reported prevalence and type of sleep disturbances in patients with different functional bowel disorders (FBD) and to determine if this prevalence is related to involvement of the upper or lower gastrointestinal (GI) tract, perceived disease severity, or psychological comorbidity. METHODS: We enrolled 505 new FBD patients from an academic referral center specializing in fu

Evaluation and diagnosis of sleep disorders patients.

Using the biopsychosocial model, physicians can thoroughly assess patients with sleep disorders in the office setting. A careful sleep history, drug history, general medical assessment, and psychiatric evaluation along with an appraisal of the interplay between the patient's condition and his environment can provide all of the elements needed for diagnosis and treatment formulation. The main components of the sleep history include: defining the specific sleep problem, assessing the disorder's clinical course, differentiating between sleep disorders, evaluating the sleep-wakefulness patterns, q

Do periodic leg movements influence patients' perception of sleep quality?

BACKGROUND AND PURPOSE: Periodic leg movements in sleep (PLMS) are a common finding in various sleep disorders. Whether PLMS are an epiphenomenon or are causally related to the presence of sleep-wake disturbances is still being debated. We investigated the relationship of the occurrence of PLMS to patients' perception of sleep quality during a night of polysomnography in various sleep disorders. METHODS: The retrospective evaluation included PLMS recordings over two nights of 78 consecutive patients diagnosed with a restless legs syndrome, primary insomnia or insomnia associated with a psychi

Sleep-related rhythmic movements and rhythmic movement disorder beyond early childhood.

INTRODUCTION: Sleep-related rhythmic movements (SRRMs) are common in young children and become less prevalent with increasing age. When SRRMs significantly interfere with sleep and/or affect daytime functioning, potentially resulting in injury, rhythmic movement disorder (SRRMD) is diagnosed. OBJECTIVE: The aim of our study was to assess clinical comorbidities, types of SRRMs, sleep stage/wakefulness distribution during night, and age-dependence of these parameters. MATERIAL AND METHODS: In sum, 45 patients (age range 1-26 years, mean age 10.56 ± 6.4 years, 29 men) were clinically examined f

Orexin receptors: possible therapeutic targets for psychiatric disorders.

RATIONALE: Orexins, comprising orexin-A and orexin-B, are neuropeptides with extensive projections throughout the central nervous system. They are implicated in a variety of physiological processes through their receptors, orexin type 1 (OX1) and orexin type 2 (OX2) receptors. Among the physiological functions of orexins, their role in sleep/wake regulation has garnered significant attention. Consequently, three orexin receptor antagonists that block both OX1 and OX2 receptors (dual orexin receptor antagonist; DORA) are available on the market for the treatment of insomnia. Additionally, anoth

Wolfram syndrome 1 regulates sleep in dopamine receptor neurons by modulating calcium homeostasis.

Sleep disruptions are quite common in psychological disorders, but the underlying mechanism remains obscure. Wolfram syndrome 1 (WS1) is an autosomal recessive disease mainly characterized by diabetes insipidus/mellitus, neurodegeneration and psychological disorders. It is caused by loss-of function mutations of the WOLFRAM SYNDROME 1 (WFS1) gene, which encodes an endoplasmic reticulum (ER)-resident transmembrane protein. Heterozygous mutation carriers do not develop WS1 but exhibit 26-fold higher risk of having psychological disorders. Since WS1 patients display sleep abnormalities, we aimed

Real-World Use of Consumer Sleep Devices: A Rapid Review.

TOPIC IMPORTANCE: As awareness of sleep's role in health grows, consumer sleep technologies (CSTs) have gained wide use among both researchers and the public. These devices offer continuous, noninvasive sleep tracking in real-world environments. However, understanding their accuracy compared with that of polysomnography (PSG), the gold standard in sleep measurement, remains limited, particularly in free-living settings and clinical populations. Assessing CST performance and usability in these contexts is essential for guiding future adoption. REVIEW FINDINGS: This review identified 29 studies

Unrecognised sleep disturbances in patients with cirrhosis diagnosed with a portable electroencephalogram device.

OBJECTIVES: We aimed to compare sleep characteristics between patients with liver cirrhosis and healthy controls using a standardised protocol and portable electroencephalogram (EEG) devices. METHODS: We enrolled patients with early stage cirrhosis at low risk for sleep disorders (no apnoea, insomnia, alcohol use, pruritus or major portosystemic shunt; body mass index (BMI) ≤31 kg/m²). Using propensity score matching (age, sex, BMI), 18 patients with cirrhosis were compared with 18 healthy older adults from a 95-person cohort. Sleep was assessed at home using portable EEG devices measuring to

Diagnostic relevance of sleep complaints in anxiety and mood disorders.

Clinical assessments of sleep and subjective state upon waking were performed in normal controls and patients with generalised anxiety disorder, panic disorder, obsessive-compulsive disorder, primary dysthymia and major depression. Subjects were selected according to DSM-III-R criteria. As compared to normal controls, patients with generalised anxiety, dysthymia and major depression exhibit pervasive and intense complaints of insomnia, and no clear distinctions can be drawn among these groups. Patients with panic disorder do not differ from normal controls, whereas obsessive compulsive patient

Biological rhythm disturbances in mood disorders.

From earliest times, psychiatrists have described biological rhythm disturbances as characteristic of mood disorders. The present flourishing of circadian biology has revealed the molecular basis of 24-h rhythmicity driven by 'clock' genes, as well as the importance of zeitgebers (synchronisers). Winter depression was first modelled on regulation of animal behaviour by seasonal changes in daylength, and led to application of light as the first successful chronobiological treatment in psychiatry. Light therapy has great promise for many other disorders (e.g. sleep-wake cycle disturbances in Alz

Parasomnias: A Comprehensive Review.

Parasomnias are a group of sleep disorders characterized by abnormal, unpleasant motor verbal or behavioral events that occur during sleep or wake to sleep transitions. Parasomnias can occur during non-rapid eye movement (NREM) and rapid eye movement (REM) stages of sleep and are more commonly seen in children than the adult population. Parasomnias can be distressful for the patient and their bed partners and most of the time, these complaints are brought up by their bed partners because of the possible disruption in their quality of sleep. As clinicians, it is crucial to understand the charac

Sleep disorders in Behçet's disease, and their relationship with fatigue and quality of life.

Behçet's disease, a systemic vasculitis, can cause varying degrees of activity limitation, fatigue and quality of life impairment. To date, there have been no studies regarding sleep disturbance and its relationship with fatigue and life quality in Behçet's disease. We aimed to evaluate sleep disorders and polysomnographic parameters, and to determine their relationship with fatigue and quality of life in Behçet's disease. Fifty-one patients with Behçet's disease without any neurological involvement were interviewed regarding sleep disorders. Twenty-one subjects with no sleep complaints were i
